They are an effective way to:  Foster and encourage innovation in the delivery of health promotion and prevention at a community level.  Use and develop the capacity, knowledge, networks, and skills in the community to deliver local health promotion and disease prevention initiatives .  Support and build on the work already done by Medicare Local members, community groups, volunteers and organisations on the Central Coast in the area of health promotion and prevention.  Raise awareness of local health promotion and prevention needs and issues at a community level.  Raise the profile of the Medicare Local. Healthy Community and Innovation Grants

Application Guidelines

Background:

The Central Coast NSW Medicare Local (“Medicare Local”) has established a HealthyCommunity and Innovation Grants program to support multiple health promotion and diseaseprevention initiatives, programs and campaigns across the Central Coast Region and isseeking applications from our membership and the not-for-profit sector.

The Medicare Local considers the Healthy Community and Innovation Grants program aninnovative approach to supporting the significant contribution made by local community groups

Health promotion priorities and health need on Central Coast:

The priorities for health promotion in Australia have been established by the Australian

National Preventative Health Agency (ANPHA) for conducting educational, promotional and

community awareness programs relating to preventive health. These are:

  promoting a healthy lifestyle and good nutrition;

  reducing tobacco use;

  minimising the harmful drinking of alcohol;

  discouraging substance abuse; and

  reducing the incidence of obesity amongst Australians.

The Medicare Local, as a result of their local health needs assessment have also identified thefollowing health needs on the Central Coast:

Early intervention Dementia Mental Health and Mental HealthPromotion

 Access to GPs GP Workforce Aboriginal health

Nutrition Carers health Transport to health services

Child/adolescent health Case management Smoking

Suicide Palliative care Chronic disease

Oral health Domestic Violence Men's health

Child behavioural issues Cancer Seasonal fluctuations

Health Literacy Drug and alcohol Health Advocacy

Grants available:

The Medicare Local is seeking applications from eligible organisations or individuals to providehealth promotion or disease prevention programs, initiatives or campaigns (programs) whichaddress at least one (1) of the national health promotion priorities and / or at least one of theCentral Coast identified health needs, as listed above.

The following grants are available (subject to a funding cap):  15 to 20 small grants of up to $5,500 (inclusive of GST)

  2 to 3 medium grants of up to $22,000 (inclusive of GST)

  1 large grant of up to $55,000 (inclusive of GST)

Successful applicants for the small grants will receive the full value of the grant on executionof a funding agreement and prior to the commencement of the proposed activity or programand no later than June 2014.

Successful applicants for the medium and large grants will receive 80% of the grant value onexecution of a funding agreement and prior to the commencement of the proposed activity orprogram and no later than 31 March 2014. The remaining 20% will be paid prior to 30 June2014 following approval of a progress report and interim financial acquittal.

Special Conditions for the Medium and Large Grant applicants:

 Applicants for the medium and large grants are required to provide a copy of theorganisation’s audited financial statements, preferably for the 2012/13 financial year. They arealso required to provide a detailed budget as outlined in the funding agreement.

Note: Applicants for the large grant must develop and provide an innovative model of care which

improves Access to and/or Integration of health services on the Central Coast.

EligibilityTo be eligible to apply, organisations or individuals must be either a member of the CentralCoast NSW Medicare Local or a not-for-profit organisation or community group. Government

organisations are only eligible to apply for the medium and large grants.

  For those applying as a Medicare Local member, membership must be approved/confirmed before applications close (see the Medicare Local website for more detailabout becoming a member  www.cccnswml.com.au).

  Not-for-profit organisations and community groups need to be incorporated and basedin or provide services within the Medicare Local’s boundaries (Gosford and WyongLGAs).

  Programs must aim to address at least one of the ANPHA priorities or identified healthneeds as listed above.

  For small grant recipients, programs must commence before 30 June 2014.  For medium and large grant recipients, programs must commence before 31 March

2014.  All funded programs must take place only within the Medicare Local boundaries.  Any printed or advertising material must include the Medicare Local’s logo and

successful applicants give the Medicare Local the rights to publish and promote theactivity.

  Applicants must have suitable insurance to cover the proposed program (evidence isrequired as part of application).

  Successful and unsuccessful applicants from previous Medical Local grants arewelcome to apply.

