The department ‘transferred savings from other line items within the budget to claims from the state in order to pay the claims. The result was that key vacant positions were not filled at some hospitals. Statistics released by the Minister for Public Service and Administration, Senzo Mchunu (February 2021) revealed that more than a third (3301) of the 9 477 senior managers in the public service registered on Persal did not have the required qualifications for the job. In provinces, most of them were in health departments. The following has not been finalised Accredited private sector vaccination sites will be paid an administration fee to cover professional fees and the cost of consumables, and will be reimbursed by medical schemes, but these costs are still being calculated; The DoH received 24 bids to provide transport, warehousing and distribution services for COVID-19 vaccines, but has yet to award the tender . The following is still not clear: How much the price paid by medical schemes will differ from the price the government pays to vaccine suppliers; How the fees payable to healthcare providers will be pegged; and How private healthcare providers, such as pharmacies, are to be compensated for dispensing vaccines to state patients. Constitutional Court confirms alternative compensation remedy for negligence Following a Constitutional Court judgement, SA courts will now be able to consider ordering alternative compensation – instead of a lump-sum payment – to those injured as a result of medical negligence, "The Constitutional Court judgment involved an application for leave to appeal against a judgment of the Supreme Court of Appeal (SCA). "The possibility of providing physical items or state medical services, instead of money, will be welcomed by the provincial Health Departments who face staggering medical negligence claims," reported Medical Brief (14 April 2021). In this past financial year, Eastern Cape Health paid out R920-m on medical negligence claims and KZN's medical negligence claims increased by R3-bn in the 2018/2019 financial year to R23-bn in 2019/2020. Johnson&Johnson vaccination rollout on track again after suspension From Wednesday, April 28, SA’s Johnson & Johnson Sisonke “TOGETHER” COVID-19 vaccine trial rollout for healthcare workers, is back on track with a revised implementation protocol. The programme was paused on April 13, after discussions between the SA Medical Research Council, the Health Minister, and researchers about whether to pause administering the shots until there is greater clarity on the possible link between the vaccine and the blood-clotting events reported in the US and Europe. First in line will be a further 200 000 healthcare professionals and then people older than sixty. Vaccine contracts awarded The government has awarded contracts for importing and distributing COVID-19 vaccines, which is due to begin on May 17, reported Business Day (23 April 2021). Biovac and Imperial Logistics have been appointed to provide importation services, while the job of transporting, warehousing and distributing the vaccines has been split between Biovac and DSV Healthcare. Biovac will cover all the cold chain requirements for vaccines that require storage at 2°C to 8°C, at -20°C and -70°C. DSV Healthcare will only provide services for vaccines requiring storage at -70°C. A third company is expected to be appointed shortly to assist with the -20°C services. The company is negotiating with landlords and retailers as well as nursing agencies to supply the vaccination services that the 191 Dis-Chem stores plan to provide. According to Clicks CEO Vikesh Ramsunder, the retailer has the necessary infrastructure, including more than 600 pharmacies and 192 clinics, to help facilitate the vaccination roll-out. Clicks and Dis-Chem will also be able to distribute vaccines to other vaccination sites. Chantal Marx, head of investment research at FNB, said using Dis-Chem and Clicks as part of the vaccine rollout is “a very clever way of scaling distribution and makes perfect sense”. Clicks and Dis-Chem “won’t be making any money”, but she expects in-store sales to spike as the newly vaccinated wait the required 15 minutes after the jab to ensure that there are no side-effects. PHARMACEUTICAL NEWS Vaccine claimants give up right to sue In terms of new regulations for the compensation fund South Africans who claim from a COVID-19 vaccine injury fund will give up their rights to approach the courts to sue the government or health workers, should they suffer severe injury from taking the injection This follows as drug companies providing vaccines to SA, like Johnson & Johnson (J&J) and Pfizer, insisted on a no-fault compensation scheme run by the government as a prerequisite for supplying the vaccines. The regulations were published on April 22. However, it is not clear from which departments’ budget the government will source money to pay claims. Brain clots far more likely from C-19 than vaccines Reuters reported on April 15, that according to British research there is a much higher risk of brain blood clots from COVID-19 infection than there is from vaccines against the disease. The researchers said in a pre-print study that the risk of CVST was eight to 10 times higher following COVID-19 infection than it was from existing vaccines for the disease. The researchers from Oxford University, highlighted that COVID-19 was associated with more common clotting disorders than CVST - such as strokes - and that recent debate around vaccines had lost sight of how bad the disease itself could be. The team worked independently from the Oxford vaccine team which developed AstraZeneca’s shot. Malaria vaccine trial results may indicate a ‘game-changer’ The first malaria vaccine to meet a World Health Organisation (WHO)-specified goal of more than 75% efficacy has been developed by scientists at the University of Oxford, reported Reuters (23 April 2021). The shot uses Matrix-M adjuvant technology and was used in trials involving 450 children ages 17 months