The future of the veterinary profession can be seen studying in the library, learning in classrooms and treating animals in clinics at Iowa State University College of Veterinary Medicine (ISU CVM). These students are the next generation of veterinarians and leaders in our profession. They are the men and women who will be taking over veterinary clinics as the current generation looks forward to passing the torch and enjoying retirement.

Student debt has exploded in just one generation and has become an increasing threat to the veterinary profession. Last year’s veterinary school graduates walked across the stage at graduation with more than 8 times1 the debt than students did in 1983. According to the 2017 AVMA & AAVMC Report on The Market for Veterinary Education2, the mean debt in 2016 for new veterinarians was $159,064. Over the last 16 years, DVM degree debt of all veterinary students has been increasing at about $5,400 per year.

Welcome to February! PRESIDENT’SC O L U M N

I recently returned from the AVMA Veterinary Leadership Conference in Chicago with several other members of the IVMA. It was an excellent opportunity to meet with other veterinary associations from across North America. It also allowed me to get to know several of our very own Power of Ten Leadership (POTL) members. Remarkable individuals with so much potential.

This conference offered numerous discussions and workshops on leadership. “How to Release the Inner Leader in One’s Self” and “How to Develop Leadership in Others”, were just a few of the topics. In Drew Dudley’s keynote address we learned how each of us should have our own leadership philosophy and incorporate it into our day-to-day lives. A few things caught my ear but one that was so simple and straightforward was - when confronted with a situation and you don’t know what to do, simply ask yourself “What would the person I want to be do in this situation?” I think this is so important in our day-to-day lives. We need to let our actions and behaviors show the type of leader, mentor, boss, etc. we all strive to be.

Secondly, I learned about the Zeigarnik Effect. This states that unresolved tasks or projects are a source of mental tension and can actually give us a sense of discomfort, by staying at the forefront of our minds. They then tend to disappear upon completion of the task. We can use the Zeigarnik Effect to prevent procrastination in our day to day duties. Taking the time to complete something as simple as returning an email, finishing a chart, or writing a President’s column can help improve our mental wellbeing.

Finally, we learned about the psychological phenomenon called the Question-Behavior Effect. This effect suggests that asking instead of telling causes a psychological response and the person is more likely to engage in that behavior. When you start a question with the word “will” followed by the action the person has a sense of ownership and tends to be more successful in acting on that behavior. This can be used for eating healthier, going to the gym, etc. I’m sure there are other applications, but I can’t think of any right now.

Oh, before I forget! “Will you be attending the IVMA Legislative Day on February 27th to be an advocate to our profession?”

Winter Conference Speaker Spotlight

Dr. Josh ZachariasEquine Session Wednesday, February 13

Dr. Josh Zacharias is a 2003 graduate of Iowa State University College of Veterinary Medicine. He is originally from Southwest Iowa, raised on a farm. Dr. Zacharias completed an internship at Oakridge Equine Hospital in Oklahoma and then went on to a surgery residency at Purdue University. After an emergency surgery position at ISU, he went to private practice in Texas for 2 years before moving to his current position at Countryside Large Animal in Greeley, Colorado in 2010. He is boarded with the American College of Veterinary Surgery and the American College of Veterinary Sports Medicine and Rehabilitation. Dr. Zacharias has interest in orthopedic and upper respiratory surgery, lameness, and podiatry. He has a diverse caseload in Colorado, including show livestock; however, western performance horse lameness and surgery predominates, with a significant proportion of racing quarter horse orthopedic surgery. Dr. Zacharias has presented at multiple national, state and regional meetings, as well as various webinars. He was honored with an ACVS resident research award and the Veterinary Orthopedic Society Mark S. Bloomberg award.

Dr. Amelia Woolums Bovine Session Thursday, February 14

Dr. Amelia Woolums received her DVM in 1988 from Purdue University. She completed an internship at Kansas State University, a residency at the University of Saskatchewan, and a PhD at UC Davis, where she studied the immune response to bovine respiratory syncytial virus (BRSV) infection. Dr. Woolums is a diplomate of the American College of Veterinary Internal Medicine and the American College of Veterinary Microbiologists. She works in the Department of Pathobiology and Population Medicine in the College of Veterinary Medicine at Mississippi State University as a clinician and teaches in the food animal clinic, and conducts research focused on the host response in bovine respiratory disease, vaccination to prevent BRD, and immunity in cattle and calves.

The Iowa State University Veterinary Diagnostic Laboratory (ISU VDL) continues to encourage practices to improve the overall quality, value, and sustainability of the diagnostic services it provides. We have embraced this thought process by providing tools to streamline and digitize client interaction with the lab. This has manifested as the ISU VDL Client Web Portal, which allows clients to receive results, complete submission forms, access invoices, and print shipping labels all from one convenient website.

Real-Time Results

Many of our clients already utilize our ISU VDL Client Web Portal to access results regardless of the time of day. These results are in real-time with our database so results are available as soon as they are released. This provides convenience for those who operate within tight time constraints. We continue to strive for a more user friendly experience. This includes taking steps to make the portal mobile friendly so that it renders appropriately on mobile devices of all kinds in addition to providing a completely searchable and sortable interface.

