The recent Patriot’s Day explosion inBoston looms large as I write to you

this year. As a trustee of Martha’sVineyard Hospital, I received a detailedupdate at a recent Partners HealthCaregeneral board meeting regarding the careprovided to victims of this terribletragedy at both the MassachusettsGeneral Hospital (MGH) and theBrigham and Women’s Hospital. Thecare the victims received was exemplaryand in keeping with the ranking of thetwo Partners Healthcare foundinghospitals, among the best in the world. Imust say that I am very proud of ourhospital’s affiliation with the MGH andour membership in Partners Healthcare.This past year we have continued to

build on our affiliation with MassGeneral in numerous ways, all with thegoal of providing world-class healthcareto residents and visitors right here onour Island. Our radiology departmenthas completed the integration of theMVH imaging system into the MGHradiology image archiving system,allowing for improved specialist image

interpretation and increased use of ourstate-of-the-art CT scan and MRImachines for sophisticated imaging tests.Our anesthesia service has expandedusing a combination of telemedicine andon-site specialists to provide painmanagement services. Our ability totake care of critically ill newborns andyoung children has been enhancedthrough a telemedicine initiativeinvolving neonatal and pediatricintensive care specialists from the MassGeneral Hospital for Children. And inwhat I think is the most excitingdevelopment in our affiliation to date,we have partnered with the MassGeneral Cancer Center to provideenhanced cancer services. These includeweekly visits by their cancer specialistsand a nurse practitioner specializing incancer care. This new service will allowour cancer patients to benefit from theworld-renowned expertise of the MGHwhile receiving their treatment locally.Since my last report to you, Martha’s

Vineyard Hospital added a number ofnew members to our Medical Staff

including Drs. Jon Pangia, MorrisRivera, and Tammy Thome to ourEmergency Department. This summerwe look forward to the arrival oforthopedic surgeon, Mark M. Scheffer,M.D. Dr. Scheffer, comes to us fromDartmouth-Hitchcock OrthopedicCenter and will bring a wealth ofexpertise in minimally invasive jointsurgery, including joint replacements.His arrival will allow us to continue to

expand on our vision of providingworld-class care locally.Our work to enhance our information

technology in our physician practicesgoes on. This year will see the additionof the “Patient Gateway” program,which will allow patients tocommunicate with their providerselectronically. On the hospital side, weare well underway with theimplementation of a new IT system,which will do away with paper alltogether while enhancing patient carequality and patient safety.None of the exciting changes described

above would have been possible withoutthe generous support of our patients andour donors. On behalf of the medicalstaff, I would like to thank you forembracing our vision as we continue tomake Martha’s Vineyard Hospital a placewhere you will get the best of care.

Island community will always pose hugefinancial challenges, and in leading thisinstitution we will always need to be alertfor ways to contain costs and improve ourefficiency. But with our new Hospitalbuilding now in its fourth year of service,and with our membership in the PartnersHealthCare network now in its seventhyear, the focus has shifted. Quality of careis firmly in the foreground at Martha’sVineyard Hospital – as it should be – andit’s in this arena that the headlines arebeing made:

New Oncology and HematologyThe Hospital’s new oncology and

hematology services, offered in a suite ofrooms newly created in the former ERspace of our 1972 building, represent themost dramatic expansion of locally-available health care here in decades. Ateam of clinicians from MassachusettsGeneral Hospital is providing servicesthat will enable scores of Island patientsto receive world-class cancer care close tohome, without the expense and stress oftrips to Boston.

Critical Care ServicesDrawing on telemedicine technology

and the expertise of specialists at MassGeneral, Martha’s Vineyard Hospital hasalso expanded its critical care services,enabling dozens of patients who oncewould have been airlifted to Boston toreceive top-notch care right here at home.Regular pacemaker clinics are now being

held at Martha’s Vineyard Hospital, andnew pain clinic services are sparingIslanders the need to travel for theirmedical care.