Ineligible applicants include:

  Commercial for-profit entities (other than Medicare Local members)  Political organisations

Assessment

The Medicare Local will establish a panel to review applications. The programs and initiativesoutlined in the applications will be assessed and ranked based on the following:

  Meeting of eligibility criteria.  Degree of innovation (for the large $55,000 grant [inclusive of GST]).  The reach and impact of the program or initiative with regards to the number of people

the activity will reach and the potential impact on their health.  Level of evidence that the program is based on.  Sustainability past the funding period or will have an ongoing benefit for the community  Target disadvantaged members of the community such as Aboriginal and Torres Strait

Islander people or those from low social economic backgrounds;  Visibility and accessibility to the general public,  The degree of co-contribution or in-kind contribution to the program by the applicant.  How the program or initiative will be evaluated in terms of reach and impact.  Appropriateness of proposed budget expenditure.

Restrictions

The Healthy Community and Innovation Grants Program will not fund:

  Any project which does not meet the eligibility criteria.  More than one project of any value for the same organisation or the same project.  Projects which are core activities or services of the applicant organisation or Medicare

Local member.  Projects or initiatives which duplicate those that are already being offered within the

same community.  Projects or activities which are the primary responsibility of other funding agencies or

government departments.  Projects funded retrospectively.  Projects which are co-funded by other organisations which do not promote the values

or vision of the Medicare Local (eg: Tobacco or Alcohol companies etc)  Projects of a commercial nature that are for-profit or that raise funds for philanthropic

purposes for other organisations.  Projects where the applicants are in position to self-fund or where a more appropriate

funding source is available.  Capital resources such as building works, maintenance costs, vehicles and/ or

equipment requiring annual registration and office equipment (eg: computers,photocopiers etc).

  Equipment unless demonstrated that it is a vital part of the project and benefits thewider community.

  Administration costs, including ongoing operating costs of the organisation, in excessof 10% of the small grant funds and in excess of 15% for the medium and large grant

funds.  Costs to attend or organise registered sporting competitions or sporting trips.  Events or conference sponsorship, prizes or award ceremonies.  Debt repayments for an organisation.

How to apply

1. Consider the eligibility criteria, assessment and restrictions and how they apply to yourproposal.

Successful Applicants:

Successful applicants will be advised in w riting by Monday 23 December 2013 and willreceive a funding agreement to sign. Grant recipients are required to provide a tax invoice inaccordance with the funding agreement to the Medicare Local for the approved grant amount.On approval of the invoice and execution of the funding agreement, the grant funds will bepaid by direct deposit to your nominated bank account stated.

Note: The Medicare Local may request further clarification from the applicant with regards tothe project, budget or the applicant’s financial credentials prior to approving any funds. ,

Successful applicants are also required to attend a media launch early in 2014, at a date yetto be determined.

Unsuccessful Applicants

If your application is unsuccessful, it is not necessarily due to the quality of the proposed

project but may be due to the demand for the funding. A request for feedback on anunsuccessful application can be addressed to Andrew Davison (Health Care Integration andImprovement Manager): [email protected], 43652294.

Unsuccessful applicants will also be advised by Monday 23 December 2013.

Protecting Your Privacy

The Medicare Local is committed to protecting privacy and we take reasonable steps tocomply with the relevant legislation and Medicare Local policy. Information provided will onlybe utilised for assessment and contact purposes. Please note that successful applicants will

be listed on Medicare Local’s website and include the organisations name, a brief projectdescription and the grant amount received. A media release will also be distributed with thisinformation.

Funding Agreement

Successful applicants are required to comply with the following conditions:

  Funds must be used for the purpose for which they were granted. Any variations in theuse of funds must be approved in advance by the Medicare Local and confirmed inwriting.

  Funds must be expended within the timeframe specified by the applicant in the

submitted application unless an extension is requested in advance approved by theMedicare Local and confirmed in writing.

  Funded programs, activities, or initiatives must commence before 30 June 2014 for thesmall grants and before 31 March 2014 for medium and large grants.

  A Final Project Report Form must be completed and submitted to the Medicare Localonce the project is completed. The report form will be provided by the Medicare Localin early 2014.

  Medium and large grant recipients must submit a progress report and an interimfinancial acquittal by the 15 June 2014 in order to receive the final funds payment.

  The Medicare Local logo shall be used on all printed material and advertising related tothe funded project and an acknowledgement of Medicare Local’s funding must be

given by the organisation in all promotional material and reports of the project to thepublic. This logo will be provided as part of the Marketing Guide provided to successfulapplicants.

  Specific conditions may apply to some funded projects and these will be discussedwith the applicant.
