CLEARANCES AND TB TESTS

REQUIRED FOR STUDENTS

LOCK HAVEN UNIVERSITY OF PENNSYLVANIA

Students going out into the schools (public and private) K-12 or to any public agencies are required to have the

clearances and TB Test listed below. Links to clearance applications are on page 4 of this packet. The

clearances are obtained solely by the student and the results are only shared with the student.

Directions for Recording Data

Students should wait until they have all the mentioned documentation and then personally bring them into the

Clearance Office to be recorded. There is a cover sheet that needs to be completed and attached to the

clearances and TB test result (Page 3 of this packet). The Clearance Coordinator checks these documents for

authenticity, records the dates and then returns the original forms to the student in person or by mail. You will

need to update the cover sheet every time you update clearances.

NO COPIES OF CLEARANCES ARE KEPT ON FILE IN THE CLEARANCE OFFICE OR THE TEACHER

EDUCATION OFFICE. IT IS THE RESPONSIBILITY OF THE STUDENT TO KEEP THESE DOCUMENTS

IN A SAFE PLACE.

Students are permitted to mail documents to the Clearance Coordinator for recording. If you choose to mail

them you print off the cover sheet and mail with the document(s) or attach a note indicating:

1. Major

2. ID number

3. Semester/year of entrance to LHU.

Criminal Record Check (Act 34 Background Clearance) Only document with State Seal Watermark Accepted

The Criminal Background Check can also be obtained online using a debit or credit card at https://epatch.state.pa.us .

Is good for one year from the date the clearance has been granted.

Students must check “Other” under Purpose of Clearance

Must say “no record”.

Volunteer Clearances will not be accepted!

May take two to four weeks to be processed.

Applicants must make sure that they print the “official” Certification Form with the state seal watermark in the background.

Pennsylvania Child Abuse History Clearance (Act 151)

Only originals Accepted

Is good for one year from the date the clearance has been granted.

Students must check “School Employment Governed by Public School Code” under Application Purpose

Must say “no record”.

Volunteer Clearances will not be accepted!

Child Abuse History can obtained online at https://www.compass.state.pa.us/cwis/public/home

Login or Create an Account, Follow directions and note that information you will need before you start, you should have the following information readily available to help you complete your application:

Addresses where you have previously lived

Names of all individuals with whom you have lived to include parents, guardians, siblings, spouses, etc.

Any previous names you have used or have been known by

Credit Card (only) information for an application fee (or a payment code from the organization that is asking you to obtain a Pennsylvania Child Abuse History Clearance)

Follow directions and you will receive results via e-mail and you will be able to print the results from your computer. It may take 3-4 weeks to receive, so plan accordingly.

FBI Clearance (Act 114)

Only originals Accepted

All student teachers, students participating in the classrooms, and anyone employed in private or public schools must have a current FBI Federal Criminal History Record done.

This clearance is good for the entire time you are enrolled at LHU; unless you lose the original or if you have a break in your enrollment.

Must say “no record”

This clearance requires fingerprinting done at specific sites in Pennsylvania.

You must be registered with IdentoGO at https://uenroll.identogo.com/, using code 1KG6RT prior to having your fingerprinting done. You can also register by phone by dialing 844-321-2101. Locations are scattered, with the closest to LHU being South Williamsport and State College. If using Glennon, please refer to page 7 of this packet.

Enter the link indicated above into your web browser. This website will walk you through the process and show you the fingerprinting location sites. An instruction sheet is attached.

It may take 4 weeks to receive your clearance results back. You will not be able to enter schools without this; so plan

ahead. ( More directions listed on page 6 and 7)

Tuberculin (TB) Test

Only Physicians Report Accepted

Is good for two years from the date of the test. If a student has a valid TB test on record at the LHU Glennon Infirmary, the student may go there to request a TB Test Verification Form. (New students must have the TB test as part of their Medical Record for entrance to the University, so it will be part of their physical examination form and we recommend that you keep a copy of that TB Test Verification information to submit to our office.)

If a student needs a TB test, the Infirmary will do the test free of charge everyday but Thursday

The TB Test verification from the infirmary or from a private physician must include the student’s name, date of test, and the test results (which should be negative), and the physician’s name on the report. No hand written notes accepted

Act 24 Arrest/Conviction Report and Certification Form

Self-reporting clearance that is required by some school districts. Please print, complete, sign, and keep with your other clearances. (See Page 5 and 6).

Mandated Reporter Training

Copy of Certificate is required

(Act 31) Certificate of Completion required as part of Candidacy

All students are required to complete the on-line training as part of Candidacy.

This training is good for the entire time you are enrolled at Lock Haven University, unless you have a break in your

enrollment.

The 3-hour free training can be found at Mandated Reporter Training. You must first register and then take the course.

A copy of the completion Certificate must be submitted to the Office of Student Teaching, Clearance Coordinator, (RLC

609).

Note: Non-U.S. citizens must apply for clearances and a TB test as well.

Fingerprinting Directions for using Glennon Public Safety

1. Apply for the FBI Federal Criminal History Record at: https://uenroll.identogo.com . This link will direct you to registration

for Pennsylvania PDE- Colleges/Universities, Teacher Education Program site.

HUMAN SERVICE CLEARANCES WILL NOT BE ACCEPTED!

a. Enter the Service Code 1KG6RT (Use must use this code!)

b. Enter all of your information - Legal Name must match exactly on all identification documents brought to

enrollment.

c. Enter Citizenship Information.

d. Enter your Personal Information.

e. Enter your Mailing Address f. Enter the Document you will be taking for identification purposes. g. Select “Schedule or Manage Appointment”.

h. Enter SP-LOCKH for the enrollment center at LHU Public Safety or enter your zip code to find a location near

home. If you choose Lock Haven, follow the steps below.

i. After Lock Haven appears below the location box, click in the Next button beside Lock Haven

j. Choose “Walk In” and then hit submit if scheduling at LHU. If selecting another location select a time/date.

k. Next, hit Submit.

l. Make a copy of the registration which includes your UE ID# and appointment.

m. Take 1 copy with you to the location for your fingerprinting.

n. You will need to pay at the location you chose. You cannot pay online.

Fingerprinting at Glennon Public Safety Continued…

o. If you chose Lock Haven, report to Glennon Public Safety. You will have to pay before getting your Fingerprints.

Bring your Credit Card ( No Cash will be accepted), Drivers License ( or document you registered you were bringing

to the appointment), and a copy of your registration for Fingerprinting.

p. Once you receive the original document, bring the original clearance information to the Office of Student Teaching

and Field Experience Office 613 Robinson to Mrs. Kelly Raudabaugh.

q. Fingerprints are only done from 8:00-11:30 am Monday – Thursday. Please plan accordingly.
