PUBLIC HEALTH INFORMATION SYSTEM (PHIS) PUBLIC HEALTH INFORMATION SYSTEM (PHIS) Outpatient Services A total of 141,131 patients were seen through GOPD compared to 170,885 in the 1st Quarter of this year. Similar numbers were seen for the same period last year. About 13,000 patients were seen at SOPD, 14,000 through outreach, 10,000 through home visits and 12,000 by village health workers. More patients were seen through outreach and home visits in the 2nd quarter compared to the first quarter.

Minor procedures and dressings were carried out for majority of the patients visiting the GOPD particularly in the Central Division. Similar to the trends in the first quarter of 2012 respiratory conditions, followed by musculoskeletal and skin conditions were the leading cause of morbidity.

Nurses attended to about 140,000 patients followed by Medical officers (63,000) and 45,000 by Nurse practitioners Maternal Health Services At Antenatal Clinics there were a total of 1887 normal pregnancies seen compared to 2487 in the first quarter of 2012. More at risk pregnancies were seen at ANC in the second quarter (1647) compared to the first quarter of 2012 (1355). Among the complications/risk factors were those outlined in the table as follows:

Oral pills and Provera Injections were the commonest contraceptive methods followed by condoms reported by all divisions. A total of 2650 papsmears were recorded through family planning clinics and 110 through postnatal clinics. This is slightly more than the numbers reported for the first quarter of this year.

Child Health Services Immunisation

Based on the above figures, estimated coverage of MR1 was about 84.8%. This is an improvement from the quarter 1estimated coverage of about 74%. This has been estimated using ¼ of 2011 livebirths(20425) as denominator.

Among the 1 week to 2 month old children attending IMCI clinic, feeding problems such as not being able to feed particularly from Northern Division was most commonly reported. Likewise, local bacterial infection (16%) followed by diarrhoea without dehydration were also very common (15%).

Among the 2 months to 5yr old children attending IMCI clinic, respiratory conditions (particularly those classified as no pneumonia – cough and cold) was most common – prevalence of 23%; Skin conditions such as abscesses or cellulitis were also very common. Low weight was also recorded among 7% of those attending IMCI.

Compliance to Reporting and Challenges faced

The preceding analysis is based on 91% of reports from the Divisions - 89% of Central, 94% of Western, 95% of Northern and 84% of Eastern Divisions, which is an improvement compared to 81% received overall in the 1st quarter.

The major limitations faced in compiling this report include: � Unavailability of population statistics and appropriate denominators to calculate meaningful rates for

example contraceptive prevalence rate. Hence absolute numbers have been used for this which can limit comparisons

� At present all reports (even zone reports are being received directly at National Level) making it difficult for entry and compilation whilst ensuring quality of output. Hence it would be appreciated if the Divisions could compile the PHIS reports to ensure submission of only 1 consolidated report to the National level per division each month. This would foster accuracy and timeliness in providing feedback to you.

There were a total of3103admissions in the second quarter compared to 3661 in the first quarter of 2012, comprising of 52% females and 48% males. Injuries, Poisoning and external causes accounted for about 23% of the total admissions, followed by infectious and parasitic diseases (18%), diseases of the circulatory system (17%) and diseases of the respiratory system (13%). Other causes for admission comprised less than 10% of total admissions.

Most admissions were reported in the Central Division followed by the Western and Northern Divisions in the second Quarter. Whilst admissions due to injury, poisoning and external causes were most common in the Central Division, certain infectious and parasitic diseases were the leading cause for admissions in the other 3 divisions.

MORTALITY There were a total of 1342 deaths reported in the second quarter of 2012. Similar to the results of the first quarter, these constituted of 55% males and 45% females. The major causes of death included:Diseases of circulatory system (31%), Endocrine, Nutritional and Metabolic disease (25%) and Neoplasms (12%).

Diseases of circulatory system was the leading cause of death followed by endocrine, nutritional and metabolic disease. Majority of deaths reported from Central and Western Divisions. Similar trends were noted in the 1st quarter of 2012.

Most deaths reported among the 70+ age groups. A total of 77 deaths in the < 5 age group gives an estimated Under 5 mortality rate of about 15.1 per 1000 livebirths.

Similar to 1st Quarter report, there were a total of 59 infant deaths giving an estimated infant mortality rate of 11.7 per 1000 livebirths (using ¼ of 2011 livebirths as denominator). Majority of the deaths were reported in the postneonatal period followed by late neonatal period. Apart from the 3 divisional hospitals, infant deaths were also reported from Raiwaqa, Wainibokasi, Sigatoka, Nadi, Seaqaqa and Levuka.

A total of 41 fetal deaths were reported in the second quarter. Majority of deaths were from Lautoka Hospital and these were due to unspecified causes. There is a need to improve cause of death reporting through death certificates for more targeted interventions. The commonest cause of deaths among both males and females were diseases of the circulatory system followed by endocrine, nutritional and metabolic disease (which includes Diabetes) and neoplasms. There were twice the number of deaths due to diseases of the circulatory system among males compared to females. Likewise, deaths due to neoplasms accounted for twice the number of deaths among females compared to males.

Similar to national level, in Central Division, majority of deaths were due to diseases of circulatory system followed by endocrine, nutritional and metabolic disease and neoplasms. Majority of deaths reported from CWMH followed by Nausori, Korovou, Wainibokasi and Vunidawa Hospitals.The death rate is about 6.6 per 1000 population (using ¼ of 2011 Central Population), which is similar to the first quarter report (6.1).

In Western Division, diseases of circulatory system followed by endocrine, nutritional and metabolic disease and neoplasms also were the major causes of death. Majority of deaths reported from LautokaHospitalThe death rate is about 5.8 per 1000 population (using ¼ of 2011 Western population). This is slightly less than the first quarter death rate of 6.23.

Diseases of circulatory system, endocrine, nutritional and metabolic diseases were the leading causes of death, similar to national level trends. Most deaths were from Levuka, followed by Vunisea and Lakeba. The death rate in Eastern Division is about 4.7 per 1000 population (using ¼ of 2011 Eastern Population), which is slightly higher than the rate estimated in the first quarter report (3.33).

In Northern Division, the major causes of death are similar to the national level trends. Most deaths reported from Labasa Hospital, Taveuni and Nabouwalu, similar to the trends in the 1st quarter. The death rate in Northern division is about 5.9 per 1000 population (using ¼ of 2011 Northern Population), which is also similar to first quarter result. NON-COMMUNICABLE DISEASES Diabetes

A total of 61 new cases of Diabetes were reported in 2012 compared to 121 for the same period last year. 15 facilities had reported in the second quarter, which is an improvement from the first quarter reporting during which only 6 facilities had reported. Among those facilities that reported, Wainibokasi had the highest number of new cases followed by Valelevu and Lami.

The age distribution of new cases has been similar to the pattern last year as well as for the same period last year, with most number of cases in the middle age population particularly 55-59 age group. In 2012 most cases were reported among the Female I-Taukei population compared to Female Indo-Fijian population last year. Cancer The statistics on cancer are presented using the ICD-O codes, compiled from histopathology, cytology reports, PATISPLUS reports and Death Certificates.A total of 126 cases of cancer were reported in 2nd quarter 2012 compared to 263 for the same period last year. Of these 67% were females and 33% were males. Majority of the cases were reported in the 55-59 age group

Overall less number of cancer cases were reported in 2012 compared to the same period last year. Apart from unknown primary sites, Prostate, colorectal and Esophageal cancers were the main cancer sites reported among males in the 2nd quarter for 2012.

Similar to trends among males, there were in total less number of cancer cases in quarter 2 of 2012 compared to the same period last year. The main cancer sites reported among females in the 2nd quarter for 2012 include cervix, breast, ovary and endometrial cancers.

There were 3 cases in the paediatric population (0-14 years) in the second quarter of 2012 compared to 5for the same period last year. 2 cases were among unknown primary sites and 1 case involving the nervous system. MENTAL HEALTH The source of information on mental health was through the St. Giles Admissions Register. There were a total of 107 admissions, comprising of 52% males and 48% females in the second quarter of 2012.

Majority of cases were in the 30-34 age group compared to 45-49 age group in the 1st quarter of 2012. Similar to 1st Quarter results, most admissions were among single individuals.

Apart from other causes for admission not specified, majority of admissions were due to Schizophrenia and other Psychotic Disorders. This was followed by Bipolar Affective Disorders.

TUBERCULOSIS (TB) Data on TB was obtained from the National TB Programme. A total of 52 new cases of TB reported in the second quarter compared to 50 in the 1st quarter of 2012. There were 4 relapse cases – 2 from Central and 2 from Western and 1 default case.

Overall the number of new cases amongst males and females was evenly distributed. However in the Northern Division 3 times the number of females were diagnosed compared to males. 66% of total TB cases were smear positive in the 2nd quarter, 2012. About 53% of the cases were tested for HIV at national level

Of the 22 patients who were evaluated for outcomes in the second quarter, the pie chart indicates that majority were cured (68%).

Notifiable Diseases Data for Notifiable Diseases are obtained from the Notifiable Diseases Certificates, every week-ending. April recorded the highest number of cases, followed by May and June. This can be attributed to the timeliness of reports being sent by the facilities to HIU. The leading diseases are Viral Illness, ARI, Diarrhea, Influenza and Conjunctivitis. The number of cases reported in June is quite low and can be attributed to the timeliness and under reporting from the facilities in June.

The most common diseases for Suva Rural Areas are ARI, Viral Illness and Diarrhoea. Valelevu has the highest number of diseases being reported, and can be attributed to the larger population catchment.

Among the facilities in the Serua/Namosi subdivision, Navua has the highest number of cases. The most common diseases are ARI, Viral Illness, Influenza and Diarrhoea.

The highest number of cases recorded for Rewa Sub division was reported from Nausori and included ARI and Viral Illness.

