Highlights of HSE Activities for the Month - July, 2016
Highlights of HSE Activities at Work Centres are summarized below:
1. Behavior Based Safety (BBS):
BBS schedule meetings of functional committee and Steering Committee organised at various sites.
1. BBS follow up visit by representatives from M/S Obul Consultancy has taken place all along JLPL, Ahmedabad, Chhainsa, Vadodara, Mumbai, Hazira & Jaipur.
2. Emergency Response and Disaster Management Plan (ERDMP) / Mock Drill:
ERDMP of Gandhar has been reviewed & verified by the accrediting agency M/s. CEIL, Mumbai representatives, and found to comply with requirements of Petroleum & Natural Gas Regulatory Board.
ERDMP of Nasirabad has been reviewed & verified by the accrediting agency M/s. CEIL, Mumbai representatives, and found to comply with requirements of Petroleum & Natural Gas Regulatory Board.
Level II on site Mock Drill conducted in presence of PNGRB ERDMP accrediting agency M/s. CEIL, Mumbai representatives.
GAIL Usar & Vizag participated in the emergency response drill conducted by their Mutual Aid partners HPCL & BPCL respectively.
Quarterly On-Site Mock Drill organised at Gandhar, NCR, Pata & Samakhiali.
3. OISD / T4S Audit
OISD audit has been conducted at GPU, Gandhar..
OISD audit has been conducted for the Desu- Maruti pipe line & Desu Sonipat Bahadurgarh pipelines under NCR region.
T4S audit as per PNGRB Regulation of Agartala & Puducherry region pipeline system has been conducted by M/s Intertek Moody International Private Limited.

4. External Safety Audit/QRA
QRA of Vijaipur Complex has been conducted by M/s DNV-GL.
5. Other Highlights
Public Awareness meet on “Pipeline Safety” conducted at Village Vankamamidi near SV 35 in the Pochampalli Taluka of Nalgonda District under Cherlapalli region.
Pipeline Safety Awareness program conducted for school children at Village Debar Bari of Jhabua district.
Pipeline safety committee meeting conducted for villagers at Panchayat Bhawan Village Debar Bari under Jhabua region.
Safety awareness and emergency handling program conducted for Kerala State Fire & Rescue department and for customers under Kochi region.
A mass Public Awareness programme on “Ensuring Safety in urban area in case of Emergency” was organized for all the residents of Sattivani Pallem and nearby residents along the RoU in the Urban Area of Vizag City at Chainage 3.5 Km of Vizag- Secunderabad LPG pipeline.
Community Awareness program on Natural Gas Pipelines safety and Dos & Don’ts in case of leakage are being conducting by hiring local focus group and arranging in PL ROU Villages.

Near-miss Incidents Reported by Work Centres:
Near miss incidents have been reported from various sites in the month of July 2016. Notable near-miss incidents are summarized as below:
Agartala: On 27.07.2016, construction of Stores Building is under progress at Anand Nagar Terminal and due to this construction, various civil related activities are going on in front of the Terminal. During cleaning pigging activity for the section from Konaban Tap-off Point to Anand Nagar Section Terminal of the pipeline (10” dia. X 20 Kms.). GAIL Officer, Sh. S C Reang, SM (O&M) is about to hit by the extended bamboo piece of scaffolding of Stores Building construction, just outside the gate while coming out from the Terminal after completion of the pigging activity. He could manage to avoid and escape any bodily/ head injury because of alertness and moreover, he was wearing the Safety Helmet. Otherwise, he could have been injured. Immediately, the supervisor of the contractor was instructed to cut the extended piece of the bamboo scaffolding and the same was cut and removed to make the space clear and for free movement. Chhainsa: On 08.07.2016 at 1655 hrs, at the rear entrance of administrative building a metal framework structure is provided to enhance the aesthetic look of the building. On 08.07.2016 at around 5 PM due to sudden change in weather condition, strong winds started blowing which forced a large piece of metal sheet installed at the top of the framework to come out from its fitting, resulting into falling of the sheet on the ground. Sh. Yogesh who was going to take refuge in the building due to the rain narrowly escaped an injury from the falling sheet. Jhabua: On 08.07.2016, security guard Sh. Jagdish Prajapati, when approaching to watch tower no 2, got minor first aid injury due to tripping by an earth cable which was left loose near the walkway. He could not see the earth cable. Though a serious did not happened but there was potential of sever injury had he fallen on ground. Kailaras: On 02.07.2015 at 15:35 hrs, at RR Station Sone Ka Gurja- During Pigging activity, contract labor was engaged to dispose the debris and the black mug received during the pigging. After disposing the debris, It was observed that one of the contract employee was going to light the bidi, immediately he was stopped as his clothes were dipped with the black mug and training was imparted to all the contract workers and supervisors for the hazards and the fire risk associated with the pigging debris. Security guards were instructed not to allow any persons with flammable items at the work site. Nasirabad: On 26.07.2016 at 15:45 hrs, Fire water line modification job was done in LPG process area. During plant round it was observed that support frame were removed but the grouting bolts were left on ground as it is. The Unsafe condition was having potential to be converted into any accident. Samakhiali: On 27.07.2016 at 16:30 hrs, Main bank of CO2 cylinders of SV-18 operated by push button in the telecom room, SOV of master cylinder operated and when CO2 gas started

coming into the common manifold, gasket of common service line failed and gas started leaking into battery room also, However since it was operated by push button and no one was present in the battery room, no injury was reported. Vadodara: On 22.07.2016 at 09:02 hrs, Foot Patroller from ONGC has reported leakage in 8" GNAQ- Magnad Pipeline near Khanpur village (at 6-6.5 Km chainage) at 9.02 Hrs. on 22.07.2016 in common RoU of ONGC and GAIL- O&M Undera has confirmed that the minor leakage is on GAIL Pipeline at 11:49 Hrs. of 22.07.2016 to RGMC-Vadodara. Vijaipur GPU: On 26.07.2016, during Heat exchanger shifting job in Phase 2 LPG unit area, after successful bolting and installation of new heat exchanger, the sling of crane got stuck in the name plate of the heat exchanger. This was not observed carefully by neither crane operator nor supervisor. As a result, when the boom lifted, a huge shock has come in the structure which could have had a serious effect. Luckily nobody was affected.
