1.0 INTRODUCTION

In carrying out building external works, the hazard of excavation or trenching to construct drainage including piling work have to identified and assessed to control the risk. Normally, the soils settlement results from the deformation of soil from the imposed of load, volume changes of soil cause by seasonal conditions and mass movements of ground in unstable areas. The risk of hot work for road premix and others including oxygen cutting steels for roofing work also have to identified. Besides, the risk such as formworks fabrication and installation, concreting works, reinforcement steel works and etc. have to assessed.

Hence, it too often the workers would also encounter occupational safety and health problems during constructing the external work, such as, the workers fall into the excavated area, collapse by soils during constructing drainage, collapse by retaining wall, exposed to hot work and etc. If these workers sustain injury from accidents at workplaces, not only workers but also their families would suffer harm and pain caused by the accidents. This can also lead to delay and even suspension of works, thus incurring losses. What is more, the additional costs that arise from accidents, such as compensation, medical expenses, insurance premiums, litigation would bring losses to property owners, residents, commercial tenants, property management companies and incorporated owners. To ensure the safety, health and welfare of the workers in these external works, the health, safety, security and environmental monitoring and safety precautionary measures is priority for the project.

2.0 PURPOSE

This HIRARC details the tools for identifying occupational safety, health and environmentally hazard and its control measure by the company and all its contractors. It to analyse the companys occupational, safety, health and environment work related risk, by:

Identifying the probabilities of hazard that will occur based on work task. Assessing the work related risk by using risk matrix. The control measure on work related risk.

This HIRARC tools is being implemented in accordance with the Factories and Machinery Act (1967) and Occupational Safety and Health Act (1994) and their regulations.

2.1 SCOPE

The HIRARC tool described is for all activities on the work-site whether by company, contractor, short-term or advisory personnel and will revised once there is identified new hazards.

2.0 INTRODUCTION TO HAZARD IDENTIFICATION, RISK ASSESSMENT AND RISK CONTROL (HIRARC)

HIRARC is an integration tool to identify, assess/measure and to control hazard and risk of any workplace and its activities. By strictly implementing it, it will eliminate, reduce or control the possibility for any accidents to occur.An established Occupational Health and Safety Management System Standard (OHSAS 18001) requires organization to identifying the occupational risk and health of their process and workplace to determine those that have or can have an intolerable risk to the interested parties. Further to that, as required by the law, each organization needs to have safe system of work to ensure harm is prevented to all employees including visitor and contractors. Safe system of work includes safe work procedures and instructions. In order to know that safe system of work is adequate, hazard identification and risk assessment and risk control (HIRARC) need to be conducted.Risk is something that we as individuals live with on a day-to-day basis. People are constantly making decision based on risk. Risk is the combination of the likelihood and severity of specified hazardous event occurring. In mathematical term, risk can be calculated by the equation:-

Risk = Likelihood x Severity

Likelihood is an event likely to oocur within the specific period or in specified circumstances and, the Severity is an outcome from an event such as severity of injury or health of people, or damage to property, or insult to environment, or any combination of those caused by the event.

2.1TERM AND DEFINATION

Hazard means a source or a situation with a potential for harm in term of human injury or ill health, damage to property, damage to the environment or a combination of these.

Hazard Control means the process of implementing measure to reduce the risk associated with a hazard.

Hierarchy of control means the established priority order for the types of measures to be used to control risks.

Hazard identification means the identification of undesired events that lead to the materialisation of the hazard and the mechanism by which those undesired events could occur.

Risk means a combination of the likelihood of an occurrence of a hazardous event with specified period or in specified circumstance and the severity of injury or damage to the health of people, property, environment or any combination of these caused by the event.Risk assessment means the process of evaluating the risk to safety and health arising from hazards at work.

Risk management means the total of evaluating the risk to safety and health arising from hazards at work.

Risk management means the total procedure associated with identifying a hazard, assessing the risk, putting in place control measure, and reviewing the outcomes.

2.2 PURPOSE OF HIRARC

The purposes of HIRARC are as follows:-a) To identify all the factor that may cause harm to employees and other (the hazard);b) To consider what the chances are of the harm actually be falling anyone in the circumstances of a particular case and the possible severity that could come from it (the risks); andc) To enable employees to plan, introduce and monitor preventive measure to ensure that risks are adequately controlled at all times.

2.3 PLANNING OF HIRARC ACTIVITIES

HIRARC activities shall be plan and conducted:-a) For situation:-i. Where hazard appear to pose significant threat;ii. Uncertain whether existing control are adequate, or/ andiii. Before implementation corrective or preventive measures.b) By organization intending to continuously improve OSH Management System.

It should be the duty of the employer to assign trained personnel to lead a team of employees associated with one particular process or activity to conduct HIRARC.

2.4 PROCESS OF HIRARC

Process of HIRARC requires 4 steps:-a) Classify work activity;b) Identify hazard;c) Conduct risk assessment (analyse and estimate risk from each hazard), by calculating or estimating-i. Likelihood of occurrence, andii. Severity of hazard;d) Decide if risk is tolerable and apply control measure (if necessary)

High 15-25A HIGH risk requires Immediate action to control the hazard as detailed in the hierarchy of control. Actions taken must be documented on the risk assessment form.

Medium 5-12A MEDIUM risk requires planned approach to controlling the hazard and applies temporary measure if required. Actions taken must be documented on the risk assessment form.

Low 1-4A risk identified as LOW may be considered as acceptable and further reduction may not necessary. However, if the risk can be resolved quickly and efficiently, control measures should be implemented and recorded.

Hence, Risk can be calculated using the following formula:
