COURSE (this completed form must be attached to all copies of certificates both forwarded to PHQ with

the MPR and in the class notes of Alliance)

choice) {*for a replacement certificate you will require the RED certificate number located at the bottom

left hand side of the original Transport Canada Marine certificate – preprinted onto the paper}

New certificate (this certificate can be saved after it is filled in and this is useful if you have multiple

students in a class, as you can save to your computer file under the students name for each certificate,

removing only the Certificate number, Surname and Given Name, CDN and Date of Birth before you add

the next student to the certificate) printing all after you have compiled and check the data for each

student)

Date beside signature would be last day of class

Name of Duly Authorized is to be the name of the instructor for that class –

Print on Transport Canada certificate paper, place branch stamp in appropriate spot and photocopy.

Have both instructor and student sign both certificates. We keep the copy, to be attached to the

following form and sent to PHQ with the MPR – **a copy needs to be scanned and attached to the class

notes.

Scan a *pdf of the certificate (s) and the REPORT OF STUDENTS HAVING SUCCESSFULLY COMPLETED AN

APPROVED COURSE and save it under the class notes in Alliance

REPRINTED CERTIFICATE FORM

If a student comes in as they require a replacement certificate, find the copy of the old certificate in

Alliance – scanned under the class notes

The number in the bottom left hand corner of the original certificate, which was already on the

Transport Canada paper is the “This training certificate is a replacement for training certificate number”

and the date signed is the “issued on” the rest is self-explanatory as per the original certificate:
