PREFACE

This book consisting of clinical case studies ofimmunologically-related diseases is a companion volumeto “Immunology, A Short Course” by Coico and Sunshine.The idea for the book grew out of our belief that theunderstanding of the complicated and sometimes confusingsubject of immunology would be greatly facilitated bya study of the diseases of the immune system in whichnormal immune processes go awry. This view wasstrengthened by the complementary belief that the diseasesof the immune system can be best understood through afirm grasp of normal immune mechanisms. The editorsand authors have therefore created reality-based casesof typical patients with immunologic diseases and leadthe reader through a detailed and logical clinical coursebased on both standard of care medical principles and athorough knowledge of the underlying immunologic defect.

In addition, each case is accompanied by an in-depthdiscussion of the normal and pathological immunologicalprocesses related to the disease state being discussed. Ineffect, a careful reading of the cases as a whole providesthe reader with an alternative course in immunologythat amplifies and complements the various sections ofthe companion “Short Course.” Overall, we hope thissomewhat unique way of learning about immunology andits associated diseases will offer an effective path to themastery of a difficult subject and, at the same time, deepenyour knowledge of clinical medicine.

This book is designed to pique your interest in and increaseyour understanding of the immune system in normal indi-viduals by showing you what happens when it goes awryin disease states. This book contains a series of clinicalvignettes. Each is based on one or more real patients suffer-ing from one of the main types of immunologic disorders.You follow each patient through the initial workup and lab-oratory studies to observe how the diagnosis and treatmentplan are developed. You then follow the patient’s subse-quent course to learn how the disease typically evolves and,at times, how it requires changes in therapy.

Each clinical story has been calibrated to illustrate themain features of the disease under study and to discuss thedisorders of normal function that result in these features.However, the disease manifestations presenting in a singlepatient usually do not encompass all of the manifestationsof a particular disease entity that can occur in a large num-ber of patients. Thus, where appropriate, we have includeda section describing the characteristics of the disease in theentire patient population. Finally, each vignette morphs intoa discussion of the normal immune system and the defectsthat have led to disease, that is, the mechanism of the dis-ease as we now understand it. This provides a platform fordiscussion of many of the major immunologic processesthat collectively make up the immune system.

You will encounter several recurrent themes as youread the vignettes. One theme that will become apparentto you is that immunologic disease of almost any type hassome genetic basis. This is quite obvious in the congenitalimmunodeficiency diseases that are most often due to a sin-gle gene mutation. This is also true of autoimmune diseaseand malignant disease; however, in these cases the diseasesare multigenic. It is apparent that the genetic abnormali-ties associated with autoimmune diseases consist of genepolymorphisms that are also present in large numbers ofnormal individuals. In order to give rise to disease, thesepolymorphisms must be present with other, additional genepolymorphisms and the genetic abnormalities in autoim-munity actually consist of multiple polymorphisms actingin tandem. Thus, one of the key challenges of researchinto the cause of multigenic diseases is to understand howeach of the component polymorphisms contributes to apathologic change in immune function. In malignant trans-formations of cells of the immune system, the neoplasm isthe result of mutations in one or more key genes, usuallyinvolved in cell cycle control and thus affecting cell pro-liferation. The effects of these mutations combine with theindividual’s inherent (genetic) ability to control that pro-liferation. In malignant disease, in contrast to the situationin immunodeficiency or autoimmunity, the mutation is asomatic mutation that occurs in a very limited number of

at least partially differentiated cells in the body. In immun-odeficiency or autoimmune diseases, the genetic mutationor genetic susceptibility polymorphism is inherited in thegermline and is therefore present in all the body’s cells.

A second theme emerges from the inherent complexityof the immune response. Since the immune system consistsof an astonishing array of different cell types and subtypes,understanding immune processes often involves dissectingthe functions of each of these cells as well as their complexinteractions. In studying immunologic diseases, one findsagain and again that the key to understanding the mecha-nisms of immunologic disorders is to identify the specificcell subtypes initiating disease and how each one functionsin the normal immune response as well as in the diseasestate. A related theme is really a testament to the econ-omy of cell biology in general in that cells of the immunesystem with different phenotypes and functions often useidentical intracellular signaling pathways and transcriptioncontrol mechanisms. Therefore, characterization of diseasesthat affect many different cell types frequently leads tothe identification of abnormalities of intracellular molecularprocesses occurring in those cells.

