k!mp!sisi dari pen)eluaran kesehatan masyarakat terhadap sum(er penerimaan3 Data
utilisasi dari masyarakat dan pelayanan s0asta untuk men)etahui resp!n demand

Desentralisasi
Ke(ijakan desentralisasi dimaksudkan untuk men*iptakan pr!ses pen)am(ilan
keputusan pu(lik yan) dem!kratis dan mem(erikan pelayanan masyarakat yan) jauh
le(ih (aik3 Tujuan kesejahteraan mensyaratkan pemerintah daerah untuk (isa
menyediakan layanan pu(lik kepada masyarakat l!kal se*ara ek!n!mis- tepat )una dan
sesuai ke(utuhan3 8alu- desentralisasi ju)a diharapkan (isa men)uran)i kemiskinan
le0at- misalnya- penin)katan trans+er dana spesi+ik untuk sekt!r6sekt!r tertentu yan) jadi
pri!ritas karena (ersin))un)an den)an !ran) miskin3
Desentralisasi sesun))uhnya merupakan alat9instrument untuk me0ujudkan tata
kel!la pemerintahan yan) e+isien dan partisipati+3 Harus dipahami (ah0a desentralisasiadalah instrumen yan) k!mpleks sedemikian sehin))a tidak (isa hanya dikaitkan den)an

Desentralisasi fiskal telah dimulai dijalankan secara penuh pada tanggal 1 Januari
2001. ,elaksanaan desentralisasi fiskal yang dilaksanakan berdasarkan UU No. 22 Tahun
1999 tentang Otonomi Daerah dan UU No. 25 Tahun 1999 tentang Perimbangan Keuangan
antara Pemerintah Pusat dan Pemerintah Daerah.

Dampak dari sumber daya pemerintah Kabupaten terhadap pengeluaran kesehatan
masyarakat
'erdasarkan hasil penelitian- penerimaan dan pen)eluaran kesehatan masyarakat
menin)kat se*ara si)ni+ikan selama 5 tahun pertama desentralisasi di Ind!nesia3 T!tal
perkapita penerimaan ka(upaten menin)kat dari Rp354;3$=>- di tahun "##4 menjadi
Rp3;?23$25 di tahun "##;3 ,enerimaan yan) ter(esar (ersum(er dari al!kasi DAU %Dana
Al!kasi Umum.- tetapi men)alami penurunan dari >;< di tahun "##4 menjadi ??< di
tahun "##53 ,en)eluaran kesehatan masyarakat di ka(upaten ju)a men)alami
penin)katan- dari Rp3"?3#;> pada tahun "##4 menjadi Rp3543$;$ di tahun "##53 An)ka

Selain itu- pem(erlakuan hi(ah DAK- yan) diperuntukkan (a)i in+rastruktur-menin)katkan pen)eluaran pem(an)unan dalam jumlah yan) nyata- serta (ersi+at relati+
terhadap pada pen)eluaran kepe)a0aian3 Namun demikian- setelah tahun "##5- trend ini
tampaknya (er(alik- den)an pemerintah pusat sekali la)i meme)an) peranan yan) (esar
dalam hal pen)eluaran pem(an)unan.
Aliran dana dari pusat ke ka(upaten9k!ta san)at mempen)aruhi pemerataan-

antar pemerintah menemukan (ah0a ka(upaten miskin merupakan penerima utama
pendanaan sejak di(erlakukannya desentralisasi3 Studi terse(ut menyimpulkan (ah0a
