The Piedmont Regional Dental Clinic (PRDC) is closing its doors to the public on Veterans Day, Wednesday, November 11, 2015, to provide free dental care to low income Virginia veterans from 8am to 5pm. Veterans will receive oral cancer and blood pressure screenings as well as their choice of an extraction, a filling or a cleaning at no cost as our way of saying thank you for your service to our country. To qualify for free dental care on November 11th you must: • Be a veteran of the US uniformed services (bring your DD Form 214 or prior versions of discharge papers, Virginia Veterans ID Card, Retiree Military ID Card or Veterans Administration ID Card to your appointment) • Have no dental insurance coverage • Have a household income at or below 200% of the federal poverty level Bring proof of ALL household income for one month (this includes anyone living in the household with you). For example pay stubs for one month, food stamp award letter, proof of income letter, Social Security statement, disability statement or current 1040 Federal Income Tax Statement, and we’ll help you figure it out. • Show photo ID • Make an appointment ahead of time by calling PRDC at (540) 661-0008 H MAKING AN APPOINTMENT IS IMPORTANT H Space is limited and appointments are going fast so call PRDC today!