Our third theme, and one that should give clinicianspause in their approach to treating patients, evolves fromthe realization that the immune system is finely tuned toact as a totality. Therefore, any breakdown or manipu-lation of one function of the immune system inevitablyaffects another immune function. A consequence of thisis evidenced in the fact that a deficiency in one arm ofthe immune system may lead to dysregulation of anotherarm, resulting in an overexuberant response against selfor autoimmune disorder or, alternatively, even allowingthe proliferation of mutated cells (neoplasms). One shouldnot be surprised therefore to learn that seemingly disparatedisease manifestations—immunodeficiency, autoimmunity,and malignancy—not infrequently appear together in thesame patient.

The first section of this book (Unit I) is devoted toimmunodeficiency diseases. In most cases these are congen-ital immunodeficiency diseases. In that primary immunode-ficiency diseases involve, for the most part, a defect in a dis-crete component of the immune system, they have providedimmunologists with an unparalleled opportunity to studyand understand the function of these discrete components inhumans. These diseases, some uncommon and some rare,are true “experiments of nature,” and by learning aboutthem you will enhance your understanding of the normaldevelopment of the immune system. This section also con-

tains a chapter devoted to one of the most important sec-ondary immunodeficiency disorders of our time, acquiredimmunodeficiency syndrome (AIDS). Analysis of AIDS,a disease in which the immune defect develops predom-inantly from the elimination of mature cells, has allowedimmunologists to assess the immune system as a totality inwhich most of the immune system becomes dysfunctional.In this book we follow in the footsteps of these immunol-ogists in that we use these various immunodeficiencies toteach about the specific as well as the general functions ofthe human immune system.

Units II (Immediate Hypersensitivity and Mast CellDisorders) and III (Autoimmune Disorders) illustrate theconsequences of dysregulation of the immune systemwhether due to responses to outside (exogenous) agentssuch as allergens or infectious organisms, to internalself-antigens as a result of the breakdown of tolerance, orto a combination of the two.

Lastly, Unit IV Malignancies of the Lymphoid Sys-tem, demonstrates what happens when the internal cellularcontrols of lymphocytes fail to properly regulate their lifecycle. Here again it becomes apparent that the inability ofeven a subset of lymphocytes to function normally (becauseof malignant transformation) leads to poor regulation of theremaining nonneoplastic cells, often resulting in the clini-cal sequelae of immunodeficiency (infection) and loss oftolerance (autoimmunity).

We envision this book to be used at a variety of lev-els based on the background and needs of the reader. Thepresentation in most chapters is based on two current meth-ods of medical school teaching. One is block learning, inwhich all aspects of an organ system or disease are stud-ied together: pathophysiology, epidemiology, biochemistry,and treatment or pharmacology. The second is the methodof case-based learning, in which students take an active rolein dissecting, learning, and teaching a case. In many waysthis mimics real life, in which you analyze the problem infront of you, be it in diagnosing and treating a patient orin developing a research project to better understand a dis-ease. By incorporating into each chapter up-to-date researchand theories and freely admitting what is unknown, thereader can easily appreciate the uncharted intellectual terri-tory ahead and the possibility of personally contributing tonew knowledge through further research. We therefore hopethat this book will motivate a new generation of scientiststo study immunology through the lens of disease. Enjoy!

INTRODUCTION

This first unit examines several inherited immunodeficiencydiseases as well as the acquired immunodeficiency syn-drome (AIDS). As stated in the introduction, the studyof congenital immunodeficiency diseases (and their ani-mal models) has provided scientists with an unparalleledopportunity to define the function of the individual com-ponents of the immune system. Such an opportunity wasin fact encountered by immunologists in the 1960s and1970s who were conducting the initial series of investi-gations of patients with X-linked or Bruton’s agamma-globulinemia (Case 1), severe combined immunodeficiencydiseases (SCID; Case 2), and DiGeorge syndrome (Case3). In analyzing children with these congenital immunode-ficiency disorders and in the development of animal modelswith similar findings, it became apparent to immunologiststhat the adaptive immune system consists of two main com-partments, one populated by T lymphocytes and the otherby B lymphocytes. In this way, one of the main organiz-ing principles of modern immunology became established.Students approaching the study of these rare diseases areencouraged to adopt the approach of the early investigatorsin realizing that a thorough understanding of these diseaseswill lead to an in-depth understanding of the immune sys-tem as a whole.

