After you Sign Up…
Four Things:
1. Pick health insurance plan through Michigan Enrolls
2. Pick a primary care provider (PCP) - Doctor
3. Schedule an appointment with your PCP (Doctor) (must be within 60 days of picking your health insurance)
4. Complete the Healthy Michigan Plan Health Risk Assessment form
