Gallup poll asking the public to rate professions based on their honesty and ethical standards.

Gallup, Nov. 26-29, 2012

The British Medical Journal (BMJ) has asserted through a number of articles (Noakes, TD. Commentary: Role of hydration in health and exercise, BMJ 2012; 345:e4171; Pitiladis, Y, Beis, L. To drink or not to drink recommendations: The Evidence, BMJ 2012; 345:e4868; Cohen, D. The truth about sports drinks, BMJ 2012; 345:e4737); that use of sports drinks during exercise are NOT supported by evidence and that there is little evidence that dehydration is an important contributor to heat related illness or that fluid losses during competition have an adverse effect on performance.

Attempts by athletes to keep up with fluid losses, or even “stay ahead of thirst,” are counterproductive and potentially dangerous as even a 2% surplus of total body water

can lead to edema, whereas greater increases can lead to hyponatremic encephalopathy and death. But Doug Casa, Ph.D. (Casa, DJ; Stearns, RL; Lopez, RM; et al, influence of hydration on physiological function and performance during trial running in the heat; J Athl Train 2010 Mar-Apr; 45(2): 147-56) felt “the BMJ article did not clearly state that during intense exercise in the heat, hydration can help keep your temperature down.” But again, are sugared sports drinks as easily assimilated during intense exercise, as just plain water?

In a related article on non-Nutritive Sweeteners (NNS); are they as good as we would like to think? In a rigorous review by Mattes and Popkin (NNS consumption in humans: Effects on appetite and food intake and their putative mechanisms. Am J Clin. Nutr. 2009; 89:1-14) it was concluded that “none of the 9 plausible theories of an adverse effect of NNS are currently supported by available evidence” (Christopher Gardener, Ph.D.).

However, using or choosing a food or beverage with a NNS may be healthier but not healthy, ie: “why isn’t water being selected over diet or regular soda?” (Gardener). As stated in the 2012 AHA Scientific Statement (American Heart Association): “The evidence reviewed suggests that when used judiciously, NNS could facilitate reductions in added sugars intake, thereby resulting in decreased total energy and weight loss/weight control and promoting

Five Takeaways From the BMJ Sports Drink Series from Kevin Lomangino, Clinical Nutrition Insight, Sept. 2012

The BMJ takedown of sports drinks timed to coincide with the flurry of marketing surrounding the Olympic games in London, shines a welcome light on the disconnect between the billions in sales these products generate and the limited evidence that supports them. What was missing, however, is an important element of sound evidence-based analysis: perspective from someone who does not share the authors’ interpretation of the data. We tried to add that nuance to Clinical Nutrition Insight’s coverage, especially when it comes to the controversial relationship between hydration and heat illness during intense exercise in hot conditions. But readers should not lose sight of some very important conclusions of the BMJ series that bear repeating:l There is no evidence that an average exerciser benefits from sports drinks as opposed to water.l Thirst is an adequate guide to hydration needs under most circumstances.l Excessive emphasis on hydration may promote hyponatremia, an increasingly prevalent phenomenon at

endurance events with serious and potentially fatal consequences.l Modest dehydration (2-4% loss in body weight) does not seem to be harmful under most conditions and might

actually be beneficial in some competitive situations.l The only people who are likely to benefit from sports drinks are trained athletes during extended endurance

exercise.

Eastern Shore Hosts First Mission of Mercy EventMarch 15-16, 2013

More than 100 million people in the United States are without dental insurance. On the Eastern Shore there are thousands of adults who don’t receive adequate or even basic needed dental care, resulting in a silent epidemic of dental and oral disease. That’s why the Mission of Mercy is so important!

Mission of Mercy is a free adult dental clinic provided by volunteer dental professionals and a volunteer staff of several hundred people.

Try to visualize the typical Mission of Mercy (MOM) patient. Usually the first thing that comes to mind is a homeless person or family, someone on government assistance, unable to get needed dental care for obvious reasons. However, that first mental picture can’t be any further from the truth. The people in need include the average person mingling and socializing among us. They are friends, coworkers, people we interact with on a daily basis. They could be your grandparent, mother, father, sibling or any other relative. With the tough economic times we are in, it could be anyone. It could be you.

This Mission of Mercy event is scheduled for March 15-16, 2013 in Salisbury. This will be the first of its kind on the Eastern Shore and to make it a success, volunteers, both dental and general, are needed.

There are many ways you can volunteer in the Mission of Mercy. We need your energy, your time, your skills and talents:• 100+ volunteer oral health professionals• 200+ lay volunteers • Donations of supplies and food• Financial donations• Support from the Eastern Shore community• Support from Eastern Shore service organizations

For more information, or to sign up to volunteer, please visit us at: www.easternshoremissionofmercy.org F

able to provide her. She did so by speaking at the House of Delegates meeting in Ocean City, to express her gratitude directly.

If you would like to help us continue supporting the dental community, please put a check in the little box next to “Charitable and Educational Foundation” on your dues invoice. If you would like to make a larger donation, you may write in any amount, or you may call us at 410.964.2880.

We hope you had a great holiday, and we thank you for helping others do the same! F

The Dentist Well-Being CommitteeThe Dentist Well-Being Committee is a continuing committee of the MSDA. The Committee assists dentists and their

families who may be experiencing personal problems. The Committee has helped many dentists over the years with problems such as stress, drug dependency, alcoholism, depression, medical problems, infectious diseases, neurological disorders and other illnesses that cause impairment.

Many in our profession are unaware that our committee is not just here to help dentists. Our services are available for all members of the dental team, including dental assistants and radiation technologists, as well as for dental students. Dental hygienists in Maryland have their own Dental Hygienist’s Well-Being Committee, which is available to assist hygienists, assistants, and radiation technologists.

The Committee provides a confidential, non-disciplinary avenue for dental team members to seek appropriate help for problems. Referrals are completely confidential. Any dental staff member who seeks help as a self-referral can be assured that their information will be held in the strictest confidence.