GENERAL PRINCIPLES IN THEWORKUP OF IMMUNODEFICIENCY

The following description of the differential diagnosisand workup of patients with possible immunodeficiencyis based on broad principles concerning organization ofthe immune system as well as specific approaches toevaluation of immune function. Nevertheless, it is notdesigned to give you computer algorithms into whichyou can simply plug in data and arrive at a correctdiagnosis. Much of what is known in medicine, andin immunology in particular, would never have beendiscovered if physicians had restricted themselves toprescribed algorithms. However, orderly thinking and aframework for problem solving are needed as an approachto understanding these patients, along with the realizationthat each patient is sufficiently unique that arrival at adiagnosis will usually require some creative thinking.

Primary and Secondary ImmunodeficiencyDiseases

Immunodeficiency diseases may be divided into twocategories: primary and secondary. In primary immunode-ficiency diseases , which are either inherited or acquired,the immunodeficiency is the underlying abnormality.

Secondary immunodeficiency disorders are those thatarise from a nonimmunologic abnormality that hascollateral effects on the immune system. This distinctionthus separates inherited immunodeficiencies and theirusual complication, recurrent infection, from the infectiousdisease caused by the human immunodeficiency virus(HIV), in which the infection itself is the cause of theimmunodeficient state. It should be borne in mind thatprimary immunodeficiency diseases are infrequent (withone or two exceptions) and thus one must always searchfor a nonimmunologic cause of frequent infections; forexample, one must rule out secondary immunodeficiencybefore an expensive and difficult immunologic workup isundertaken.

Whether primary or secondary, the immunodeficiency,almost by definition, will lead to impaired host defense andtherefore increased susceptibility to infection. This leadsto the axiom that an immunodeficiency disease should besuspected in any patient with recurrent infections who doesnot have predisposing factors and should particularly besuspected in a patient with unusual infections.

Two Systems

The first level of characterization of immunodeficiency dis-orders can be based on which of the two major branchesof the immune system is defective: the innate immune sys-tem or the adaptive immune system. The approach to theevaluation of these systems may differ considerably.

Innate Immune System. Our innate immune sys-tem starts with the physical or anatomic barriers that protectus from the outside world. These include our skin andepithelial barriers and protective secretory molecules suchas mucus. We will not discuss these in this section but dotouch upon them in the section on autoimmune diseases(see Unit III).

After breaching these physical barriers, the body standsready to respond to insult with antigen-nonspecific cellsand molecules that do not require prior sensitization orhave memory. The former include granulocytes (predomi-nantly neutrophils and eosinophils), macrophages, and nat-ural killer (NK) cells whereas the latter include the com-plex of molecules comprising the complement system andsecreted molecules such as lactoferrin and defensins. Thecells of our innate immune system have evolved from sim-ilar cells found in lower organisms: phagocytes are presentin species as primitive as sponges and sea anemones. How-ever, in contrast to these organisms, our innate immuneelements have a dual role. On one hand, they mount adefensive response by themselves while allowing the bodythe time to generate a specific adaptive immune responseto the challenge; on the other hand, they interact with andfacilitate the response of the adaptive immune system inboth its afferent and efferent phases.

In recent years it has been discovered that the cellsof the innate immune system have an elaborate systemof receptors with which they recognize and respondto categories of molecules associated with potentialpathogens. These molecules, called pathogen-associatedmolecular patterns (PAMPs), are general molecularsignatures of classes of organisms identifying the type ofinfectious agent (e.g., bacteria, parasite) invading the body.They are recognized by pattern recognition moleculeswhich serve as receptors for the PAMPs. The most widelystudied of this group of receptors are the Toll-like receptors(TLRs) of the innate immune system. These receptorsrecognize and mount responses to the prototypic moleculesassociated with both pathogens and commensal organisms.Signaling through these receptors initiates activationof intracellular signaling, stimulation of phagocytosis,secretion of small molecules (particularly cytokines of theinterferon family), and other innate immune responses.These responses provide host defense on their own andcommunicate with the adaptive immune system.

