The aim of the Thank you Awards is to recognise and celebrate the dedication andachievements of individuals and teams who work at Sheffield Teaching Hospitals NHSFoundation Trust. First and foremost our nominations show where staff have workedabove and beyond the call of duty; going that extra mile to ensure that patients,colleagues or visitors receive only the very best service. Tonight’s winners, highlycommended and all nominees highlight the very best examples of team working,commitment, dedication, leadership and innovation.

Welcome to the 2016 Thank you Awards whichcelebrates the outstanding dedication andachievements of individuals and teams who work atSheffield Teaching Hospitals NHS Foundation Trust.

All of the individuals and teams nominated have worked above andbeyond the call of duty; going that extra mile to ensure that patientsreceive only the very best care. Tonight’s winners and highlycommended nominees highlight the very best examples of teamworking, commitment, dedication, leadership and innovation. Withouta doubt, the finalists are a credit to themselves and their teams.

Each nomination has been through a strict and rigorous judgingprocess by a panel which includes senior nursing, medical andmanagement representatives, voluntary groups, governors andpatient representatives. We received over 300 nominations in totaland the judges were unanimous in the view that all of the nominationscould have been finalists. All the nominees should feel rightly proudthat their teams and patients felt them worthy of a nomination.

The Thank you Awards play an important part in the Trust’s abilityto show its appreciation to the many dedicated individuals whomake up our large team and we know from experience what apositive impact the awards have on the individuals involved, bothpersonally and professionally.

May we also take this opportunity to sincerely thank our sponsors,Sheffield Hospitals Charity, The Sheffied Star and the Royal VoluntaryService (RVS), for their ongoing support. You can find out moreabout both charities on the inside back cover of this programme.

Patient SafetyAward

Patient Safety Award - presented by David Throssell, Medical Director

Q2 Multidisciplinary team

The team have been nominated forsuccessfully implementing safety huddlesto prevent pressure ulcers and patientfalls. The huddles have helped staff toachieve a run of 142 days without apressure ulcer, compared to a pre-huddlerate of one every 18 days.

The team meet daily for ten to 15 minutesto discuss each patient and their riskfactors, and which interventions need tobe implemented. All of the multi-disciplinary team, including doctors,nurses, allied health professionals,domestic staff and housekeepers, areinvolved and have ensured the huddlestake place daily from Monday to Friday.

Reducing pressure ulcers and falls canhelp to reduce length of stay, improvepatient experience and reduce costs.

Patient Safety Zone

The team have been nominated for theirwork in creating the Patient Safety Zone(PSZ), which helps to improve safety byensuring patients receive the correctinformation, medication and treatment.

The PSZ was trialled in the renal unitand the team are now looking toimplement it across the Trust.

It ensures that certain key actions arecarried out in the presence of the patient,without interruption. Staff introducethemselves by name, check the patient'sname and date of birth and ensure alltest samples are labelled andinvestigations ordered.

The PSZ has improved efficiency byensuring the right patient receives theright treatment for the right outcome.Reduced interruptions help to reducethe number of errors, including in bloodlabelling.

Clinical teams are fully engaged in theprocess, with drop-in sessions availableto discuss the PSZ and any issues theyface.

Clinical Education Team

The team have been nominated for theinvaluable support they provide to newly-qualified midwives, who describe themas “fantastic.”

Support they provide includes organising'pop-up' training sessions for juniormidwives, 'how has it been for you'listening sessions where staff can discussany issues they have, and individualisedtraining plans for returning midwives.This ensures all staff have up to dateclinical knowledge and skills, enablingthem to provide the best quality of patientcare.

They also lead on patient safety projects,such as the neonatal early warningsystem.

Consultant midwife Alison Brodrick, whonominated the team, praised them fortheir can-do attitude and commitment.

She said: “What makes this team standout is their flexibility and motivation toimprove patient care. I have never heardthem say 'we can't do that.' They worktogether to deliver solutions. They aremotivated to continually improve servicesand patient experience. They areinvaluable.”

Innovation and ServiceImprovement Award

Innovation and Service Improvement Award - presented by Neil Priestley, Director of Finance

Young People's RheumatologyTherapy Team

The Young People's RheumatologyTherapy Team comprises Zoe Cox(physiotherapist) and Dervil Dockrell(occupational therapist).