Orthopedic and Sports SurgeonAfter a national recruiting effort, the

Hospital announced this spring that Dr.Mark M. Scheffer, orthopedic and sportssurgeon, is joining the medical staff inJuly. Dr. Scheffer, who brings a broadexpertise in joint replacement andarthroscopic surgery, says that ourHospital’s affiliation with Mass Generaland access to its expertise figured heavilyin his decision to come here.

Integrated Care ManagementThis spring, Martha’s Vineyard Hospital

launched the Integrated CareManagement program, based on a similarinitiative pioneered at Mass General andfocused on improving patient carethrough a team approach that coordinatescommunication among patients and theirdoctors, specialists and home careproviders.

Comprehensive RestorationAnd at the Windemere Nursing Home

and Rehabilitation Center,a comprehensive restoration of thebuilding’s exterior is underway, fundedwith $2 million from Mass GeneralHospital and $1 million from PartnersHealthCare. This completes three years ofrenovations at Windemere, which beganwith improvements to the patient carefacilities inside the building – work which

now brings the physical plant up to alevel that matches the compassionate caredelivered daily by its staff.

Behind these headlines are countlessstories of the small transactions thatmake up a year in the life of acommunity hospital – from theemergency room to the maternitydepartment, from the offices of ourprimary care doctors to the pharmacy,the lab, the rehab center and thecafeteria. Our partnership with MassGeneral and the Partners organization

When Angela Rhoderick started worklast summer as the new volunteer

coordinator at Martha’s Vineyard Hospital,one of the first tasks she set for herself was tosurvey every Hospital department in search ofnew volunteer opportunities. “I did that,” shesays, “and it was such an eye-opener –because there are so many ways for volunteersto help and make a difference here.”

Seeing all these opportunities, and drawingupon her own long career experience withIBM, Ms. Rhoderick set up a volunteermanagement system called Volgistics – apackage of software that manages allvolunteer records, helps define each volunteerservice clearly, and creates schedules thatmake it easy to see where the openings are.

But Ms. Rhoderick understands that thebest software is only a tool, and thatvolunteering at Martha’s Vineyard Hospital isfinally about people helping people. In lessthan a year, she’s grown the ranks of Hospitalvolunteers from fewer than a dozen to morethan sixty people. This year, in the quietmonths of January and February when manyIslanders are away on their vacations, 31Hospital volunteers delivered more than 20weeks worth of service.

“We’re growing not only in our number ofvolunteers,” Ms. Rhoderick says, “but also inthe range of services and the hours our

volunteers provide.”One of the Hospital’s newly-minted

volunteers is 17-year-old Jacqueline Menton,a junior at Martha’s Vineyard Regional HighSchool who’s looking ahead to medical schooland thinking about a career in psychiatry.Jackie has been volunteering since February,bringing cheer to residents in Windemere’sAlzheimer’s unit and in the Hospital’s patientcare unit each week in a new program Ms.Rhoderick calls the Friendly Visitors service.

Jackie had learned about opportunities atthe Hospital from a high school classmate,Sophie Ulyatt, who volunteers regularly inthe medical records department. Now she’sworking with Ms. Rhoderick to develop theFriendly Visitors service, volunteering in thepatient care unit every Friday afternoon.

“It takes a very special person who cancommunicate and who wants to spend thetime and make a difference and changesomeone’s day,” says Ms. Rhoderick. “Ourvolunteers from the high school are soamazing – they have such an interest incaring for others and in helping.”

Jackie says her volunteer hours at theHospital are challenging, but satisfying: “Ioften go home and take a nap, to be honest.But this experience is developing my socialskills. Especially the patient visits. I’m only 17and not totally at home in my own skin yet,and this is helping me, pushing me, todevelop in that way. I’m learning to connect

with people, and I feel so rewarded.”Visiting with patients at the Hospital gives

her a healthy perspective on the dailyconcerns of high school life, Jackie adds:“Coming to the Hospital and Windemereafter high school, it’s so refreshing to not belike, ‘Oh, what is she wearing?’ and ‘Who isshe dating?’ It’s really nice to not have thatfor a few hours.”