Adaptive Immune System. Our adaptive immunesystem is designed to respond to a specific challenge, usu-ally that of a foreign organism. In addition, it has memoryfor that antigenic challenge, a property lacking in the innateimmune system. Immune memory allows a more rapid andeffective response on second encounter with the organism.The adaptive immune system may be subdivided into twoarms, the humoral immune response and the cell-mediatedimmune response (see below).

Two Arms of Adaptive Immunity

The two arms constituting adaptive immunity arethe humoral or antibody immune response and thecell-mediated immune response. The final effector cellof the humoral response is the B cell or its differentiatedcounterpart, the plasma cell, and the product of thesecells is immunoglobulin or antibody. In contrast, thecell-mediated immune response is mediated in its finalstep by one of the many types of T cells. In actuality,interactions among different cell types are required forboth responses, and these include cells of the innateimmune system such as dendritic cells and macrophages,which serve as antigen-presenting cells. Of course, as wewill see, the humoral immune response, a prototypic B-cellresponse, reaches full capability only with assistance(help) provided by T cells. In addition to direct cell–cellinteraction, much of the communication among cellsis achieved through the local release of small proteinmolecules, cytokines and chemokines, which are productsof cells of both the innate and adaptive immune systems.

Immunodeficiency diseases (Table UI.1) can be cate-gorized on the basis of the branch that is defective: theinnate immune system or the adaptive immune system. For

the innate immune system the possible defective compo-nents include (1) nonspecific effector cells, for example,phagocytic cells, including defects in their ability to migrateto sites of infection, the TLR signaling pathway, or theirsoluble products of defense and (2) complement compo-nents. For the adaptive immune system the defect may liewithin the (3) T-cell or cell-mediated immune response,(4) B-cell or antibody-mediated immune response, or (5)both T and B cells or combined cell-mediated and humoralimmunity. Mutations resulting in the defective function-ing of cytokines and chemokines (the protein molecules bywhich the cells communicate) and/or their receptors mayunderlie some of these diseases and are therefore incorpo-rated into the above listed categories based on which celllineage is dependent on these molecules for cell differenti-ation and function.

The above categorization of immunodeficiency is sim-plistic since it does not fully incorporate the fundamentalreality that the immune system has an elaborate network ofintercommunication as well as some redundancy in the useof small molecules. For example, a “pure” T-cell defectmay affect both the humoral and cell-mediated immuneresponses. Even both the innate and adaptive immune sys-tems can be perturbed by a deficiency in a single molecule.This cross–talk and interdependence are part of what drivesthe need for an orderly approach to the analysis of thepatient with possible immunodeficiency.

PRACTICAL APPROACH TO THEWORKUP OF IMMUNODEFICIENCYPATIENTS

History and Physical Examination

When a patient presents with recurrent and/or unusual infec-tions, the types of infections usually govern the approachto analysis. However, the details of the history and physicalexamination are of paramount importance in the inves-tigation of possible immunodeficiency. The obvious firstconsideration is the patient’s age. Many of the more severeimmunodeficiency diseases are congenital (present at birth)and/or genetic (inherited), and patients present as infantsor young children. Other primary immunodeficiencies areinfluenced by environmental factors and may thus becomeevident at a later age (teens or twenties) for unclear reasons.Of course, secondary immunodeficiencies are the conse-quence of non–immune system abnormalities and their ageat presentation is dictated by the underlying disease.

Another age-related distinction arises from the fact thatthe infectious consequences of humoral immunodeficienciesdo not become apparent until after six months of age whenthe maternal antibodies protecting the newborn disappear.On the other hand, cell-mediated immunodeficiencies willpresent at birth or very soon thereafter and may result in

failure to thrive. Finally, patients with defects in phagocytesor the complement system may present at any age.

Given the fact that most primary immunodeficiencieshave a genetic component, a family history is anotherimportant element in the analysis of these diseases. Arelevant family history may reinforce the perception thatthis child is having more than his or her share of the usualchildhood infections and may also suggest a pattern ofinheritance, either autosomal or X-linked. Questions aboutearly deaths in an extended family, particularly those fromrepeated infections, need to be specifically asked sincean inherited immunodeficiency disorder may have gonepreviously unrecognized.

An essential aspect of the patient’s history is the pat-tern (location and type) of infections that the individualhas experienced since this will help identify the particu-lar arm of the immune system which would be required forhost defense against that category of organism (Table UI.2).In particular, it is critical to know if the patient has beensubject primarily to bacterial infections or to viral, proto-zoan, and fungal infections. Patients with recurrent infec-tions caused by pyogenic (pus-forming) bacteria, usuallytargeting the respiratory or gastrointestinal tract, will mostoften be suffering from a B-cell or humoral immune abnor-mality, since the primary means of fighting extracellularbacterial organisms rests with the synthesis of specific anti-bodies to that organism. By contrast, patients with viral,protozoan, and fungal infections are more likely to have aT-cell-mediated immunodeficiency since T cells are the pri-mary defenders against intracellular organisms and are alsorequired for defense against large fungal organisms. Theseinclude opportunistic infections , the clinical consequencesof organisms that do not cause disease in immunologi-cally intact individuals. Finally, recurrent skin infectionsand impaired wound healing will be more suggestive of aphagocytic cell deficiency, whereas systemic infection withencapsulated organisms and autoimmunity in older individ-uals will prompt consideration of a complement deficiency(Table UI.2).

The physical examination of the patient will likelyyield two pieces of information. First, it will direct yourattention to the presence of a currently ongoing acute orchronic infection. In addition, examination may provideevidence of preceding infections, that is, their location,severity or frequency, and consequences, which the patient(or in the case of children, the patient’s parents) mayhave forgotten to mention. For example, repeated severeear infections (otitis media) can result in scarring ofthe tympanic membrane. Second, a complete physicalexamination will provide information on possible structuralabnormalities and their underlying causes. The sizes of thesecondary lymphoid organs—the lymph nodes, spleen,and tonsils—are of particular importance. A clinical“pearl” is the observation of underdeveloped tonsils inany patient inherently incapable of generating germinal

centers (the structure necessary for high-affinity antibodyformation). The presence of other anomalies, such ascraniofacial or cardiac malformations or abnormalities ofpigmentation, will suggest certain associated immuno-deficiency syndromes. Failure to thrive, a symptomparticularly prominent in T-cell defects, can be determinedsimply from the infant’s pattern of growth in height andweight.

While a thorough history and physical examination arecritical in suggesting and guiding evaluation of an immuno-deficiency syndrome, in the final analysis, with a suspicionof an immunodeficiency disorder based on recurrent and/orunusual infections, a laboratory workup of the patient mustbe undertaken to confirm and identify the immune defect.

During the development of humoral immunity, themajor effector cell, the B cell, synthesizes its product,immunoglobulin (antibody), in response to antigenassociated with pathogenic organisms, particularlyencapsulated bacteria. In contrast, during the developmentof cell-mediated immunity, the T cell is the major effectorcell, either as a CD4+ T cell (acting through the releaseof cytokines or via cell–cell interactions) or as a CD8+cytotoxic T cell that will lyse virally infected targetcells. However, it is important to remember that both

types of immune responses depend on T helper cellsand antigen-presenting cells such as macrophages ordendritic cells for the generation of the effector cells. Evencomplement components, which act as chemoattractants,opsonins, and as part of the antibody-mediated lyticprocess, impact the humoral and cell-mediated immuneresponses. In practice, therefore, one starts the evaluationof the patient with quantitating the final product or celltype in each arm of the immune system, evaluating themfunctionally and then, if necessary, working backward inthe activation and developmental scheme until the blockor deficiency is identified. In patients with combinedimmunodeficiencies (both humoral and cell mediated), theblock could occur in a mechanism early in developmentand be common to both T and B cells or it could simply bepresent in T cells and interfere with help in both antibodyproduction and cytotoxic T-cell maturation.

Quantitative Assays. The simplest initial test isthe quantitation of products and cells that are present inthe peripheral blood. Even when the clinical presentationpoints to a specific arm of the immune system, it is commonto perform the simplest quantitative tests to establish thatthose parameters are normal in the patient. We can assigneach quantitative test to an arm of the immune system asfollows:

Cellular Composition of Peripheral Blood. Thefirst step in such quantitation is a complete blood count(CBC), which results in the quantitation of the white bloodcell types (differential count) present in the circulation;neutrophils, lymphocytes, monocytes, eosinophils, andbasophils.

Further quantitation of immune cells in the bloodinvolves analysis of the lymphocyte subsets by flowcytometry (see Unit V for assay descriptions) to determinethe number and percentage of mature CD4+ T cells,CD8+ T cells, B cells, NK cells, and other cellular subsetsin the circulation. This analysis is based on expressionof characteristic cell surface markers (CD: clusters ofdifferentiation) and is usually undertaken even if thenumber of lymphocytes in the peripheral blood is normal.

Immune-System Related Molecules in Periph-eral Blood. Quantitation of the secreted protein productsthat are essential for immune function is also conductedon peripheral blood samples. For suspected B-cell defi-ciencies, assays are performed to determine the levels oftotal serum immunoglobulins along with individual iso-type quantitation and analysis (IgM, IgG, IgA, and IgE).Levels of complement components , generally C3 and C4,are also part of a standard initial evaluation. Quantitation ofprotein molecules is generally performed by enzyme-linkedimmunosorbent assay (ELISA; see Unit V), which allowsdetection of low levels of specific proteins.

Analysis of Results of Quantitative Assays.Clearly, the absence of a specific cell type (such as matureB cells) or a molecule (such as an immunoglobulin isotype)will direct your further analysis. In particular, the absenceof a mature cell population will now require analysis ofthe immature cells of that lineage, cells which may only beaccessible from the primary lymphoid organs. Additionalquantitative analysis may be undertaken subsequently asindicated by results of qualitative or functional assays.This might include further characterization of activationand memory markers on lymphocytes or adhesionmolecules on granulocytes as examples. A number ofimmunodeficiency syndromes are characterized by defectsin surface molecules that impair cell function but whichdo not result in altered cell numbers.

All laboratory results are compared with those frompatients of similar age (as well as sex and sometimes eth-nicity). In fact, immunoglobulin levels will be affected byboth the patient’s age as well as his or her prior exposureto antigen (vaccination history). In a young child, levelsof some immunoglobulin isotypes are normally quite lowand, as such, an inability to produce an effective immuneresponse may not be apparent from these quantitative stud-ies but will require a specific challenge.

Functional Assays and Further QuantitativeAssays. Even the presence of normal numbers ofmature circulating cells does not guarantee their functionalcapability. Depending on which arm of the immune systemone suspects has a deficiency, several functional assaysmay be undertaken in vitro (in the test tube) and/or in vivo(in the patient).

B-Cell Functional AssaysProliferation Assays. The ability of the cell to

proliferate (divide) in vitro in response to nonspecific stim-uli (or mitogens) is a cross between a functional assayand quantitative assay and in humans is usually performedusing peripheral blood cells. This test does not evaluate thecell’s ability to perform its differentiated function; rather,it measures a preliminary step that is necessary for mostlymphocytes, T cells, and B cells to go through in thecourse of activation. B cells may be incubated with poke-weed mitogen (PWM), Staphylococcus aureus Cowen’s I,and/or stimuli directed at the B-cell receptor (BCR). In thelatter case, the assay is designed to evaluate the functional-ity of pathways normally taken to stimulate the division ofB cells during an immune response. These include use ofanti-μ antibody (the μ heavy chain is part of the BCR forantigen) and anti-CD40, a molecule crucial to the activa-tion of the B cell. Thus, the selective proliferation of the Bcell in response to some but not other signals is an impor-tant determinant of the ability of the B cell to respond tostimulation specific for a variety of pathways.

Specific Antibody Production I N VITRO

Retrospective Study (Prior Immunization). Antibodytiter that exists as the result of prior exposure, whetherto common antigens in the environment or to vaccination,is a simple way to assess B-cell function. However, theusefulness of these studies depends on the age and his-tory of the patient. For instance, positive titers would beexpected to antigens in vaccines administered during child-hood and thus antibody levels may be measured for specificantigens, such as measles, mumps, rubella, tetanus, anddiphtheria. One expects low levels of specific IgM andIgG antibodies to such antigens even in normal individualsunless they have received a recent challenge. In additionto these organism-specific antibodies, all individuals areexpected to have isohemagglutinins , “natural” IgM anti-bodies to blood group antigens not expressed in the patient.These are also believed to be stimulated by organisms andappear to be the result of environmental exposure to organ-isms expressing antigens cross-reactive to human bloodgroup antigens. Therefore, levels of isohemagglutinins areexpected to be low in children less than one year of age.

Prospective Study (Active Immunization). For a moredefinitive assessment of B-cell function, one can admin-ster a vaccine and then perform sequential measurements

of specific antibody titers. However, because patients withimmunodeficiency may develop active infections if givenlive attenuated viral vaccines, only nonliving antigens canbe used to evaluate such patients. Evaluation for titer andimmunoglobulin isotype at two and four weeks followingimmunization will provide information on the ability torespond to antigen and to switch immunoglobulin isotypes(see Case 4). Pneumococcus and Haemophilus influenzaevaccinations are commonly used as polysaccharide antigensand tetanus and diphtheria as protein antigens. The lattertype will be more dependent on T-cell help to generate aneffective response.

Specific Antibody Production IN VITRO

The B Cells on Their Own. Many of the nonspecificmitogens used to stimulate a proliferative response willresult in the production of immunoglobulin by the B cells.These products may be measured in the culture media andquantitated by ELISA according to the isotype produced:IgM, IgG, and IgA.

T–B Collaboration. B cells may also be stimulated toproduce immunoglobulin by coculture with T cells activatedwith anti-CD3/anti-CD28. Depending on the combinationof patient versus control T cells, patient versus control Bcells, or both, the function of each cell type and their abilityto cooperate with one another can be measured. The assayalso allows the evaluation of the function of T helper cellsof the TH2 subset. This parallels the natural situation ofantibody response to T-dependent antigens.

T-Cell Functional AssaysProliferation Assays. As with the B cell,

nonspecific mitogens, in this case phytohemagglutinin(PHA) and concanavalin A (Con A), are used in vitro totest the ability of T cells to respond with cell division.A more physiologically relevant stimulation test employscross-linking anti-CD3 antibody or anti-CD3 plus thecytokine interleukin-2 (IL-2), a T-cell growth factor, tostimulate the T cells.

T-Cell Functional Assays IN VITRO. T helper cellfunction for antibody production was discussed above.

Cytokine Assays. The ability of T cells to produceand secrete their cytokine products after stimulation can bemeasured by harvesting the culture media and quantitatingthe specific cytokines in ELISAs. The cytokine productsare characteristic of specific T helper cell subsets.

Mixed Lymphocyte Culture (MLC) and Cell-MediatedLysis (CML) Assays. As will be discussed in some of thecase studies, T cells recognize foreign molecules in the con-text of the individual’s major histocompatibility complex(MHC) antigens and also recognize foreign MHC itself.

The recognition of foreign MHC in vitro can be used asa means of determining the ability to generate cytotoxicT cells and the ability to provide the help necessary forthat generation, in this instance the T helper 1 subset. Inthe MLC assay, T cells are incubated with fixed cells (sothat they do not divide) from an unrelated individual andthe proliferation or cytokine production by these T cells ismeasured after several days. For the cytotoxicity assay, theresponding cells are transferred from the first culture andincubated with labeled target cells that originate from theindividual used for stimulatory cells in the first culture. Thekilling of those target cells is then a measure of the functionof the cytotoxic T cells (CD8+ T cells).