Submissions

Online submissions help streamline the submission process by transmitting information directly into the ISU database. The “Submissions” tab includes all of our submission forms, in an easy to access menu, sorted by species.

Invoices

More recently, we have added an “Invoices” tab allowing for those who handle invoices and associated billing to access all their ISU VDL invoices in one place. This tab allows users to sort by status (paid/unpaid), search by time period, and view charges, credits, as well as any outstanding balances. The page also provides the ability to select and download invoices for any bookkeeping needs.

In order to facilitate easier shipping, the ISU VDL recently introduced the “EZ-Ship” program which allows clients to utilize printed UPS shipping labels to ship packages to the ISU VDL from anywhere in the contiguous United States. These labels come prepared with your return address and the ISU VDL shipping address. Charges are not accessed until the package is delivered to the ISU VDL and are placed on the accession invoice (if more than 1 accession is in a box then charges will be split evenly).

YOU can help - $19 in ‘19The IVMA Foundation is asking for your generous assistance to meet goals. A new fundraising campaign, $19 in ’19, is being launched to help build up the scholarship fund and help as many deserving students as possible.

The Foundation is asking EVERY IVMA Member to contribute $19 in 2019 to the IVMA Foundation.

What can $19 do?

If each of the 1570 IVMA members contributed $19 in 2019, the Foundation would have an additional $29,830 to add to the endowment toward future IVMA Scholarships. By increasing the endowment reserves, the Foundation will be able to increase scholarships in both size and number without depleting the reserves of the endowment. The Foundation is working towards increasing funding for recruitment scholarships for incoming freshmen that would be renewable (multiple year) over the course of the student’s study at ISU CVM.

The IVMA Foundation is supported through donations from IVMA members and Foundation Fellows, memorial gifts, scholarships set up by IVMA members, and scholarships created by IVMA sponsors. What can $19 do?

If each of the 1570 IVMA members contributed $19 in 2019, the Foundation would have an additional $29,830 to add to the endowment toward future IVMA Scholarships. By increasing the endowment reserves, the Foundation will be able to increase scholarships in both size and number without depleting the reserves of the endowment. The Foundation is working towards increasing funding for recruitment scholarships for incoming freshmen that would be renewable (multiple year) over the course of the student’s study at ISU CVM.

One of the IVMA’s missions is to advocate for issues affecting the veterinary profession at the state and local levels. In order to shine attention on bills that are introduced to the Legislature that are of interest to Iowa veterinarians, the IVMA sponsors Legislative Day each year. This event allows IVMA members from around the state to visit the Capitol and talk directly with Iowa Legislators.

We need YOU to help make a strong showing of veterinarians. The more IVMA members who participate in this effort, the stronger the voice the IVMA will have.

Advocate for the veterinary profession at the 2019 IVMA Legislative Day on February 27. During this event you will represent the veterinary profession, deepen your understanding of the policymaking process and help shape future legislation.

Hello again! Do you remember how the last wellness article talked about choosing wellness? Well sometimes that becomes more difficult than a ‘choice’ and people can struggle with more than just finding the motivation to choose healthy actions. Sometimes the stress becomes fatiguing or anxiety grows overwhelming. At times the workload and emotional pressures can be intense and individuals don’t realize they’re struggling. These are times when folks need to keep up wellness practices like regular exercise, talking with friends and colleagues, moments of mindfulness and balanced nutrition, yet during those times it can be the most difficult.

It is essential to have self-awareness and consistent self-reflection to ensure staying on track with managing stress effectively and noticing when strain is taking over. It is also important to be aware when colleagues or friends are in distress and know how to respond. An integral part of a healthy workplace culture is an open, non-judgmental atmosphere, social support and providing wellness resources. With the 2018 Merck Animal Health Veterinary Wellbeing Study (Volk, John, et al. 2018) showing low wellbeing for veterinarians, it’s vital that individuals and organizations as a whole work together to ensure health and wellness for all staff. The same study also showed even those with high levels of wellbeing experience depression, anxiety, compassion fatigue or burnout.

Stress can be viewed as cognitive, physical and emotional strain resulting from life demands and pressures.

Burnout is unchecked stress over time; a slow process leading to feeling overwhelmed or disengaged.

Compassion Fatigue is a consequence of stress related with caring for suffering and traumatized people or animals; long term or sudden exposure to trauma as a helping/medical professional. Humans are wired to take on the emotions of

others and people in helping roles where the stakes are high tend to absorb stress and emotional strain before realizing it.

If you or a colleague were experiencing extreme stress, burnout or compassion fatigue, would you know the signs? Would you know how to respond? The key is to Notice, Ask and Refer.

If you have concerns about a colleague it’s important to reach out and speak up. Expressing concern for someone’s emotional wellbeing can feel uncomfortable, but doing so shows empathy and care for them.

