OUR PACE PROVIDES TOTAL HEALTH CARE

Since 1995 we have provided complete care through the PACE (Program of All-inclusive Care for the Elderly) model. We provide care in our PACE centers, in your home, and also work closely with local hospitals.

From learning about nutrition to competing in stationary bike races, our Adult Day Health programs enrich the lives of our participants with medical, socialization, social work, personal care, physical and occupational therapy services.

PACE services include, but aren’t limited to:

• Primary, specialty and emergency medical care• Home care including nursing, personal care,

homemaking and meal preparation• Medications, medical equipment and supplies• Transportation and companion services• Adult Day Health Center including activities,

exercise programs, and meals• Physical and occupational therapy• Behavioral health, counseling and social work• Assisted living and nursing home care• Full dental care• Hearing aids• Acupuncture, chiropractic care and other

medically necessary treatments

Upon enrollment participants must receive all health care, primary care and specialist physician services–other than emergency services–as authorized by PACE, or be fully and personally liable for costs of unauthorized services.

ELIGIBILITY

To be eligible for PACE you must:

• Be 55 years old or older

• Live within our service area (see back for list of towns)

• Be screened by our nurse as meeting nursing facility clinical criteria

• Have the ability to continue living safely at home