For anyone wondering if there might bevolunteer work they’d enjoy at Martha’sVineyard Hospital, Jackie has this message:“I’d say that they can definitely findsomething they’ll like here, because there areso many different things to do.” Ms.Rhoderick reinforces that by noting that heroffice has a thick service portfolio now, wherepotential volunteers can explore the wholerange of volunteer opportunities.

The ranks of volunteers at Martha’sVineyard Hospital include people of all ages,but Angie Rhoderick admits that her highschool volunteers bring a uniquely youthfulenergy into the mix: “The students we’vebeen getting from the high school are sointelligent, they’re passionate, they’rehardworking. I say to Jacqueline, can youplease go back to the high school and bringus ten more just like yourself?”

Martha’s Vineyard Hospital launched anew initiative in April. The program,

developed by Massachusetts GeneralHospital, is called Integrated CareManagement. It is based on the premise thatmedically complicated patients haveimproved clinical outcomes and enjoy ahigher quality of life when an outpatient caremanager assists with the care coordinationand communication between patients andtheir team of doctors, specialists and homecare providers.Cheryl Kram, R.N., our new outpatient

high-risk care manager has been workingwith Dedie Wieler, R.N., the Hospital’s chiefquality officer, to adapt the program to theneeds of the Vineyard community.

In her new job, Ms. Kram will manageapproximately 100 Vineyard patients whohave been identified as medically high-risk.They might have multiple medicalconditions that complicate their care, orchronic and severe conditions like diabetesor congestive heart failure.Ms. Kram’s job is to keep in touch with

the patients when they’re at home, makingsure they follow the care plans that keeptheir medical problems managed. Whenher patients are hospitalized here on theVineyard, she’ll work with Gayle Poggi,R.N., the Hospital’s inpatient casemanager, to set up a discharge plan thatenables the patient to be stable and healthy,by connecting them with appropriateservices and resources. And, when Cheryl’spatients are hospitalized at MGH or anyother Partner’s Hospital, she will work withtheir case managers to set up thosedischarge plans as well.Ms. Kram noted that one of her first

tasks with each patient in this program isto understand their circumstances at home,and to connect them with services they might

need in the community. “Regular meetingswith the primary care doctors are built in toreview patients and how they are doing,” she

says. “And a great thing about this program isthat there’s a provision for home visits, so Ican go to someone’s home and see howthey’re living their day – how they’re takingtheir meds, how they’re cooking their meals,how they’re managing.”Ms. Kram will measure the success of this

new program by how patients are doing –mentally and physically – and whether theprogram is able to effect some change thatsupported them out in the community, livingindependently, taking care of themselves,having some quality of life and being able todo the things they want to do.“For those of us who know Cheryl,” Ms.

Wieler says, “it was obvious that she was aperfect fit. She has personal experience as avisiting nurse, a hospital case manager, and ina physician office setting, so she brings all thepieces of the puzzle to the table. In addition,Cheryl is familiar with the support servicesthat are available on the Island. She’s the totalpackage.”Says Ms. Kram, “I feel this brings my 30

years of work experience together, all in onejob.” �

ReachingOut to Improve Patient Care

Mark M. Scheffer, M.D., a surgeonspecializing in orthopedics and sports

medicine, brings a new level of surgicalservices to Martha’s Vineyard Hospital whenhe joins the medical staff on July 15.

A 1989 graduate of the VanderbiltUniversity School of Medicine, Dr. Schefferspent his residency at the renowned MayoClinic in Rochester, Minn., working with Dr.Mark Coventry, a pioneerin the development of hipreplacement surgery.

While serving in theUnited States Air Force,Dr. Scheffer was amember of the medicalteam at the USAFAcademy in Colorado,where he cared for thehockey team and had plenty of opportunityto practice sports medicine. “The Air ForceAcademy has 2,000 cadets bangingthemselves up every day,” he says, “doingcombat training and sports. We got reallygood at sports injuries – knees, ACL tears,shoulder injuries, sprains, fractures. I also dida lot of joint replacements for retired militarypeople – there’s a huge population there.”

