Functions and Key Activities of the MSD Safety Committee
Functions of safety committee and representatives Represent all research groups within MSD Stimulate leadership and staff participation in safety program Advise Division management and EH&S on safety and health matters Perform essential monitoring, educational, investigative and evaluative tasks Serve as contact point for EH&S matters in each research group Serve as conduit for bringing EH&S information back to research groups
Key Activities Recommend changes to existing safety rules or the development of new rules Recommend improvements in hazard identification and control measures Report and discuss unsafe conditions Review accidents, incidents and close calls in MSD and generate “Lessons

MSD Materials Safety (see handouts)
Improper Disposal of Waste Contaminated with Flammable Solvents (November 2006)
Student Injured in Peroxide Explosion (January 2007)
Post the Materials Safety in you labs, make sure they are reviewed and discussed during a group meeting
MSD Safety, 1-17-07-9
MSD Injuries and Incidents
Lab Ergonomic Injury Student developed nerve inflammation in arm:
– Extensive bolting and unbolting of vacuum system
– Fine manipulative work under microscope with no arm support
– Operation of ergonomically poor adjustment knobs
– Contributing factor–The postdoc that usually helped had left, so the work was done by one person rather than two.
MSD Safety, 1-17-07-10
MSD Injuries and Incidents
Injury Assembling Cabinet Administrator injured while installing shelf in
new cabinet in B 67 Unexpected assistance from co-worker caused
her to loose balance, minor back injury Lesson learned: Coordinate work with
colleagues
MSD Safety, 1-17-07-11
MSD Injuries and Incidents
Fluorine Exposure when Changing Cylinder Failed to install gasket when changing 1000 ppm
fluorine gas cylinder Opened valve without shutting gas cabinet door Smelled strong odor of fluorine No injury to students

Peer-to-peer training of Principal InvestigatorsReimplementation of fines for SAA problemsDisciplinary action for repeat safety violationsStrong EH&S component in P2R process for staff scientists
MSD Safety, 1-17-07-19
MESH–General Concerns and Observations
Finding StatusEH&S evaluation of “Line management authorized work” lacks depth
Project Hazard Guide being implemented to review all work
Training of PIs to recognize hazards inadequate
PI inspections not effective
Lab-specific training to each PI on the recognition of hazards EHS027 Monthly inspections by tech Quarterly tech/PI inspections
Management of SAAs is not effective Clear guidelines written; inspect more frequently; calendar system; fine repeat offenders
ISM Plan outdated To be updated by 2/1/06
MSD Safety, 1-17-07-20
MESH–Specific Concerns–Building 2
Bldg Finding2 Dewar not seismically restrained

MSD Safety, 1-17-07-24
Integrated Functional Appraisal
Reviewed compliance with formal work authorization documents (AHD, RWA, XA, BUA, LAS, SSA)
Conducted by John Seabury and others from EH&S Key finding
Some PIs are not taking AHDs seriously!
John Seabury presentation on MSD IFA findings
MSD Safety, 1-17-07-25
McCullum-Turner Review–Institutional
Threshold for “formal authorization” of work is too high
Some hazards are not adequately addressed by LBNL
policy, e.g., oxygen deficiency associated with cryogen,
magnetic fields
Limited safety accountability feedback mechanisms for
Post-Doctoral and Graduate Students
Wide variability in the planning/review of work between
Divisions
Lack of a safety program for vendors
MSD Safety, 1-17-07-26
McCullum-Turner Review–MSD
NCEM Really liked the direct supervision and training of users at NCEM Not so happy with management EH&S walkthroughs at NCEM Noted the lack of safety program for vendors at NCEM
TMF Liked the policy for EH&S with nanoparticles at TMF Liked the Foundry Safety Subcommittee; recommend chartering “Some evidence that work is being conducted in the Molecular
Foundry before specifying hazard controls” “Molecular Foundry does not have a policy addressing the
safety review of new or modified equipment”
MSD Safety, 1-17-07-27
Laser Safety from SAA Inspection
Lab 1 Student using 3B laser without required protective eye wear
Lab 2 Postdoc using class 4 laser system without required eye wear Postdoc not completed Laser Safety Training, Laser Eye Exam Not been added to or read AHD for lasers
Major failures to follow AHD and LBNL
laser safety requirements!
MSD Safety, 1-17-07-28
SAA Inspections & Management
In the December EH&S/DOE inspection, several SAAs were not following required practices, mostly due to labeling errors:
2-263A (Beeman/Haller) 2-358 (Orenstein) 67-4203 (Talapin) 67-4209 (Talapin/Shevchenko) 67-5201, 5209 (Zuckerman)
MSD Safety, 1-17-07-29
SAA Inspections & Management
Requirements for running an SAA: Label each container Completely fill out each label Date each label Place and update the SAA sign as needed Store only waste in the SAA Use secondary containment Dispose of containers that have been in use for 6 months or more Segregate solvents, halogenated solvents, acids, bases and other
incompatible materials Assign an SAA manager and backup manager Replace departed SAA managers No use of glass bottles > 1 quart for flammables
MSD Safety, 1-17-07-30
Internal Inspection–Molecular Foundry
Conducted by Paul Johnson and Rick Kelly in November One program had major problems relating to:
Waste management Chemical inventory Peroxidizable chemical management and testing Unauthorized use of toxic gas (ammonia) No labeling of squeeze bottles Improper chemical storage (no secondary containment)
MSD Safety, 1-17-07-31
