A completed Registration Form A signed Supplier Coordination Agreement Scheduling Coordinator Designation Form (if applicable) A completed EDI Trading Partner Agreement A completed Credit Application Form A completed Electronic Funds Transfer Authorization Agreement
(consolidated billing only) Proof that the Electricity Supplier has obtained a license from the
Commission Proof that the Electricity Supplier is signatory to applicable PJM
agreements either directly or through a Scheduling Coordinator