to five years in Burkina Faso and “was well-tolerated”, the team at the university’s Jenner Institute said. The Serum Institute of India has agreed to produce the malaria vaccine once approved. Lesotho grower is Africa’s first to sell medicinal cannabis flower to EU A Lesotho-based cannabis cultivator, MG Health, has been approved by the European Union (EU) as the first manufacturer in Africa to export the medicinal cannabis flower as an active pharmaceutical ingredient to the EU, reoprted Business Insider (16 April 2021). MG Health operates a cannabis cultivation and manufacturing facility in the mountainous region near Mohale Dam. Meanwhile doctors in Ontario in Canada are still hesitant to prescribe medical cannabis to patients suffering long-term pain – 20 years after it was first introduced and despite hundreds of thousands of Canadians using medical cannabis – according to a study published in the Canadian Medical Association Journal. GENERAL NEWS Quality of HIV care higher in clinics than hospitals According to the latest research from Wits University and Boston University in the US, HIV patients in SA experience huge gaps in the quality of care although 96% of South Africans live within 10km of a healthcare facility that provides antiretroviral treatment (ART). Quality of care was higher in clinics versus hospitals; in rural health facilities rather than urban areas; and in larger facilities. Quality of care was lower in areas with greater poverty. KZN scored best and Limpopo worst on starting treatment on time, retaining patients in care, suppressing viral load and monitoring patients for treatment failure. Researchers are convinced that targeting low-performing facilities for additional support could reduce the overall burden of disease and understanding the differences in quality of care over time could guide interventions to improve health outcomes. South Africa's Coronavirus Update: (28 April 2021) Cases Reported: 1,578,450 Deaths: 54,285 Recovered: 1,503,611 SA’s ‘sugar tax’ shows results According to a senior researcher at Priceless-SA, the latest study has found a 51% reduction in sugar, a 52% decrease in calories and a 29% reduction in the volume of beverages purchased per person per day. This follows the implementation, in 2018, of SA’s “sugar tax”. The e-study was recently published in Lancet Planetary Health. The study highlighted that the announcement and implementation of the tax coincided with large reductions in purchases in terms of volumes and sugar quantities from taxable beverages, with non-significant changes for non-taxable beverages. New app delivers medicine A new SA app, PharmaGo, was launched in March to facilitate free same-day and next-day deliveries for essential and over- the-counter medication, reported Financial Mail (15 April 2021). PharmaGo, created by Raees Carim, passes the customer’s prescriptions, uploaded on the app, to a “pharmacy panel” available only to the pharmacist filling the prescription. This ensures information security and discretion and is exclusive to PharmaGo. Payment is via credit or debit card and is sent direct to the medical aid. At launch, PharmaGo had 73 pharmacies on board. It uses existing distribution networks for deliveries. NEWS FROM MEDICAL AIDS DoH declares certain DSP practices by schemes irregular Certain practices by medical schemes in selecting Designated Healthcare Providers (DSPs), as well as imposing excessive co-payments on members, have been declared an irregular and undesirable practice by the Department of Health (Section 61 of the Medical Schemes Act No131 of 1998). A notice to this effect was published in the Government Gazette (23 April 2021) This follows information provided by the Council for Medical Schemes (CMS) regarding the way in which some medical schemes: Unilaterally and without restriction, determine the criteria to apply when selecting their DSPs; Select DSPs without engaging in a fair procurement process; Unfairly limits the number of selected service providers (including pharmacies) available to provide healthcare services to the members; and Oblige members to use only the selected limited number of DSPs or risk paying exorbitant co-payments. Regulations will be published in the next 6 months to provide guidelines. Medical Schemes have turned into a 'business science' Read More Prof Marietjie de Villiers, former deputy dean of the medical school at Stellenbosch University and former deputy chairperson of the Medical and Dental Professions’ Board, says medical aids "have turned into a business science". According to De Villiers the days are long gone when medical aids were to make it easier to pay for patients and easier to collect money for doctors. They have become "entrepreneurial businesses and a focus on the client or member is no longer the point, ”reported Chris Bateman in Mail & Guardian (16 April 2021). “A combination of an outdated regulatory environment well behind the global cost- efficiency curve, ineffective fee-for-service structures, high specialist and hospital costs, plus an ever-ageing medical scheme membership create what one funding expert calls a ‘death spiral’.” To read more, click on the button below C-19 vaccine: Medical Schemes won’t subsidise non-members Three medical schemes, Discovery Health, Fedhealth and Profmed, have poured cold water on the idea that they could subsidise vaccinations for people who do not have medical cover, reported TimesLIVE (14 April 2021). Discovery CEO Adrian Gore said as a result of global vaccine shortages, procurement should be left to the state, though with strong private sector collaboration. However, Profmed CEO Craig Comrie said while manufacturers were not yet willing to sell vaccines directly to private business organisations, the scheme had had discussions with “many” manufacturers. Fedhealth principal officer, Jeremy Yatt, said subsidising vaccinations would be illegal as a medical aid is a not-for-profit organisation, and the funds therefore belong to the members who contribute every month.
