Grant A Wish, Inc, is a not for profit, tax exempt, 501 (c) (3) organization, that grants wishes to needy, disabled and abused children. Grant A Wish, Inc was founded in 1981 by Ms. Anne Blair, to help make a difference in the lives of children in and around the Chicagoland area. Ms. Blair with her legion of supporters and sponsors over the years have granted thousands of wishes to children in and around the Chicago area . Over the last 25 years. Grant A Wish had sponsored most of its wishes to

children throughout the Chicago Public School System, primarily K-8 and strived to expand its services in its general wish organization. Our programs are directed for newborns up to 16 years of age

and up to the age of 21, if you are physically or mentally challenged. As of 2019, we are providing more gifts to community based organizations through our Grant A Wish 2019 Annual Holiday Gift

Program. Most of the children ask for basic items, like a pair of new shoes, a gift card, a coat, a game or toy, musical equipment and generally advertised products for the Christmas Holiday. Grant A Wish provides with the help of our sponsors and supporters many of these children"s holiday wish

Agencies that apply for our Grant A Wish 2019 Annual Holiday Gift Program, we provide toys or gifts to the children that are connected with hospitals, elementary schools, day care centers, churches,

shelters, community based organizations and other agencies that provide for needy, disabled and abused children. Many of the organizations that specialize in helping children directly with special

needs we help to provide holiday joy. In the early days of the 1980's & 1990's, Grant A Wish would provide between 4,000 to 5,000 gifts to children throughout Chicago with the help of Ms. Blair's legion of supporters. As of 2019, times have changed and the economic reality for so many has changed. Our sponsorship and supporter base has improved and but we still face many challenges. We have instituted

some cutbacks, but we are still continuing in Ms. Blair's memory to make things better for needy, disabled and abused children.

We are working to make improvements to enhance our programs. We welcome your agency in applying for our Grant A Wish 2019 Annual Holiday Gift Program. Many of you are on the front

lines of dealing with special needs children that sometimes can be challenging. Grant A Wish applauds you for all of your hard work to help these children and we thank for applying.

HOW DOES T H E 2019 ANNUAL HOLIDAY G I F T PROGRAM WORK? Agencies applying for the 2019 Annual Holiday Gift Program must first fill out an agency application attached to this agency guideline packet. Fill out the information requested on the

application, then before the cut off deadline date submit the application to Grant A Wish, Inc by mail before or on Friday October 4, 2019 by 5:00 pm. We will make a determination whether your agency has been approved to receive Christmas gifts for 2019. I f you are approved or denied, we will inform you of our decision. I f you are approved than we will give you our finalized decision as to the number

of children we will be granting Christmas gifts. There is a $2.00 surcharge per gift to help our organization with a donation for our program. We are expecting new agencies to apply for our

Grant A Wish 2019 Annual Holiday Gift Program. Grant A Wish, Inc... Gifts That Make A Difference at CHRISTMAS...

There are several things that your agency will need to know if your agency is selected to move forward with the process to get your holiday gifts. Below is several points that must be considered.

The first thing that will happen is your agency will receive a packet of information about all of the things you will need to do to make this 2019 Annual Holiday Gift Program a success. Your agency will receive a invoice for the amount owed or a receipt for funds paid for the 2019 Annual Holiday Gift Program. You will receive a clean copy of your application that was received by us with approval information on it.

es- You will also receive a contract that your agency must sign between Grant A Wish, Inc and your agency that you will agree to host a Holiday party for your children to receive the gifts from us. With a date, time and other pertinent information. Also you must agree to take video or photographs of the events, let us come witness the distribution of the gifts and take pictures of the disbursement of the gifts. Your agency must agrees to pick up the gifts, unless delivered?.

cr You will receive a color coded Grant A Wish blank wish form with spaces for 20 children's names on each sheet. You will receive the number of sheets to cover the number of children, we have agreed to provide gifts for. On each sheet is a pre assigned wish number for each child. You must enter their name, their age, their sex and a 1st & 2nd request on each Ime. The 1st & 2nd request is design to give choice, if one item is unavailable or hard to find, then the next item can be obtained by our sponsors. In some cases the sponsors may purchase both gifts, based on cost and it is at the donors discretion. We do not fill items that are deemed inappropriate for children, cell phones, play guns, and items that seem to be unreasonable for a child to possess. We encourage agency children to ask for things like new clothing, shoes, coats, games, appropriate toys, dolls, electronic games, etc within the $60.00 limit per toy or gift per child and other items that children see advertised that doesn't encourage negative behavior.

K T You have to agree to provide thank you notes if requested from the children to our Grant A Wish sponsors for each child receiving a toy or gifts. Thank you for applying and we hope that this year's program will be exciting for the children.

APPLICATION FOR THE GRANT A WISH A v ^ ^ ^ 2019 ANNUAL HOLIDAY GIFT PROGRAM Your agency must have at least 75% needy, disabled and abused children at your facility to be considered for the Grant A Wish 2019 Annual

Holiday Gift Program you must also show profit of not for profit status. We are in our 38th year of providing holiday gifts to children. Agencies

that have received gifts for two consecutive years will be unable to apply, until two years have passed. We will make the final determination on

the number of gifts that we can provide. We are asking that the price of each gift not exceed $60.00 per child. Applications will be on line from

Sunday September I , 2019 to Sunday September 30, 2019. Your application should be submitted as soon as possible and will need to be

returned to us by Friday October 4, 2019 by 5:00 pm. I f your application has not been received by the date & time, you will be unable to apply

for this year's 2019 Annual Holiday Gift Program. Only a limited number of agencies will be accepted this holiday season. You will receive a

call or letter that you have been approved or denied by phone, mail, email, or faxed back to you on or before Friday October 11, 2019. Your agency will have to pay a $2.00 surcharge for each gift received in advance?Total amount to be paid $