For the past 15 years, Dr. Scheffer hasbeen an orthopedic surgeon at theDartmouth-Hitchcock Orthopedics Centerin Concord, N.H. He saw the Vineyard jobadvertised around Christmas time. Aftervisiting the Island for a long weekend inJanuary, he decided this job was for him.

The combination of training at Mayo,

with an emphasis on joint replacements, andspending time in the military with a focus onsports injuries, has given him experiencethat’s useful here, Dr. Scheffer says. “A lot ofyoung doctors now are trained to be ultra-specialists in one area: a knee, hip or ashoulder. My spectrum of practice has beenmuch broader. I’ve felt very comfortable formany years now in the operating room; I’ve

done thousands ofsurgical cases. I feelcomfortable doingarthroscopic cases,trauma cases, jointreplacements. Ithink my range ofexperience is reallyideal for the Island.”

Dr. Schefferplans to offer a new service as well - a newapproach to hip replacement surgery that hebelieves will someday be the standard. Calledanterior hip replacement, it involves anincision at the front of the hip instead ofthrough the buttocks or the side of the hip.This approach, permits the surgeon to reachthe hip socket without cutting through majormuscle groups.

“After anterior hip replacement,” Dr.Scheffer says, “people are typically up on theirfeet the afternoon after surgery, if done in themorning, and sometimes they’re leaving thehospital the next day. My experience is thatpeople come off the crutches and startwalking much faster; it’s a much quickerrecovery.”

Dr. Scheffer says about two-thirds of hissurgeries are arthroscopic – involving accessto injuries through tiny quarter-inchincisions. As with anterior hip replacement,one great advantage of arthroscopic surgeryis that because the incision wound is somuch smaller, recovery time is shorter.

Dr. Scheffer says the affiliation ofMartha’s Vineyard Hospital withMassachusetts General Hospital was a bigfactor in his decision to come here. “Withour integrated X-ray services, I’ll be able tocommunicate in real time with MassGeneral, looking at the same exact imagesthey’re looking at, conferring withorthopedists and radiologists up there.Knowing there’s that kind of support andtechnology, and that resources are there toback me up – that’s so important.”

The satisfactions of work as an orthopedicsurgeon, he says, are deep.

“In other areas of medicine, often you’remanaging chronic illnesses, and there reallyisn’t a cure. Orthopedics is a great way to takesomeone who’s injured, often depressed bytheir injury, immensely dissatisfied about notbeing able to do the things they enjoy – andreturn them to a level of function thatrestores not only physical health but mentalhealth as well. There’s nothing moregratifying, because you really do get thatclosure. You can say goodbye to people whoare better – they’re fixed – and they go backto doing the things they enjoy again.

“When people can’t exercise, their healthsuffers – blood sugars, weight, and blood

pressure all go up – all these other healthproblems can develop secondarily from anorthopedic injury or a degenerative condition.We can reverse that. To see people come in ayear later, having lost thirty pounds, is sogratifying. I had a patient who bicycled acrossthe country after I fixed his knee. It was oneof the proudest moments of his life. I had apatient who was able to participate in thesenior Olympics after recovering from hisorthopedic procedure. Shortly afterwards, hearrived in my office and presented me withhis medal, saying that he couldn’t have doneit without me.”

Dr. Scheffer and his wife Susan are avidsailors. They look forward to their arrival andnavigating in Vineyard waters. �

Martha’s Vineyard Hospital embarkedthis spring on the biggest expansion of

medical services in its recent history, with thelaunch of a new comprehensive medicaloncology and hematology service program.

In a suite of newly renovated rooms in the1972 Hospital building, expanded cancer carefor Island patients is now being provided by ateam of six clinicians from the Mass GeneralCancer Center.

Chemotherapy infusions are beingadministered by infusion nurses Mondaythrough Friday, and by advanced practicenurse Jane Kelly, APRN, BC everyWednesday through Friday. Jane worksMondays and Tuesdays at the NantucketHospital, which is also an affiliate of the MassGeneral in the Partners HealthCare system.

