The 14th International Symposium on Functional Medicine, 21st Century Endocrinology: Thyroid and Adrenal as Sentinel Organs, held this past May in Tucson, Arizona, was a fantastic success with our largest number of participants to date. For the first time, we asked faculty, clinicians, and medical writers to compile and review clinical pearls from each plenary presentation and concurrent session. As an added benefit for IFM members, Symposium attendees, and purchasers of Symposium program materials, we are sending out these 14 th Symposium Clinical Pearls for use in your practices. We’ve included a few snippets from our comprehensive list in the Integration section of this newsletter (see page 3). We’re also thrilled to announce another first-time accomplishment: Five of the Symposium talks have been synched together—the PowerPoint slides and the audio are integrated in one multimedia presentation. Check out page 2 for descriptions of these talks and information on ordering them. Another milestone ... Jane Murray, MD, and I will be presenting three workshops at the upcoming American Academy of Family Practice Scientific Assembly in Chicago on October 3–4. Our workshops will present functional medicine as a new model for chronic disease. We’re looking forward to this opportunity to expand the reach of functional medicine. We know that many of you are very active in your communities and professional networks, helping to spread the word about functional medicine. Please let us hear about your activities. We now have a variety of materials and presentations to help support you with your advocacy work, and we’d like to keep abreast of the efforts being made to introduce functional medicine to clinics, hospitals, governmental agencies, insurance companies and HMOs, medical and other health professions schools, and patients/consumers. 21st Century Endocrinology Thyroid and Adrenal as Sentinel Organs

The Sentinel Role of Adrenal and Thyroid in Complex Chronic Illness – Jeffrey Bland, PhD, CNS

Alterations in hypothalamic-pituitary-adrenal (HPA) function do not define a disease, but rather an alteration in a systems biology outcome. The use of singular molecular interventions to manage individual symptoms is not the optimal long-term therapeutic approach for maximizing favorable outcomes

in these patients. Dr. Bland describes the best clinical approaches, which identify the origin of altered function and individualize the treatment to the patient’s needs.

Stressed to Death: Aging, Long-term Stress, and Allostatic Load – Sonia Lupien, PhD

Stress is a concept that has been written about in medical texts and bandied about in popular culture for the past half-century, and yet we’re still grappling with its meaning and consequences. Dr. Lupien’s work has been instrumental in

helping to quantify the effects of chronic stress on memory and other brain functions. This talk explores results from her research, which suggest that there might be a “time window” in human aging during which interventions with at-risk individuals achieve the most benefit to prevent or modify cognitive impairments.

Stress, Prenatal Imprinting, and Childhood Developmental Disorders – Bethany Hays, MDA growing body of evidence suggests that the most rapidly increasing adult chronic diseases are being “programmed” in utero. In this talk, Dr. Hays provides the current state of the science regarding prenatal imprinting and health risks. Her presentation will help you understand the potential effects of imprinting to help you identify important risk factors and improve patient care.

The Widening Web of Celiac Disease and Thyroid Dysfunction – Thomas O’Bryan, DC

Celiac disease is significantly underdiagnosed, as many people present without the GI symptoms most commonly thought to be associated. Dr. O’Bryan proposes that gluten sensitivity be viewed as a

multisystemic disorder, with celiac disease as an autoimmune manifestation of that disorder. In this presentation, he outlines the criteria that clinicians need to know in order to appropriately test and monitor for gluten sensitivity.

Thyroid Assessment: Controversies and Conundrums – Alan McDaniel, MDAbnormal thyroid dysfunction has been defined as a thyroid-stimulating hormone (TSH) level above the reference range. However, the correlation between thyroid symptoms and TSH levels is still unclear, and there are many reasons why relying on laboratory assessment alone may misinform the clinician. In this presentation, Dr. McDaniel explains the relationship of thyroid dysfunction to specific signs and symptoms, as well as the relative merits and pitfalls of specific laboratory evaluation.

From David Quig, PhD, on Laboratory Assessment of Iodine and Selenium • Do not give high-dose iodide without first checking selenium because insufficient selenium increases oxidative stress in the thyroid gland and increases the risk of autoimmune thyroiditis. • Red blood cell selenium provides a good indication of selenium status, but whole-blood selenium gives a more complete picture of selenoproteins.

Hoption Cann, PhD, on Iodine Intake, Thyroid Function, and Cardiovascular Disease Risk • Almost 25% of women in the United States show evidence of iodine deficiency. • The high consumption of seafood in Japan is linked to the highest population intake of iodine. Their rates of coronary heart disease are remarkably lower than in the West, while life expectancy is higher. Iodine intake in excess of the RDA (150 mcg per day for adults) may offer significant benefits to cardiovascular function. • Following the initiation of iodine supplementation in deficient populations, there is a marked rise in thyroglobulin and TPO antibodies, found in autoimmune thyroiditis. Therapeutic use of selenium has been shown to decrease antibody concentrations Combined iodine and selenium may be indicated for many patients, and seafood is an excellent dietary source.
