    Self medication in INDIAN CONTEXT

    In economically deprived communities, most episodes of illnesses are treated by self-medication.

    Due to the Illiteracy and high cost of modern medicines urban areas, difficulties arise in accessing

    modern healthcare. Drug retail shops frequently serve as the public's first point of contact with the

    healthcare system. In India, pharmacists and pharmacy attendants play an important role in

    fostering self-medication among the public. Combination preparations containing 'hidden' classes

    of drugs and food supplements or tonics of doubtful value were commonly used in India. Non-

    doctor prescribing of medicines is also common in developing countries. A "doctor" in this context

    is defined as someone who has obtained a bachelor of medicine and bachelor of surgery (MBBS)

    degree in allopathic medicine. In India, certified health assistants (CHAs) and community health

    volunteers (CHVs) carry out preventive and curative health activities in rural areas.

    Complementary and alternative medicine (CAM) practitioners are found in both the rural and

    urban areas. They occupy an important place in the village community. 'Compounders' who have

    worked for some time under practitioners in cities and have now set up their own clinic, as well as

    CHAs and medical shop owners are easily accessible providers of healthcare in such rural areas.

    INTRODUCTION TO SELF MEDICATION

    Definitions:-

    Self medication is the use by lay persons of non prescription medicines for symptoms and minor

    ailments, with those persons bearing the full responsibility for his or her own treatment. Or The

    selection and use of medicines by individuals to treat self recognizable illnesses and symptoms.

    Or The treatment of common health problems with medicines especially designed and labeled for

    use without medical supervision and approved as self and effective for such use.- By World self

    medication industry (WSMI) In India, though the phrase has no legal recognition, all the drugs

    that are not included in the list of prescription drugs are considered as

    non-prescription drugs (or OTC drugs). Prescription Drugs are those that fall under two schedules

    of the Drug and Cosmetics Rules, 1945: Schedule H and Schedule X. Drugs falling under Schedule

    G require the following mandatory text on the label: Caution: It is dangerous to take this

    preparation except under medical supervision and hence are not advertised to the public

    voluntarily by the industry. In India, the import, manufacture, distribution and sale of drugs and

    cosmetics are regulated by the Drugs and Cosmetics Act (DCA) and its subordinate legislation, the

    Drugs and Cosmetics Rules (DCR). Prescription to OTC Switch Medicines for self-medication are

    often called 'nonprescription' or 'over the counter' (OTC) and are available without a doctor's

    prescription through pharmacies. India is inching the OTC way. Indian OTC healthcare is in

    nascent stages and ranks 11th in the global OTC market. But, urban India is catching up with the

    concept, thanks to the advent of technology, improving literacy levels, increasing health awareness

    and high work stress levels. In fact, Indian consumers, today, are confident about sharing

    healthcare responsibility, especially in case of common ailments. Considering the changing

    mindset and likely changes in regulatory framework, such as, OTC guidelines and open

    distribution, it is reasonable to estimate that within the next ten years, India will become a major

    contributor to the world of OTC market. Currently, aches/pains, cough, colds, hyperacidity, minortopical infections, and indigestion are major OTC categories. Emerging categories include cuts,

    wounds and burns, muscle pains and sprains, diarrhea and constipation. There are many

    Prescription products, which could be revitalized through OTC switches. An analytical

    interpretation of various data placed the focus on vitamins, cough & cold, antacids, antipyretics

    and NSAIDs as opportunity areas for switch in India.

    medication.

    -To identify the source from where patient(s) gets information for

    self medication.

    -To determine in which proportion various class of drugs used in self

    medication.

    1. Gather secondary data on Self Medication from books, research

    articles, and websites.

    2. Survey of Consumers and Retailers, a structured questionnaire being

    administered to the respondents.

    territory of Surat city.

    Retailers (Chemists) selected from different territory of Surat city

    as per the convenience.

    DATA ANALYSIS

    This survey on Extent and attitudes of consumer towards self medication carried out by using

    200 Consumers and 50 Chemist, in Ahmedabad territory during the period of June-July, 2010.

    There are separate questionnaire developed for both different group of respondent. The outcomes

    derived from analyzing collected data are as follows. Note: I denote the basic findings with A. and

    Survey findings with B.

    Consumer survey indicates that there were 171 out of 200 took drugs for healing some basic

    ailment like common cold, pain, acidity, infection etc as self medication in the past. From further

    analysis it was found that there is no significance difference in self medication consumption

    between single and married respondent. It was also shows; there are significant relationship of

    extend of self medication with gender, age, and family income. Self medication was more

    prevailed in male (90%) as compare to female (70%). Extend of self medication increases with

    increase in age, and decreases with increase in family income.

    And chemist survey point out that there were 47 out of 50 chemists sells medicines to their

    customers without medical prescription. And Out of total customer visited to drug store per day,

    31% of them came for self medication.

    On asking that who is going use that medicine, which you bought without medical prescription

    various respondent gives response as per above table. In this case maximum respondents bought

    medicine for him/her self and secondaly for his/her family member. Someone else included his/her

    boss, manager or friend etc.

    When any respondent walked to take self medication, He/she was lacking the knowledge about the

    drugs which they took in self medication. It was supported by research data that, Out of 47, 42consumers (72%) ask for advice related to safety of drugs, rout of administration and/ or a

    particular drug. There were 60%, 83% and 52% of respondent ask for rout of administration, safety

    of drugs and its alternative drugs respectively. And 29% of respondent ask for other advice like

    illness specific drugs, duration of drug therapy to cure the illness etc.