“Patients living on Martha’s Vineyard nowhave access to one of the finest cancertreatment programs in the United States,”says Dr. David Ryan, chief of Hematology/Oncology at MGH and clinical director at theMass General Cancer Center. “We are verypleased that our cancer providers are workingtogether with Martha’s Vineyard Hospital as ateam.”

“It’s soup to nuts”, says Tim Walsh, CEO ofMartha’s Vineyard Hospital. “We tried toexpand oncology service here, several times,before we became affiliated with Partners. Butit just wasn’t possible – we couldn’t make itwork.”

Tim Sweet, chairman of the Hospital board,says “Now all the pieces have fallen into place.I think that from the patients’ point of view,we’re finally beginning to see all the greatadvantages of joining Partners HealthCare.”

The opening of the Mass GeneralHematology/Oncology Service on theVineyard has been welcomed enthusiasticallyby the Island’s primary support and advocacygroup for cancer patients, the Martha’sVineyard Cancer Support Group. “We’reabsolutely thrilled,” says the organization’spresident, Jane Carroll.

A diagnosis of cancer is challenging enoughby itself, says Mrs. Carroll – but the Island’sdistance from chemotherapy services has been

a huge additional burden. Describingthe grueling trips of a woman thesupport group recently helped withher travel expenses, she says: “She getson the boat, she gets on the bus, shegets out at South Station, takes the Tto the Mass General. And afterwardsshe takes the T back to South Stationand the bus down and the boat home.That’s after a round of chemo.”

Dr. Richard T. Penson, clinicaldirector of medical gynecologiconcology at the Mass General CancerCenter, is heading up the team of sixdoctors bringing this new service tothe Vineyard. Joining him are Dr.Christopher G. Azzoli, whose specialexpertise is lung cancer, Dr. Jeffrey A.Barnes (lymphoma), Dr. MarciaBrowne (breast, GI and lymphoma),Dr. Donald P. Lawrence (head, neckand melanoma), and Dr. LidiaSchapira (breast cancer).

“A standard course of chemotherapyis six cycles, administered every threeweeks,” says Dr. Penson, “and thetypical appointment takes half a dayor more.” That means Vineyardcancer patients traveling to Boston fortreatment most often have to stayovernight off-Island, at great costboth in money and in time.

Jane Carroll knows the drill by heart, havingdriven her late husband Tom to Boston forregular cancer treatments for more than twoyears. “I can’t tell you how many snowstorms Idrove through to get Tom to hisappointments,” she says. “If we could have

done those treatments down here, what amajor savings on money, stress and time thatwould have been.”

Dr. Penson says he appreciates that after around of chemotherapy, most patients wantnothing more than to get home and curl upunder the covers of their own bed. With thisnew service, he says, “For many patients,they’ll be able to receive world-class carewithout ever having to leave the Vineyard.”

Mrs. Carroll says the entire board of theMV Cancer Support Group was impressed byDr. Penson’s recent presentation to them onthe new Island cancer service. “He told us thateven if you’re a patient with a doctor atanother off-Island hospital, you can get yourchemotherapy done here. And they’vearranged so that people on their summer

vacations can also get their chemo here. That’sjust wonderful.”

“The new infusion suite is a great space,”says Dr. Penson. “We do think that a lot ofcancer care can be transferred to the Island.We’re committed to delivering compassionatecare, ensuring that patients get the standard of21st-century medical care, right at home onthe Vineyard.”

Patients under the active care of a medicaloncologist can transition their care to one ofthe new physicians from the Mass GeneralCancer Center and will be seen in the MassGeneral Hematology/Oncology Service atMartha’s Vineyard Hospital. In March, a team of seven inspectors fromthe College of American Pathologists

(CAP) paid a visit to the Martha’s VineyardHospital Lab. CAP, which serves as the lab’saccreditation and certifying body, visitsmember hospitals every two years with adaunting amount of requirements to workthrough.

“It was a long day,” says Lena Prisco PhD,lab director, but she quickly adds: “It wentfine. There are always a few items that requirefollow-up on inspections like this, but theHospital lab was recommended for a fulltwo-year accreditation.”

