(2) Design a plan for how school districts and vocational rehabilitation will work together to improve the post-school outcomes for persons with disabilities.

Start with the End in Mind
Expected Outcome
Expected Outcomes: List what you anticipate will be the result
or changes that will occur if your plan has been successful.
These should be specific, measureable things that happen as a result of completing the activities.

2. Participants will be able to write a plan that assures the expected outcomes are met.

who will be completing the task.

Goal After you have identified
your expected outcomes, activities, person responsible, timeframe, and outputs/products, it is time to write your goal.
The goal should be a SMART (Specific, Measurable, Attainable, Realistic, and Time Bound) goal and aligns with the expected outcomes.
