Per Revised Code section 4503.29, the Ohio Bureau of Motor Vehicles (BMV) works with the Ohio Department of Veterans Services (DVS) to create and maintain license plates displaying military valor and service.

The Purple Heart license plate may be issued for display on passenger vehicles,non-commercial

trucks, recreational vehicles, house vehicles, motorcycles and non-commercial trailers at no cost (you may select for own characters for a small annual fee).Eligibility: Applicant must submit a copy of their DD 214 showing award of a Purple Heart.

The Former Prisoner of War license plate may be issued for display on passenger vehicles,

non-commercial trucks, recreational vehicles, house vehicles and non-commercial trailers at no cost. Eligibility: Only members of the U.S. Armed Forces who were imprisoned by an enemy of the United States may apply; Applicant must complete form BMV 4507 and submit one of the following: (a) documentary evidence of POW status from one of the Armed Forces of the United States; (b) telegram advising family of Prisoner of War status; (c) letter from the War Claims Commission; (d) letter from U.S. Department of Veterans Affairs.

The Disabled Veteran license plate may be issued for display on passenger vehicles, non-commercial

trucks, house vehicles, motorcycles, motorhomes and non-commercial trailers at no cost (you may not select your own characters).Eligibility: Applicant must complete form BMV 4507 and submit a current letter (dated within the past year) from the Veterans Administration stating that he/she is 100% disabled or that he/she received a monetary allowance to have his/her vehicle modified.

Note: Veterans who are qualified for issuance of this plate may purchase an unlimited number of Additional Disabled Veteran license plates at the cost of a standard license plate.

The CongressionalMedal of Honor license plate may be issued for display on passenger vehicles, non-commercial

trucks, recreational vehicles, house vehicles and non-commercial trailers at no cost. Eligibility: Only recipients of the Congressional Medal of Honor may apply; applicant must submit documentary evidence of receiving this award.