They have developed from scratch adedicated young persons' rheumatologytherapy service, providing therapytransition for young people withrheumatic disease. It covers patientstransferring from the Children's Hospitalas well as therapy treatment for 16-25year olds in the young person'srheumatology clinic.

They have established new carepathways which require only oneconsultant appointment rather than twoor three, and go out of their way toeducate teachers and employers aboutinflammatory joint disease.

They have successfully developed ayoung-person centred, holistic therapyservice which is unique outside ofLondon. Their approach is uniquelytailored to the needs of young peoplein a vulnerable and turbulent time. It isastonishing in its depth and breadth ofservice given it is run by only twomembers and developed in just threeyears.

Minor Ops Theatre Team

The Minor Ops Theatre Suite opened in2015 with a vision of offering traumapatients same day surgery, often savingthem a second or third trip to hospital.

By working together on a modestbudget, the team have succeeded inreducing waiting times for patients (oftenfrom days to hours) and saving moneyfor the Trust.

The theatre has freed up main theatresfor more complex cases, saving time andresources. In its first year the theatresaved the Trust £200,000 and is beingused as a model to improve the efficiencyof other departments.

Staff have been very supportive of theproject and are encouraged to voice idesfor improvement. They have evensourced artwork via donations to improvethe patient environment.

Patient feedback has been very positive,and the theatre is serving them well.

Cardiology investigations RHHcampus

The team have been nominated for theirwork in developing a more efficient andtimely system for tests which can helpto prevent stroke.

Stroke is a devastating consequence ofAtrial Fibrillation (AF), and it is thereforecrucial to detect AF by implementingpreventative strategies.

The cardiology investigations teamworked with the stroke team to put inplace a service improvement plan witha high priority pathway for requesting,tracking and return of results. This hasreduced length of stay for some patientsand allowed very rapid performance oftests and improved pathway of care forthe patients.

The team have been hugely supportivein trying to offer on the day investigationfor high risk patients, reducingunnecessary trips for patients who areoften elderly and speeding up the timeto diagnosis and treatments. They havefrequently arranged urgent in patienttests within 24 hours and have made ahuge effort to meet increasing servicedemands.

The improvements have been clearlyshown in the recent audit for outpatientinvestigations.

Patient Experience Award - presented by Hilary Chapman, Chief Nurseand Hilary Shenton, Deputy Chair of Sheffield Hospitals Charity

Central Park District NursingTeam

The Central Park District Nursing Teamhave been nominated for the care theyprovided for a patient throughout hispalliative diagnosis and to the end of life.

Melanie Burton, District Nurse TeamLeader said: “The Central Park districtnursing team go above and beyond forpatient care and helping to facilitate apatient's end of life wishes. This was anextremely complex and emotional timein this district nursing team and I feel theyall coped amazingly well with such adifficult situation.

During the period of care this patientwent on his honeymoon abroad and atthe time was taking complex medications.The team did a large amount of behindthe scenes work to ensure all the rightdocumentation were in place to enablethis patient to fly to a different country,including having documentation translatedinto the correct language and lookinginto airline criteria to ensure any specialmeasures were taken involving themedications or the patients care needs.”

The patient came first and was at thecentre of everything. The team completedall tasks, even those not within their roleto take ownership and ensure his end oflife wishes could be achieved. Theyremained united as a team during thedifficult and emotional aspects at thistime and delivered above and beyond.”

Kareen HumphreySupport Worker

Kareen Humphrey has been nominatedfor the huge impact she has on thepatient experience on the stroke ward.

As a support worker, Kareen alwaysgoes above and beyond the call of dutywhen caring for her patients. Forexample, when washing and dressing apatient will ask them how they wouldlike their hair styled and does this forthem.

Kareen will talk to each patientindividually to improve their experience.She will always listen to a patient's needsand then acts upon this by relaying theinformation to appropriate professionals.

Sophie Bright, Physiotherapist said:“Kareen sees the person rather than thepatient and therefore is able to addresstheir individual needs. Due to her naturemany patients know Kareen by nameand request to be seen by her as sheimproves their experience so greatly.”

In response to her positive and proactiveattitude, Kareen was chosen to be thefirst support worker to undertake atherapy training programme that aimsto promote a 24 hour therapy approachon the stroke ward.

Rachel Teasdale SmithSenior Occupational TherapistStroke Unit, RHH

Rachel Teasdale Smith is a highlydedicated Occupational Therapist basedin the Stroke Unit at the Royal HallamshireHospital. She has been nominated forfrequently going above and beyond thecall of duty in order to meet the needsof the patient.

Sophie Bright, Physiotherapist said:“Rachel always puts her patients first,she will often work around the scheduleof the patient and carers rather thanonly her own working hours. Forexample, when a patient wanted to bedischarged in time for Christmas, butthe family could only come to discussdischarge on the evening of December23rd, Rachel stayed much later than herworking hours in order to meet withthem to facilitate this.”

“As a result whenever patients come tomyself on the rehab ward after beingseen by Rachel on the Acute Stroke Wardthey always sing her praises and haveclearly been touched by her caringapproach and input.”

Rachel has also been praised for herdedication to improving the skills ofothers. She has a great relationship withthe whole MDT from support workersto Consultants. As a result of this she isable to encourage therapeutic practisein all stages of patient care.

The VolunteerAward

The Volunteer Award - presented by Annette Laban, Non-Executive Directorand Non-Executive Director Tony Buckham

S4C's Sheffield Church CouncilCommunity Care

Sheffield Churches Council forCommunity Care (SCCCC) are a hugelycommitted team of mainly volunteersthat co-ordinate a range of integratedservices that support Sheffield TeachingHospitals staff and patients.

The team have been nominated for theircommitment to providing an excellentservice which includes delivering medicineor equipment to patients, visiting patientswhich may not have family, caring forpatients pets if they have been admittedto hospital, helping patients in thecommunity with their grocery shoppingand lots more.

Suzy France, Occupational Therapyassistant said: “SCCCC are alwaysextremely polite, caring andunderstanding, always putting respectfor patients first along with patient safety.We could not function without them!”

Karen McSweeneyVolunteer, Discharge Lounge

Karen McSweeney has been nominatedfor having a huge impact on bothpatients and staff in the patient dischargelounge at NGH.

Karen has been praised for significantlyimproving patient experience, takingtime to sit with them and comfort themand preparing drinks and snacks. Shehas also developed a 'boredom box' forpatients which includes games andpuzzles to help time pass.

Karen also goes above and beyond tosupport the team, she has undertakentraining to be able to assist with patienttransfers during busy periods and oftenstays past her time.

Adam Nelson, a support worker in thedischarge lounge said: “Working withKaren is a joy, all the staff can't wait forFridays when she comes to help us, asshe is always positive and happy go luckyand is an effective and efficient memberof the team.”

Nicola HallArts activities volunteer

Nicola Hall is an arts activities volunteer,helping patients with arts and craftsactivities which can provide a diversionfrom monotony of the hospital day today experience but often also a diversionfrom pain or discomfort.

Nicola works closely with dementiapatients where she provides a 1 to 1bed-side arts or crafts activity. Inaddition, she also helps out with thearts group activity on Osborn spinalinjuries wards.

Nicola has been nominated for goingabove and beyond expectations, alwaysputting patients first and understandingthe needs and abilities of the patientsshe works with.

Mir Jansen, Arts Coordinator said: Nicolais reliable, understanding, patient,encouraging, and her communicationskills are fantastic. She enjoys what shedoes and we always receive very positivefeedback from the staff on the wardsshe works on. Absolutely fantastic!”

Behind The Scenes Award

Behind the Scenes Award - presented by Kirsten Major, Director of Strategy and Operations

Dr Gillian Hood, ConsultantAnaesthetist & Director ofMedical Education. STH

Gill has been nominated for workingincredibly hard behind the scenes toprovide extensive educational, pastoraland emotional support to anaestheticstrainees, her colleagues and trainees inother medical specialities, for over 25years.

Nominator Dr Susannah Thoms said:“What makes Gill really stand out is herunwavering passion, extensive experiencein education and profound dedicationto high quality training.

“I am not alone in saying that my careerwouldn't be where it is, and I wouldn'tbe the doctor that I am, without Gill.”

Gill has supported hundreds ofanaesthetic trainees, and trainees fromother medical specialities, includingrunning weekly exam preparationsessions in her free time.She has also made a huge contributionto the psychological wellbeing ofanaesthetic trainees and consultants,which will have directly impacted uponpatient care and outcome. She has avery good eye for spotting whensomeone is feeling overwhelmed orupset, and counsels the traineeappropriately.

Gill is an outstanding anaesthetist whois phased by nothing, is able to teachwhilst providing complex anaestheticsand has a compassionate, kind bedsidemanner.

Healthcare Governance Team

The Healthcare Governance Team hasbeen nominated for the huge amount ofwork undertaken during the Care QualityCommission (CQC) inspection process.The process lasted ten months, and theteam worked tirelessly to ensure deadlineswere met and the inspection was asuccess.

The team coordinated and submittedthousands of documents, preparedhundreds of individual areas for inspectionand responded to numerous furtherrequests for further information andsupport.

It involved significant extra hours for thewhole team, who went above and beyondin managing the work without complaint.

Nominator Sandi Carman, Head of Patientand Healthcare Governance, said: “Theircommitment has been exemplary and hascontributed to the overall positive ratingfrom CQC.

“The team were positively engagedthroughout the process and collectivelydelivered a positive result.”

Mrs Amanda BurnsHousekeeperRobert Hadfield 3 NGH

Amanda was nominated in recognitionof the positive contribution she makes tothe patient experience every day.

She goes through menus with patientsand encourages them with their eatingand drinking, is engaged with wardimprovement and the friends and familytest, helps ensure that the ward is cleanand assists other staff with errands suchas stock ordering.

She is a truly caring person who will bendover backwards to help put patients atease and make sure they get what theyneed while in hospital.She has also developed the housekeeperrole and will advise and assist otherhousekeepers.

Nominator Vanessa Marshall, Senior Sister,said: “Amanda contributes positively tothe patient experience and all patientsand visitors speak very highly of her andwill comment on her kindness, her caringattitude and her lovely personality.

“All the ward staff think highly of Amandaand feel that she plays a pivotal role inthe team. We would be lost without her.”

The Leadership AwardThe Leadership Award - presented by Martin Temple, Non-Executive Director and Jacquie Kirk, Trust Governor

Claire Powell, Skin CancerClinical Nurse Specialist

Claire is exceptional and in a uniqueposition bridging dermatology, plasticsurgery and oncology for skin cancer care.

She has engaged and motivated staff acrossthese services to come together in theImproving Flow program. She has ledcollaborative innovation and improvementto enable staff to initiate the design of asuperb one visit patient experience. Clairehas inspired and shared her enthusiasmfor improving skin cancer by bringingtogether key staff from across the hospital.

The work Claire is leading has improvingpatient experience at its foundation. Usingcoaching and improvement science skills,Claire has enabled staff to test and realisethe benefit of an integrated skin cancercare experience.

Nominator Tom Downes praised Claire forher leadership skills. He described her as“rigorous and committed in the approachto her work, frequently going above andbeyond.”

Jacky RawlinsOperations Director, SYRS

Jacky has proved to be a hardworking,inspirational and creative leader sincecoming to Sheffield 17 years ago.

She has been instrumental in ensuringpatients are treated and cared for in theright environment and on time. She ispassionate about new technology andtechniques, and has always ensuredthrough her leadership that patients canbenefit from modernisation.

In her time at Sheffield she has led onand been involved in many newdevelopments, including new theatres,critical care units and the Metavisionelectronic patient information system.

In her current role as Operations Directorfor South Yorkshire Regional Services,she has overseen improvements includinga reduction in cardiac waiting lists andthe modernisation of the way patientpathways are managed.

Jacky is an energetic, determined andprofessional person, who has neverforgotten her nursing roots and alwaysputs patient care first.

Ann MattoMatron Cardiology

Ann has been nominated for the sensitivemanagement she has shown in takingover leadership of a geographicallyseparated team.

Heart failure nursing is a clinically complexjob for nurses dealing with a largepopulation of often very sick patients.Ann has successfully combined twoteams and forged a coherent way ofworking which has improved the serviceby increasing access to this expertresource in the community, in clinic andfor in-patient and ambulatory patients.

Ann has provided strong and patientleadership to foster real team unity, andis always accessible and inclusive in herapproach. This has helped to provide animproved service for patients deliveredby a more empowered and united team.

