Transforming health care through better information

The recovering patient you greeted, the neighbor you discharged, the anxious mother you comforted—they are the reasons for OneCare. And the reasons include each of us, too. Because OneCare will help us make good on our commitment to help every community we serve be a healthier community.

OneCare is all about providing excellent care every day with a shared, universal electronic health record for each patient. Patient records will be available across sites of care and will provide physicians and staff access to information to offer the best care possible.

“This is one of the most expensive and complex but vitally important programs that we will have undertaken as One CHI. But if we give our caregivers the tools they need, and our patients the care they are entitled to, it will be a journey worth taking together.” –EugeneWoods,CHISVPOperationsandCEO

With OneCare, gone will be the days of rescheduling appointmentsduetomissingpaperworkorfiles.Andpatient safety and clinical outcomes will improve.

Imagine your patient, Jimmy, injured on the job. Heneedsx-rays,surgery,post-operativetherapyanda series of follow-up appointments. All along Jimmy’s care journey, his physicians and nurses will have access to every single test result completed, procedure done and care given.

Powered by technology

Designed by the people who deliver the care and powered by information technology, OneCare is being developed using leading-edge software and evidence-based content to advance patient care through information.

“Right now our many systems, electronic and paper, cannot be coordinated or accessed easily or efficiently. When our electronic health record is implemented, CHI will be one of the first health systems of our size to have a common, standardized electronic health record regardless of location.” –JosephWilczek,CHISVPOperationsandCEO

OneCare has three components: Ambulatory Care, HospitalPlusandAccessPlus.Eachcomponentaddresses the unique needs of each provider as well as each patient.

“With OneCare, we are going to be able to provide our caregivers with the information they need to meet our mission and our vision, and to give the best possible care we can to every single patient who walks through our doors.” –KathleenSanford,RN,CHIChiefNursingOfficer

AMBULATORY CARE The Ambulatory Care initiative will be implemented in physician practices through 2014. This includes: • Ambulatory Electronic Health Record (AEHR)—

a digital patient health record that will be shared across clinics and care teams

• Physician Practice Management System (PPMS)—

• Oncology Electronic Health Record (OEHR)—universalelectronichealthrecordspecifictocancer patients in clinic and radiation therapy settings, linked to cancer registries, cancer research and survivorship programs

“We will be able to move information across the care continuum. Something we’ve

dreamed about as clinicians for many, many years will now be available to us.”

—Beth O’Brien, RN, CHI SVP Operations

HOSPITAL PLUS For the Hospital Plus initiative, design teams of nurses, clinicians and physicians across CHI are developing clinical content, documentation standards and computerized physician order entry (CPOE) that will be used to build advanced electronic health records for all CHI hospitals using either Meditech 6.0 or Cerner.

Meditech 6.0 will be implemented in 29 hospitals and will include administrative, patient billing and general financial applications as well as: • Nursing and physician documentation in all

patient care areas including the emergency department, operating room, and laboratory, radiology and other ancillary departments

• Medication management including bar-coded medication administration (BCMA)

• Electronic prescribing • Computerized physician order entry (CPOE) • Scanning-archiving and data repository • Standard interface package and report packages

Cerner will be implemented for clinical functions in five MBOs for a total of 16 hospitals. Cerner implementation will include: • Nursing and physician documentation in all

patient care areas including the emergency department, operating room, and laboratory, radiology and other ancillary departments

• Medication management including bar-coded medication administration (BCMA)

• Electronic prescribing • Computerized physician order entry (CPOE)

These advanced and comprehensive electronic health records will ensure the delivery of safe, quality patient care that is streamlined and efficient for all users.
