What are we going to do? How will we do it? Who will be involved?

Timeframe Outcome Activity

- Improving MND Care survey - Regional service mapping - Other identified data sources (including

those provided by NHS and Social Care e.g. HES data)

(Link to RDP Outcome 2 below: activity 2c)

LDT Ongoing 1c, 1d, 1e

Use influencing and negotiation to improve health and social care services not funded by the Association where there are identified shortfalls

Utilise LDT meetings and wider consultation to help identify shortfalls & inform local clinical and social care priorities, and work with regional staff and HSCPs to influence better provision. Target commissioners where there appear to be fundamental shortfalls in provision. In particular:

Social Care Pathway

Improve access of pwMND to early intervention/Social Care. Audit numbers of pwMND involved with Social Care (stepped approach)

In particular look further into time it takes for social care funding for adaptations in North Yorkshire

Respiratory Pathway

Improve access to community respiratory services pathways in line with NICE guideline/Transforming MND Care audit tool (timely) in Sheffield Care Centre catchment, following recent change in service

(Link back to RDP Outcome 1; outcome activities 1c, 1d and 1e) West Yorkshire Gaps (Calderdale, Huddersfield, Halifax, Kirklees, Holmfirth). Anecdotal negative experience of

Own anecdotal data (St Michael’s hospice) suggests that issues are evident for pwMND, e..g not taking on any additional work:

Document issues/cases affecting local pwMND

Palliative Care Need identified for hospice/palliative care beds in Bridlington/East Riding of Yorkshire (not all MND patients want to be cared for in care homes or moved to Beverley)