It’s not as if the CAP inspection is theonly time the Hospital lab checks itsprocedures and the accuracy of its methodsand instruments. During daily operations,quality control is run and in alternateinspection years the lab does its own self-inspection, Dr. Prisco says, and allthrough the year, CAP submits randomunknown samples to be sure the lab’swork is spot-on.

Dr. Prisco, who serves as a CAPinspection team leader herself, says thepeer-review process at its best is quite auseful experience. “You can learn a lot ofthings visiting other labs,” she says.

The biggest advances at the Martha’sVineyard Hospital laboratory since thelast CAP inspection two years ago havebeen in two areas: expanding the numberof in-house tests the lab performs, andhandling both test orders and thecommunication of lab resultselectronically.

In all, Dr. Prisco says, the Hospital labnow performs about three dozen tests in-

house that were sent off-Island as recently astwo years ago. There’s an improvement inpatient care and a potential cost savingspassed along to patients, she says. “When atest is done here,” she says, “we can contactthe doctor faster.”

Perhaps one of the most dramatic changesat the lab is its ability to test for Lymedisease and return a result in as little as 60minutes to a doctor in the emergency room.Another example is that the lab can now dohormone tests, enabling Martha’s VineyardHospital to provide laboratory support forfertility treatments.

Meanwhile, the work of the Hospital labhas been fully integrated into the newelectronic medical records system sinceJanuary. “There’s no more hand entry orfaxing,” says Dr. Prisco, “which is moreaccurate and has really decreased thepaperwork. People are still used to paper,but they are thinking more electronicallynow.”

Most of the lab’s older equipment wasupdated when the Hospital moved into itsnew building, but they had to replace onefailing chemistry instrument, a workhorsenine years old, with a new one earlier thisyear. Because that instrument’s work is so

broadly essential, bringing it online tookalmost four months.

Still, somehow Dr. Prisco has also foundtime to set up a new continuing educationprogram for the technicians in the lab. “AsI’m reading and finding materials, I push ittheir way. Here in the lab, you never stoplearning.” �

Windemere Nursing & RehabilitationCenter is receiving much-needed

exterior renovations this spring and summer,thanks to financial support from its corporateparents – a $2 million grant fromMassachusetts General Hospital and $1million from the Partners HealthCareorganization.

“This couldn’t be coming at a better time,”says Ken Chisholm, Windemere’sadministrator. The work now underway atWindemere has been a long time in theplanning: Windemere applied to the statefour years ago for a Determination of Need(DON), and over the past two years has spent$300,000 on a range of interiorimprovements – a new boiler, new showers,new beds and furniture in every resident’sroom, as well as new carpeting and otherrenovations to the recreation area, cafeteriaand conference room.

After all this interior work, attentionshifted to the exterior of the nursing home,which dates to 1994. The price tag wasoverwhelming, Mr. Chisholm says:

“When we filed that DON, we really

didn’t know how Windemere was going to beable to afford this work. But everything’scome together – we had this whole projectplanned and ready to go, the state approvedit, and now Mass General and Partners havestepped up to pay for it.”

The exterior work involves new siding,new windows and doors, and new decks forthe entire facility. “When this project iscompleted at the end of September,” Mr.Chisholm says, “We’re going to be in goodshape, top to bottom, inside and out.”

As pleased as the administration and staffare with the physical renovations, they haven’tforgotten that Windemere’s mission is thedaily care of its residents. The nursing home’s

string of awards and high scores onnational quality-of-care measuressuggest that this mission hasn’t beenlost in the carpenters’ bustle.

For the second year in a row,Windemere received the highest 5-star rating granted by the federalgovernment’s Medicare program,which is based on detailed healthinspections, ratings of staff andmeasures of quality of care. Thenursing home received a perfect score:“no deficiencies found.” And U.S.News & World Report Magazinelisted Windemere among the bestnursing homes in the nation.

“We’re very proud,” says Mr.Chisholm. “It takes a lot of work tomaintain this high rating. We’realways looking to improve.”

Windemere offers short-termrehabilitation, independent livingaccommodations, and long-term care,with specialized programs for thosewith Alzheimer's and dementia.
