Cherry Creek Schools Parent Community: Cherry Creek Schools requires families to update their household and student information each year including emergency contacts, medical, and policy acceptance forms, along with school specific forms. These forms will be made available online beginning July 1, 2016 and are part of the annual check-in processes for the start of the 2016-2017 school year. Parents who submitted information last year can save time by accessing the system to review, update, and save information previously stored. Parents can always return to the online forms to update information that may change during the school year (i.e. phone numbers, emergency contacts, medical updates, etc.). In July you will receive an email informing you that the Parent Online Forms system is ready for 2016-2017 annual check-in. Some important notes about accessing the system:

1) You can register and/or login to the system at http://my.cherrycreekschools.org .

2) Parents who already registered to use the system in prior years will use the same username

and password. Parents can change/reset their password at http://my.cherrycreekschools.org .

3) New parents to CCSD will need to create a username and password to use the system by registering at the Internet address above. To register, parents should have the following information available: Parent First and Last Name, Birthdate and Student ID for at least one of their children, and a Personal Email Address. Spelling for all personal information must match District Admissions. Student ID Numbers are available on school report cards, as well as original Admissions documents.

4) Students and families not yet admitted to Cherry Creek Schools will not have access to the

system.

Cherry Creek Schools requires each family to update their student information. Be aware that updating student forms electronically is only part of the check-in process. Each school has additional steps for each student to complete. Check with your school for more information.

Padres de familia de las escuelas de Cherry Creek: El Distrito escolar de Cherry Creek requiere que las familias actualicen sus datos cada año, incluyendo contactos en caso de emergencia, datos médicos, formularios de aceptación de las políticas escolares y formularios específicos de la escuela. Estos formularios estarán disponibles en línea desde el 1 de Julio del 2016 y son parte del registro anual para el comienzo del año escolar 2016-2017. Los padres que ingresaron esta información el año anterior, pueden ahorrar tiempo ingresando al sistema para revisar, actualizar y guardar la información previamente almacenada. Enclosed is information that will allow us to team together to provide your child with an outstanding educational experience. Please note that most of our check-in information and forms are being provided through the district website http://my.cherrycreekschools.org. Thank you for taking the time to review the material with your child and for providing the necessary information electronically. Eaglecrest is committed to being an exemplary learning community focused on preparing each and every student for college success. A Message from the Counseling Department Welcome back Raptors! We hope you had a wonderful summer and are ready to jump into your new classes and have a fantastic start to your school year! Each year, a great deal of thought, time, energy, and budgeting is involved in creating student schedules. Last February, students were provided with information and counsel regarding course selection. Once courses were selected requests were signed by parents and then reviewed by counselors and department coordinators. Throughout the summer, Eaglecrest’s Registrar and Eaglecrest counselors have spent time rechecking student schedules and making final adjustments when needed. Each of these steps was taken to ensure that every student is placed in appropriately challenging coursework. Due to the amount of time and energy taken to design the best master schedule possible based on the courses student selected, schedule changes in August will not be permitted. While schedule changes are not possible, if there is a legitimate error then students can work with their counselor to request a schedule repair. Schedule repairs must meet one of the conditions listed below. Students can email their counselors about schedule repair concerns or students can drop in for schedule repair questions Monday, August 1st from 8-10 A.M. It is the philosophy and belief of the faculty and administration of Eaglecrest High School that students may be scheduled into classes during all four periods of both A Days and B Days. Please do not request a particular study period. Reasons for needing a schedule repair: • A schedule that does not include a sufficient number of credits (Freshman must have 8, Sophomores must have 7, Juniors must have 6, Seniors must have 5) • A schedule that does not include sufficient core classes (4 academic classes or 3 AP

courses) • A schedule that places the student in the wrong level of a course • A schedule that doesn’t include a course(s) required for graduation (Seniors only)

Why should you pay your Activity Fee? That is a good question because most parents and students are curious about where that $30 goes. The money is divided among Athletics, Activities, and PTO. Some of the benefits of the Activity Fee are:

SECURITY INFORMATION AND INSTRUCTIONS HOURS EHS Security Staff is on site Monday through Friday 6:30 a.m. to 7:00 p.m. throughout the school year (excluding most Cherry Creek School District holidays). Most after-hour events will also be manned, including weekends. Eaglecrest High School ends the academic school day at 2:44 pm. Students involved in SUPERVISED clubs, sports, and academics may remain on school property until the completion of their activity. All other students must be off school property by 3:00 p.m. Students may use the school library for study and research purposes after school while open. BUILDING ACCESS Access to Eaglecrest High School’s main building, mobile classroom, and fields is restricted to Cherry Creek District employees, currently enrolled Eaglecrest High School students, and approved visitors. All visitors are required to check in with valid driver’s license or ID at the main security window prior to continuing to any other part of the property. The main entrance is located on the southwest side of the building. LOCKERS Eaglecrest High School Security issues academic hall lockers to all freshmen during Check–in. Locker numbers and combinations will be printed on the student schedule that will be picked up at check-in in August. SECURITY INFORMATION AND INSTRUCTIONS (pt. 2) PARKING

• Only juniors and seniors are allowed to park on campus. • All EHS student fines need to be paid in full. • Student vehicles must be registered for authorized parking on

campus. • Non-registered vehicles parking on campus or cars in areas other

than those designated as student parking are subject to a $30.00 fine and towing at owner’s expense.