Tips on how to approach a distressed colleague: • Choose a place and time where you both feel comfortable. • Begin one-on-one, not in a large room where others can overhear. • Always enter the conversation with a non-judgmental attitude. • Identify the concerning behavior or symptom you’re noticing and offer support. For example, “Jim I noticed you’ve been pretty worried about a few patients lately and have talked about having trouble sleeping. I’m wondering if you want to talk.” Or “Laura, you told me you’ve been feeling tense lately and I noticed you seemed upset with your client this morning. What are you experiencing?” or “Hey, I’ve noticed you seem more tired lately. How are you doing?” • Practice active listening and don’t assume to know what they’re feeling. We can all experience the same stressors differently. • Consider asking about suicidal thoughts or intent. It’s best to be direct and ask rather than miss the opportunity to provide help. • Know your mental health and wellness resources or offer to seek them out together if needed.

February is National Pet Dental Health Month and a great time to remind your clients the importance of proper dental health for their pets and its link to overall good pet health.

Here are several ideas of how to promote dental care in your clinic:

• Run a special in your clinic for dental cleanings during February; • Host a tooth brushing demonstration, either in your clinic or at a community center such as a town hall or library; • Offer to present a public program on dental care for pets at your local library, town hall or youth/service club; • Spread the word on social media about National pet Dental Health Month and include tips on how to brush pets’ teeth. Invite the public to submit photos of their pets’ “pearly whites” and post them on your social medial pages; • Print and display the free clinic flyer about Pet Dental care found on the AVMA’s website (link below); • Hand out the Pet Dental Care Brochure available at the AVMA website (link below).

Bring Back Inactive Clients with the FREE Ready. Set. Go2! Inactive Client Initiative

As you may know, Iowa Veterinary Medical Association is a proud Associate Member of Partners for Healthy Pets (PHP) and we are very pleased to announce another program between PHP, Veterinary Medical Association Executives (VMAE) and our association. We believe this initiative, just like last year’s Forward Booking program, will have a significant impact on the health of your patients and your practice.

This collaborative program focuses on the importance of re-engaging Inactive Clients and bringing them back into the veterinary practice. This is not your typical reminder program! First, it is based upon a client’s visitation history and not a timetable for a specific service. Second, a key difference is in the messaging itself. PHP’s unique empathetic messaging consists of three compelling points: recognizing that pet owners are busy but want to provide the best care for their pets, expressing sincere concern from the practice for their pets’ health, and conveying that an annual exam is as important as food and love. This program does not replace, but is actually complementary to, your current reminder program.

Not convinced? During 2015-2016, 1,612 practices enrolled in PHP’s Inactive Client Reminder Program, which resulted in reactivating an average of 128 clients per practice (many of whom had been absent for two to three years). This led to healthier pets and healthier practices! 

The even better news is that this program is incredibly easy to utilize as it involves no staff training or changes to your practice team protocols. You need only to assign one or two staff members to identify lapsed clients and the rest is as easy as 1, 2, 3!

STEP ONE: Identify inactive clients.

STEP TWO: Email those clients with PHP’s proven and ready-to-use compassionate messaging.

STEP THREE: Start booking appointments!

Yes, pre-exposure prophylaxis is recommended for this population due to their frequent contact with potentially rabid animals.

Pre-exposure vaccination does not eliminate the need for additional medical evaluation after a rabies exposure, but simplifies management by eliminating the need for rabies immune globulin and decreases the number of doses of vaccine needed. Pre-exposure prophylaxis might offer partial immunity to persons whose post-exposure prophylaxis is delayed and might provide some protection to persons at risk for unrecognized exposures to rabies.

Q: Why is pre-exposure prophylaxis recommended?

A: Three 1.0 ml injections of vaccine are given intramuscularly on day 0, 7, and 21 OR 28. Two FDA approved rabies vaccines are available for use in people: RabAvert manufactured by Novartis and Imovax manufactured by Sanofi Pasteur.

A: Veterinarians and veterinary staff with direct animal contact who have received pre-exposure prophylaxis should have a serum sample tested for rabies antibody every 2 years. If their antibody level is less than complete neutralization at a 1:5 serum dilution by the Rapid Fluorescent Focus Inhibition Test (RFFIT), the person should receive a single booster dose of rabies vaccine.

Iowa Department of Public Health is not aware of any programs to defray the cost of pre-exposure prophylaxis. Health insurance occasionally covers the cost of pre-exposure prophylaxis; contact your insurance provider for additional information on what costs they will and will not cover.

Human healthcare providers can order human rabies vaccine. If there is an established healthcare provider who addresses workers compensation issues for the clinic, consider contacting that person. Rabies vaccine is not usually administered at local public health departments in Iowa. In certain situations, some pharmacies can administer rabies vaccine.

Pre-exposure vaccination does NOT eliminate the need for prompt post-exposure prophylaxis. If persons who have completed the pre-exposure vaccination series (after 1980) are subsequently exposed to rabies, the following protocol should be followed: two IM doses (1.0 ml each) of vaccine, on days 0 and 3. Human Rabies Immune Globulin should NOT be administered.

If the pre-exposure series was administered prior to 1980, contact the Iowa Department of Public Health for additional consultation- during business hours call 800-362-2736 and after hours call 515-323-4360 (this will connect you with the state patrol who will page the on-call epidemiologist).