    81% respondents said that they didnt get any type of unwanted advise in the drug store, whenhe/she bought a medicine without medical prescription. In this case unfortunately, we have to

    conclude that there were no ethical practice in this field of drug seling because, many of the drugs

    they were sold had side effects and complications, which required physician supervision in order to

    take those drugs. Possibly reason would be a business loss because of cut throat competition and

    lack of regulatory activity of such practices.

    B..5: From where you got the information about self medication?

    Information or knowledge about the drugs play vital role in self medication because, you have to

    directly approach the chemist to buy a medicine. So in this survey I was included some questions

    about the possible source like past prescription, advertisement and third party with orders. When it

    was asked to respondent, from where you got the information about the drugs for self medication

    out of 176 responses; 74 said that it was from past prescription, 26 from advertisement, 60 from

    third party and 20 respond it was obtained from other sources like CIMS, IDR, Drug Today and

    Internet etc. Every source of information expanded as given below.

    There are so many therapeutic classes of drugs in which consumer take the self medication.

    Amongst them I choose some classes for survey according to its important and frequency of use.

    Consumer survey indicate that out of 171 responses, 123(74%) respondent said that they had taken

    analgesic drugs and 139 (81%) respondent said that they had taken drugs for the common cold in

    the past as self medication. and 120(70%), 47(27%), 52(30%), 41(24%), 35(20%), 26(15%),

    43(25%) respondents said that they had taken drugs for headache, acidity, infection, vitamin

    deficiency, skin problems, asthma, vomiting respectively. So according to consumer survey there

    was maximum market share in common cold sector in field of self medication. And second is for

    pain killers which the consumer is used as self medication. In this survey out of 171 respondents

    who were took self medication in the past, 113 (66%) respondent use herbal/Ayurvedic drugs in

    the management illness.

    B..6.2: Which classes of drugs patient buys for self medication?

    Above chart is obtained from the survey of 50 chemist and it is indicate the sales of various classes

    of drugs used in self medication. According to data there were analgesics on the first position insales with 94% of total chemist who sold drugs in self medication. And drugs used in common

    cold closely next to it with 93% of chemist. There were antacids with 70%, anti emetics with 40%,

    anti asthmatics with 29%, vitamins with 53%, dermatological with 56% and Antibiotics with 64%

    of chemist sale the drugs of these classes.

    In case of ANALGESIC, following drugs were most frequently sale by chemist like Paracetemol

    (PCM), Ibuprofen, Nimesulide and Diclophenac Sodium. Others were algine, aspirin and acetyl

    salicylic acid. Amongst them PCM is on the first position with 27% of total analgesic selling. Than

    Ibuprofen Nimesulide and Diclophenac sodium were on the declining ordinal position with 25%,

    15% and 14% of total selling respectively.

    B..7: The factors, which are affect respondent to choose self medication without physician

    consultancy.

    There were many types of factors which affect the respondent to choose self medication as an

    alternative of conventional health care chain. Those included Economic Condition of the

    respondent, the time it takes to see a doctor, Desire of respondent to be self reliant, Advertizing of

    drugs and its formulation in media, distrust of conventional drugs and Routine/Literacy practice by

    Pharma field personnel, other educated or knowledgeable personnel.

    Amongst all possible factors which attract or causes the people to take self medication, most of the

    respondent said that they had no time to follow the conventional health care chain and this

    positioned at first with 79% of total respondent. And Economic, Advertising, Desire to be self

    reliant, Routine/Literacy practice and distrust of conventional drugs were on the declining ordinal

    position with 66%, 52%, 51%, 25% and 22% of total respondent respectively. Whereas others

    included fear of the consultancy and unavailability of physician etc.

    The consumer response to the question self medication is dangerous for our health Yes or No,

    Was very marvel with respect to extent of self medication in consumers population. Out of 188

    respondents, 176 (94%) respondent accepted that Yes Self medication is dangerous for our

    health and they gave the various reasons like most of the drugs have side effects, safety profile is

    not known by general public, timely based dose accuracy is required etc. Only 12 (6%) respond

    with No answer, backed up by argument that they never severe side effects or complication till

    date.

    RESULT

    There were 171, out of 200 consumers took drugs in self medication for healing some basicailment like common cold, pain, acidity, infection etc. in the past.

    Out of 47, 42 consumers (72%) ask for advice related to safety of drugs, rout of

    administration and/ or alternatives of that particular drug.

    81% of the total respondents said that they didnt get any type of unwanted advice in the

    drug store when he/she bought a medicine without medical prescription.

    Out of 171 responses, 123(74%) respondent said that they had taken analgesic drugs and

    139 (81%) respondent said that they had taken drugs for the common cold in the past as

    self medication. and 120(70%), 47(27%), 52(30%), 41(24%), 35(20%), 26(15%), 43(25%)

    respondents said that they had taken drugs for headache, acidity, infection, vitamin

    deficiency, skin problems, asthma, vomiting respectively.

    79% of respondent said that they had no time to follow the conventional health care chain

    as a reason of self medication.

    Out of 188 respondents, 176 (94%) respondent accepted that Yes Self medication is

    dangerous for our health.

    CONCLUSION

    Self medication is more prevailed in male (90%) as compare to female (70%). Extent of

    self medication increases with increase in age, and decreases with increase in family

    income.

    In self medication maximum people buy medicine for him/her self only.

    Out of total customer visited to drug store per day, 31% of them came for self medication.

    Most of consumers are lacking the knowledge about the drugs which they took in self

    medication.

    Maximum Consumers have doubt about the safety of drugs.

    There were no ethical practice in the field of drug selling because, many of the drugs they

    were sold had side effects and complications, which required physician supervision in