Patient Choice AwardPatient Choice Award - presented by Chief Executive, Sir Andrew Cash

Amanda Brothwell, HouseKeeper, Firth 8 NGH

Amanda Brothwell, is a Housekeeper atthe Northern General Hospital and onthe ward where Amanda is based, manypatients have difficulties eating followingoesophageal and gastric surgery. Someare unable to eat large meals, cannoteat bread and require a special diet.Amanda encourages patients to get thenutrition they need, by helping them tochoose suitable food from the menu andalso serving their meals in a certain wayto help them to eat, for example insmaller portions or on a different sizedplate.

Maggie Coates, Matron for the wardsaid: “Amanda cannot do enough forthe patients and will always go out if herway to encourage them to eat. A recentexample of this was when a gentlemanrequested a drink of oxo and becauseAmanda couldn't source it on the ward,she went out herself to get some forhim.”

Deborah Ward Clerical OfficerRadiology

Deborah is a clerical officer in theradiology department at the NorthernGeneral Hospital. Deborah has beennominated because she always goes theextra mile to help patients and showskindness and understanding at all times.An example is when an elderly gentlemanarrived at A & E and needed an X-Ray.Deborah noticed that he was distressedand he explained that his grocery deliveryfrom the supermarket was due to bedelivered later that morning and he wasworried that as he lived on his own, hewould miss the delivery and left with nofood.

Deborah wanted to ease his mind andlet him focus on getting better, so shecontacted the supermarket and arrangedfor his delivery to be redirected to hisneighbour's house, who had agreed todeliver it to him when he arrived home.The patient was extremely grateful forDeborah's actions and this made himfeel much more at ease at a time whichcan already be very stressful. Deborahreally does go the extra mile and it showshow much she cares for patients.

Mrs Jennifer Cowlam, DentalTherapist, Paediatric DentistryDepartment, CCDH

Mrs Jennifer Cowlam has worked withinthe Paediatric Dentistry Department inthe Charles Clifford Dental Hospital for26 years. During this time Jennifer hasmade an extraordinary contribution topatient care.

The Paediatric Dentistry Departmentlooks after children needing specialistdental care from the region. This includeschildren with special needs, children fromvulnerable patient groups and childrenwho are in pain, but are too young ortoo frightened to accept dentaltreatment.

Jennifer has been nominated for herexceptional care, clinical abilities, kindnessto patients, and a bit of Yorkshire straight-talking to those that are resistant tobrushing their teeth! She has motivated,supported and won over more childrenand families than anyone could count.Over the last year, Jennifer has alsocontributed to the delivery of paediatricdentistry nationally, by being therepresentative dental therapist on theworking group to develop acommissioning guide to improve the oralhealth of children.

Eve Garrett, Secretary inBreast Surgery at the RoyalHallamshire Hospital

Eve Garrett, Secretary in Breast Surgeryat the Royal Hallamshire Hospital hasbeen nominated by Jennie Smith, whois a patient undergoing treatment forcancer.

Jennie has nominated Eve for makinga real difference to her experience andmaking her feel at ease.

Jennie said: “When you are waiting forsurgery you are very worried, Eve nevermakes you feel like you are a nuisancewhen you call her no matter how busyshe must be. She always listens toeverything you have to say and makesyou feel like she cares.

Eve is very respectful when speaking toyou about your condition and I feel Eveis remarkable at her job and deservessome recognition for everything shedoes.”

Kath Moody, CommunityMidwife Team Leader,Community Midwifery,Jessop Wing

Kath Moody has been a CommunityMidwife working in the Wybourn,Arbourthorne, Manor and Gleadlessareas of Sheffield for over 30 years. Kathis known for 'being there' for bothpatients and staff anytime day or night.Carolyn Clark, who nominated Kath said:“I can't really put into words how hardKath works, quietly and relentlessly - sheis always there for her staff, mums andbabies and is admired by her colleaguesso much. She is an unsung heroin ofMaternity Services and has been fordecades in Sheffield. Kath has given somuch, to so many mums and babiesover the years and is valued and trustedby everyone in the community she worksin. She is a complete role model and anexceptional Midwife.”

Reception team

The reception teams at Sheffield TeachingHospitals have been nominated for notonly being the friendly face which greetspatients and visitors, but for all the extrathings that they do to ensure a positiveexperience. The team are always on handto help out with any enquiries but oftengo above their role to assist. Oneexample of this is when anyone visitingthe Trust are elderly or sick and may havetrouble walking, reception often go outof their way to find them a wheelchairor arrange for them to be collected byporters to make their visit easier. Thereception team have now introduced aservice in which patients can call aheadand they will arrange to have awheelchair or porter waiting for them.

Natalie Ullyatt, a receptionist from theteam said: “Putting patients first comesnaturally to us as we just want to helpin any way possible. No matter what weare asked, we try and do whatever wecan to help or find someone who can.If patients are feeling worried or afraid,we often walk with them to the clinicor go up in the lifts with them. We alldo our utmost to ensure each personthat comes to our desk is greeted witha friendly face and any queries aresolved.”

Assisted ConceptionUnit/Jessop Fertility Team

The Assisted Conception Unit is part ofJessop Fertility in Sheffield offering adedicated IVF specialist service to allcouples who are experiencing fertilityproblems.

The team have been nominated for goingabove and beyond to help patientsthrough what can be an incrediblystressful journey. Helen Clarke, Seniorembryologist at the Assisted conceptionUnit said: “IVF is an incredibly difficultjourney for our patients, one which hasoften started many years before theycome to Jessop Fertility for their firstappointment. We're not just here toprovide their treatment, but to supportthem through the whole experience.Nurses, embryologists, administrativestaff, counselling and medical staff allwork together to provide quality care ateach stage of the process, from the firstappointment to discuss the treatmentplan, through blood tests, scans andtreatment, to the day they bring theirbaby in to say hello.”

• creating a warm, comfortable and welcoming counselling room forpatients staying in the Palliative Care Unit to talk about their treatmentand receive the emotional support they need to help them cope

• increasing the accuracy of detecting pancreatic cancer tumours by providinga Specialised Ultrasound Processor

• supporting patients in Critical Care, by funding the post of a ClinicalPsychologist to help them be less anxious and worried through theirtreatment journey

• enabling patients to avoid the complications of having major heart surgeryby giving their surgeon access to a new ground-breaking 3D camerasystem which allows minimal invasive surgery to be undertaken andremoves the risks associated with having a large operation.

• ensuring thousands of patients receive the highest standard of care bysupporting doctors and nurses to take part in training. Nearly 300members of staff have been able improve their existing knowledge.

Harry Gration - Master of ceremoniesHarry Gration presents BBC Look North forthe Yorkshire region and has been abroadcaster for three decades.

He's been a regular reporter andcommentator for the BBC's Olympic andCommonwealth Games coverage. In fact,virtually every major sport has been reportedon by Harry during his career including theFIFA World Cup, Wimbledon and the OpenGolf Championship.

Harry's awards include two Royal TelevisionSociety Awards for his sports documentaries,and in 2013 he was honoured with an MBEfor services to broadcasting in the 2013Queen's Birthday Honours list.

Away from broadcasting, Harry has beenPresident of the Scarborough Cricket Festivaland is currently involved with numerouscharities all over Yorkshire.

RVS is an age positive national charity foundedover 70 years ago. We believe that every olderperson should have the opportunity and choiceto get more out of life. At a local communitylevel we deliver personal and practical supportthrough the power of local volunteers tosupport older people.

At a national level we raise awareness of theissues older people face through our mediacampaigns and research and lobby thegovernment for age related policyimprovements. We have more than 40,000volunteer and 2,000 staff and help over100,000 older people every month across thecountry.

Dr John HammondDr John Hammond, aged 89, died earlier this year. Renowned as a Sheffield GP,he made a huge contribution at Westfield Health as Medical Officer, Directorand finally as Senior Charitable Trustee. Dr Hammond was also a research fellowin Medicine and Cardiology at the former Sheffield Royal Hospital.

Graham Moore, Chairman of Sheffield-based Westfield Health, said: “John wasa true gentleman, meticulous in his approach who acted with great integrityat all times, putting principle before popularity. The people of South Yorkshireand beyond owe him greatly for his dedicated work for Westfield Health'sCharitable Trust. Under his leadership, the Trust donated millions of pounds tothe NHS and Medically Related Charities which made a real difference byalleviating suffering as well as saving and prolonging lives.”