As seen in the 1st Quarter report, there was only 1 facility which was reporting under the Tailevu Sub division. The above graph shows the reports from all the 3 facilities under this sub division. According to the Received Certificate Register for 2012 Korovou has submitted all their reports. Lodoni Health Centre has yet to submit the reports for January and May, whilst Nayavu still has to submit the 1st quarter reports to HIU.

The graph shows that the leading disease is Viral Illness for Naitasiri sub division. The above graphs for the Central Division illustrate trends for the different top ten diseases by sub division. The most common diseases are ARI, Diarrhoea, Viral Illness, Influenza and Conjunctivitis. Many facilities have yet to submit their reports on time to HIU as illustrated by the graph below:

These reports are compiled in 1st week of July, still there are nearly 50% reports pending. Western Division

The graph shows the trend for Lautoka subdivision for top ten diseases. Natabua and Viseisei health centres had the most of the Notifiable diseases. This may be due to under reporting of diseases from the other facilities.

In the 2nd quarter, all 3 facilities in the Western Division are reporting, unlike in the 1st quarter when reports from Namaka Health Centre were not received. Viral Illness is the leading disease reported from Namaka Health Centre.

Viral Illness is the leading disease reported from Nailaga Health Centre in Ba Subdivision. Other common Notifiable Diseases from Ba Sub division included Diarrhoea, Influenza and ARI and these were consistently reported from all facilities.

The graph for 2nd quarter shows that reports are being received from all the facilities underTavua subdivisions, unlike in 1st quarter report. Viral Illness/infection, conjunctivitis and diarrhea were the commonest diseases reported from these facilities.

Of the top ten diseases reported from Nadroga Sub division the leading diseases include Viral Illness and ARI, and are mostly reported from Lomawai and Sigatoka.

Viral Illness is the most common disease in Ra subdivisions. The highest number of cases are reported from Nanukuloa and Rakiraki. For Western Division there were variations in the top ten diseases. However, in overall the most common Notifiables in Western division are Viral Illness followed by ARI, Influenza, and Diarrhoea. Also noted is the poor reporting from the facilities which can influence accuracy of results and interpretation as illustrated below by the graph on reports received from the Western Division:

The graph shows that there are some facilities which did not submit reports on time and some failed to submit any reports at all.

Northern Division

The leading disease for Macuata subdivision is Influenza and most cases are reported from Seaqaqa health centre. Other leading diseases are Viral Illness and ARI and are mostly being reported from Wainikoro Health Centre.

ARI was the most frequent disease reported from Cakaudrove. There were roughly equal distribution of cases among the various facilities in this subdivision.

The graph shows that the most leading disease for Rabi is Influenza.

The leading diseases reported from Bua subdivision are ARI and Viral Illness. Most diseases are consistently reported from Wainunu health centre.

Most of the cases are being reported from the Waiyevo Health Centre. The leading diseases are Viral Illness, ARI and Diarrhoea. The Graphs for Northern division shows that the most common notifiable diseases are ARI, Viral Illness and Influenza. Some facilities did not report any diseases, and it is unclear if any cases were reported or this was due to failure to submit reports to HIU as shown by the graph below:

Viral Illness and ARI are the most common disease in the Kadavu sub division. In the 2nd Quarter, Rotuma only reported 2 cases of Fish Poisoning. Most of the reports are pending from this sub division. Most of the cases are reported from Lomaloma and the leading diseases are Viral Illness and ARI. However some facilities are not submitting reports as shown below:

Nevertheless, the reporting system for Eastern division has improved as compared to 1st quarter.

It is apparent the submission of reports from health facilities throughout Fiji still has scope for improvement. In terms of reports received for 2nd Quarter 58% was received from central, 56% from Western, 63% from Northern and 76% from Eastern. It is encouraging to note that the compliance to reporting has improved compared to the 1st quarter report from all divisions, particularly in the Eastern. Similar to the 1st quarter results, the leading Notifiable diseases for 2nd Quarter 2012 were Viral Illness, ARI, Influenza and Diarrhoea. Hence, appropriate strategies should be initiated to reduce the incidence of these diseases. HIV We thank the Family Health Programme for the following section on HIV. The graphs are based on the cumulative number of HIV cases.

There were 56% females and 43% males in the first half of this year, compared to overall 53% males and 47% for all cumulative cases since 1989. Overall majority of cases were reported in the I-Taukei population.

Heterosexual was the most common mode of transmission, followed by perinatal transmission. The 20-29 age group was the commonest age group affected.

The cumulative number of HIV cases, since the first case in 1989 is 449. There were 30 new cases reported in 2012 so far. There have been extra tests carried out in quarter 2 e.g. 300 extra tests among sex workers. Locum clinic pilot project was commenced in quarter 2 (afterhours) for vulnerable groups and there has been increased HIV awareness by President of Fiji in secondary schools followed by good media coverage. These events may have contributed to increase in testing and slight increase in HIV confirmed cases.
