5.Provideclarificationintherelevantspacetosupportapplication(ifany):

i)Clarifythemeaningofanyletterofthealphabetstatedinthename.ii)Give explanation if thename is not in Bahasa Malaysia orEnglish.iii)Ifthenamecontainsanindividualsname,statewhetheritisthenameofadirector,andtoattachacopyofhisidentitycard.iv)Ifthenameappliedforhasaconnectionwithacompanythathasalreadybeenregistered,pleaseobtainaresolutionfromtherelevantcompany.The resolutionhas tobe signed by all thedirectorsofthecompanyand,ifitisanextractoftheresolution,

it has to be signed by one director and secretary. A consentletterhastobesignedbyadirectorandsecretaryorbyatleasttwodirectors.v) If the name contains a Trademark, the applicant needs toobtainanapprovaland,ifnot,tonoteNotApplicable.vi) If the nameapplied for is for achangeof name,applicantneedstostatethecompanysnameandcompanysnumberfortheexistingcompanythatisseekingtochangename.vii)Statethenatureofbusinesstobecarriedout.viii)Othermatters.(Forincorporationofanewcompany,please

6.Theresultoftheapplication for companynamewillbeapprovedwithin 1 working day.