T-Cell Functional Assays I N VIVO

Delayed-Type Hypersensitivity. This assay is com-monly referred to as an assay of cell-mediated immunity.However, as will become apparent from its description, itactually measures the ability of T helper cells (and not Tcytotoxic cells) and antigen presenting cells (APCs, e.g.macrophages) to collaborate. The reaction is dependent onthe presentation of antigen in the context of MHC by theAPCs in addition to the interaction of surface moleculesand their receptors on the two cell types. An antigen towhich the individual should have been previously exposed,such as antigen from Candida , a common environmentalfungus, is injected intradermally in the forearm. Thedevelopment of redness and swelling is evaluated 48–72 hlater. The requirement for APCs to present antigen to theT cell and produce chemokines attracting additional celltypes to the region illustrates the interaction between theinnate and adaptive immune systems in both the afferentand efferent phases of this specific immune response. Infact, the results of this test are also dependent on vascularchanges at the site, demonstrating both the potentialdifficulty of interpreting in vivo testing and the complexityof responses as they occur naturally in the body.

Allogeneic Skin Graft Rejection. Although used in thepast, the ability to reject a skin graft from an unrelateddonor is no longer a test employed in humans (althoughstill acceptable in animal models). This assay would be thein vivo equivalent of the MHC–CML assay.

Phagocytic Cell Functional AssaysPhagocytosis. The ability of cells to phagocytize

(ingest or engulf particle through the formation of a vac-uole) may be measured using a variety of small, usuallylabeled particles. Their ingestion can be detected by eitherflow cytometry (see Unit V) or microscopy. Killed bacteria,yeast, or synthetic particles may be labeled and used as thetargets for phagocytosis.

Rolling, Adhesion, and Migration. These areevaluated by flow cytometric detection of the surfacemolecules required for each function.

Oxidative Burst. Two assays are employed to mea-sure the oxidative burst of phagocytic cells.

Nitroblue Tetrazolium (NBT) Assay. This assay teststhe oxidative burst capacity of neutrophils by measuringtheir ability to convert NBT, which is yellow, to formazan,which is blue/violet and detectable by microscopy.

Dihydrorhodamine Assay. Stimulated neutrophilsloaded with dihydrorhodamine will oxidize the dye withH2O2 and demonstrate increased fluorescence, which isdetectable by flow cytometry.

Evaluation of NK Cell Function. Although thefunction of NK cells can be evaluated by an in vitro testsimilar to the T-cell cytotoxicity assay, using susceptibletargets and without presensitization, in actuality the func-tioning of these cells is rarely measured in the clinicalsituation. Deficiencies in NK cells may be part of a largerdeficiency in the T-cell lineage and will be reflected in theperipheral blood NK cell numbers. The rarity of an NKcell defect as an isolated abnormality suggests that the NKcell’s individual capabilities are compensated for, that thesepatients have yet to be described, or both.

Complement Function Assay. The CH50 assay isa functional assay for complement that generates a kineticscurve for the rate of lysis of antibody-coated sheep redblood cells and is reported at the 50% point. The assaymeasures the presence and function of C1–C9.

SUMMARY

Given identification of a quantitative or functional defect,the specific disorder may need to be further defined. Forthe quantitative tests, such as finding the absence of asurface molecule on neutrophils, the results of the assaysand identification of the immunodeficiency disorder will bestraightforward; one only needs to think to look for thespecific defect. In analyzing of the results of functionalstudies, particularly those involving lymphocytes, the mul-titude of interactions among the components of the adaptiveas well as the innate immune systems must be considered.Figure UI.1 attempts to illustrate these interwoven parts byplacing arrows between the components of the branches andarms of the immune system that we described at the startof this chapter (Fig. UI.1). From this figure it is easy toappreciate how a defect in one cell may affect many func-tions or how a defect in one cell may be compensated forin the individual by the intact components of their immunesystem.

Once a quantitative or functional defect is identified,the underlying pathophysiology must be elucidated. Forsome of the immunodeficiency diseases, familiarity with thedevelopment and differentiation of the immune cells will berequired. Figure UI.2 is a flow chart of the development andinteraction of T and B lymphocytes and the positions of theblocks in differentiation and function that are seen in a vari-ety of immunodeficiency disorders; use of such a chart isa good approach to the problem if the adaptive immunesystem is deficient.
