livingwellCARES participants with Type
2 Diabetes all at the point of medication
escalation based on the AACE guideline
criteria
• Pre and post hemoglobin A1C and a pre
pilot cardiac stress test were obtained
from all participants
• Exercise workouts were conducted in
person and through the smartphone app
at Fitscript, the LWC Fitness Center at
Yale New Haven Health and at home for
90 days
• Participants used Glucosezone exercise
guidance based on real-time glucose
levels entered using the iHealth blue-
tooth glucometer at the beginning,
during and end of the exercise session
• Data was de-identified in accordance
with the HIPAA Act (1996)

Exercise Guidance Based on Real-Time Glucose Levels
“Given that the majority of individuals with type 2 diabetes have access to the internet, technology-based support is appealing for extending clinical intervention reach. For adults with type 2 diabetes, internet-delivered physical activity promotion interventions may be more effective than usual care. Effective internet-based programs included monitoring of physical activity, feedback, goal setting, and support from a coach via phone/e-mail”.