• Driving and parking on campus is a privilege, not a right. This privilege can be revoked if the student fails to operate the vehicle in accordance with state law and school rules. The privilege of parking on EHS school property is extended in exchange for granting consent to the school administration to search the vehicle without warning when the administration has reason to believe that the items contained in the vehicle threaten the general safety of students. To park a vehicle on school property, consent must be given to seize items, which are potentially dangerous in order to protect the common good.

• Parking permits may be purchased online or from the Security Office for $50 per student. The school is not responsible for loss or damage that may occur in the parking lot. The parking fee only allows the student to park on campus.

Dear Parent/Guardian: I would like to take this opportunity to acquaint you with the Cherry Creek School District’s procedures for administering medications to students pursuant to school board policy JLCA, Student Health Services and Requirements. These policies are designed to ensure the safety of our students: 1. The school will have received written permission from the parent/guardian to administer the medication.

When such a request is made by a parent/guardian, a full release from the responsibilities pertaining to side effects or other medical consequences of such medications also must be presented.

2. Medication will be administered by the school nurse, or by school personnel whom the registered nurse has trained and delegated the task of giving such medication. (A parent is always welcome to come to the school and administer medication to his or her own child.)

3. Medication shall be in the original, properly labeled container with the current date printed on the container, and all other parameters required for medication administration (student name, prescribing physician’s name, drug name, dose, route, time and/or frequency).

4. In some instances, at the high school level, students who demonstrate responsibility may be allowed to carry their medications on their person and self-administer such medication in accordance with the current pharmacy-labeled container, or in the case of over-the-counter medication, in accordance with the manufacturer’s label. Students who misuse this privilege may be subject to disciplinary action per applicable school board policies. Please contact your high school nurse for additional information.

The following guidelines shall also pertain to the administration of medication to students in the school setting: Rescue/Prescription medications: (Albuterol and epinephrine) should be kept in the clinic unless written permission for a student to carry these prescription drugs has been agreed to by the physician, parent and school nurse. This written agreement form is located in the school clinic and on the Health Services Web page. It requires, in part, that the student has demonstrated the necessary maturity to keep it on their person. Prescription Medications: Prescription medications require written permission for administration in the school setting from both a healthcare provider and a parent/guardian. Most pharmacies are happy to provide an extra labeled container so that a prescription can be divided and placed in a properly labeled container for school use. If the dose or the directions change, a new form/container must be submitted. Over-the-counter medications: Written parental permission is required for administration of over-the-counter medications. Since many medications resemble another in appearance, medication must be sent to the school in the original manufacturer’s container with the label intact. If a student requires a dosage outside the manufacturer’s recommendations, a healthcare provider’s authorization for the variation is required in addition to the parent’s signature. Since aspirin has been associated with Reyes Syndrome when given to

children with influenza or chickenpox, medications containing aspirin may only be administered with a healthcare provider’s and a parent/guardian’s signature. Homeopathic/Herbal medications: These medications are not considered to be over-the-counter medications, but require written permission for administration in the school setting by both a healthcare provider and the student’s parent/guardian. Some homeopathic preparations may require a review from the Cherry Creek School District Medical Advisory Board. Transportation of medications: Parents/guardians are personally responsible for the delivery of student medications to and from school, and should hand them directly to the nurse, building administrator, or other school official. In the event an approved adult is unable to bring the prescription to school, that individual shall contact the school and notify the nurse or other school official of the date the medication is to arrive, giving the name of the medication and the exact quantity of medication included in the properly labeled container. Any medication sent to the school without the labeled container will be kept in the clinic or main office to be returned to the parent. Additionally, any unused medication will be available for pickup through the end of the school year by the parent/guardian. Any medication not picked up will be discarded. The schools will not send unused medication home with students. Planning ahead: It is recommended that any student requiring a medication at school or at home have a seventy-two (72) hour supply kept in the clinic in the event of an unpredictable local emergency. Please discuss this with your healthcare provider or the school nurse if you are wondering about your child’s need for such an arrangement. Also, because health needs can change, all medication forms will need to be renewed annually. They are available from your health clinic and the District web site at cherrycreekschools.org. Confidentiality of student records. The confidentiality of academic records are protected by the Family Educational Rights to Privacy Act (FERPA). The Health Insurance Portability and Accountability Act (HIPAA) does not apply to schools or the health records maintained within the school nurse’s office. Nursing Procedures: All medical procedures performed during child’s day at school require a “Permission for Nursing Procedure” form to be signed by attending physician and parent/guardian. Procedures requiring this form include but are not limited to tube feeding, tracheostomy care, suctioning and other medical treatments. All nursing procedure forms shall be renewed annually at the beginning of the school year. Individual Health Plans (IHP). The IHP is developed by the professional school nurse to address and communicate the health needs of the student to school personnel with an educational interest in the student. The IHP development and implementation by the RN allows sharing of necessary information and planning for student health needs to maximize student access and participation in their education. All IHPs are renewed annually at the beginning of the school year and as needed in response to a change in student health status. Communications with the school nurse are essential in ensuring your student’s health needs are addressed in the school setting. If your student requires medication, nursing procedures and or an individual health plan call your school nurse the week prior to school beginning to discuss the needs of your student.
