LTC 2015 | Design Contest

Official Entry Form

Rules :

1) Page 1 must be complete and signed by the parent/guardian.

2) The artwork submissions must be included on Pages 2 and/or 3 and 4.

3) For legibility purposes, participants are encouraged to fill out the fields of the form online prior to printing out

the entry form.

4) Please print each page on a separate sheet.

5) All entries must include the words “LTC 2015”, “New York District Key Club” or “NYDKC”, and “Espionage.”

6) All entries must include the image of the New York District Key Club logo, a beaver, and incorporate the theme

of espionage into the design.

7) The winner will be notified prior to LTC 2015 via email and/or phone call.

tions and concerns.

9) All entries MUST be received by December 31 11:59:59PM. Submit to pin@nydkc.org and/or t-shirt@nydkc.org

or mail to the address below.

10) New York District Key Club reserves the right to not designate a winner for the contest.
