Instructions for Completing the E-ZPass Business Application 1 PART 1. COMPANY INFORMATION – (Page 2) Clearly print account information. If you are opening a Business Account under a company name, the name of the account must be the full legal c ompany name. If you are an individual opening a Business Account because you need five or more transponders,  please fill in your name under “Billi ng Contact.” Whether you are a company or an individual opening a business account you must select a four-digit Personal Identification  Number (PIN). Your PIN will be used for identificat ion purposes when you access your account via the automated phone system or website. Please record your PIN here and retain it for future reference: PART 2. VEHICLE INFORMATION – (Pages 3 & 4) Please provide requested information for each vehicle listed on the Vehicle Information Forms. Page 3 – For Two-axle vehicles Clearly print license plate number, state of registration and vehicle reference number (refer to vehicle reference chart on pages 6 & 7). Check the box under the  plan you want for each vehicle listed. The Business Plan is for casual use and the Baltimore Region, Bay Bridge, Bay Bridge Shoppers and Nice Bridge plans are discount plans. Refer to E-ZPass Plans on page 5 for plan expl anations. Circle the transponder type you are requesting. Circle “NONE” for vehicl es you are listing  but do not require a transponder at this time. DO NOT select any plans for vehicles where you circled “NONE” for transponder type. Box A - Total Number of Interior Windshield Transponders Add the number of vehicles that have “Windshield” circled. Enter the total in Box A. Box B - Total Number of Exterior License Plate Transponders Add the number of vehicles that have “License Plate” circled. Enter the total in Box B. Page 4 – For Three or more axle vehicles Clearly print license plate number, state of registration, vehicle reference number (refer to vehicle reference chart on pages 6 &7), and the number of axles. Circle the transponder type you are requesting, either interior windshield, interior CVSIN, exterior roofmount, or exterior license plat e. Circle “NONE” for vehicles you are listing but do not require a transponder at this time. Box C - Total Number of Axles Add the “Number of Axles” column. Enter the total in Box C. Boxes D & E - Total Number of Interior Transponders (CVISN (Fusion) transponders are for the E-Screening program)  Add the number of vehicles that have “Windshield” or “CVSIN” circled. Enter the totals in Boxes D and E. Box F - Total Number of Exterior Roofmount Transponders Add the number of vehicles that have “Roofmount” circled. Enter the total in Box F. Box G - Total Number of Exterior License Plate Transponders Add the number of vehicles that have “License Plate” circle d. Enter the total in Box G. Note: Do not order windshield transponders for vehicles that have flat, perfectly vertical windshields or for vehicles which have any metal hanging over the windshield (i.e., visor, storage rack, boom or crane). If your vehicle fits this description, you must order an exterior transponder. PART 3. PRE-PAYMENT CALCULATION – (Page 2) To open an account, you must make an initial payment that includes the purchase of transponders and an advance toll payment amount for each transponder for two-axle vehicles and for each axle for three or more axle vehicles.   E-ZPass Maryland business account holders will be enrolled in the Post-Usage Discount Plan and the Supplemental Rebate Plan for vehicles with three or more axles. The Post-Usage Discount Plan is account specific and is based on all transponders used on the account. The Supplemental Rebate Plan is transponder specific and is based on the number of trips used by a single transponder. For more information on these discount plans, ple ase refer to E-ZPass Plans on page 5. Note: Tolls paid by any two-axle vehicles that may be registered on your account are not included in calculating the Post-Usage Discount Plan or the Supplement Rebate Plan. PAYMENT METHOD – (Page 2)  OPTION 1: CREDIT CARD REPLENISHMENT There are many advantages to opening and r eplenishing your  E-ZPass account with a credit card:  It’s easy. There’s no need to worry about a separate E-ZPass payment.  It’s automatic. As long as your credit card account is in good standing, your account balance will never be depleted. To replenish by credit card, place a check mark in the box to the left of “Option 1” of Part 4 and provide a valid credit card number, type, expiration date, and signature.  OPTION 2: CHECK OR CASH REPLENISHMENT To open and replenish your account by check or cash, select Option 2 of Part 4. Cash payments must be made in person at an E-ZPass Maryland Stop-In Center. Checks must be made payable to “  E-ZPass Maryland” and may be mailed with your application to E-ZPass Maryland, P.O. Box 17600, Baltimore, MD 21297- 7600. Cash and check payments must be for the amount shown in Box 15 of Part 3. PART 5. CUSTOMER AGREEMENT – (Page 2) This section must be signed by a duly authorized representative of the company. ** IMPORTANT INFORMATION ** AVOID FEES – It is important to keep your account information current to reduce the likelihood of additional fees. Account updates and inquiries may be made at www.ezpassmd.com or by phone at 1- 888-321-6824 to: Review account information. Update credit card information. (CVISN (Fusion) transponders are for the E-Screening program) Add the number of vehicles that have “Windshield” or “CVSIN” circled. Enter thetotals in Boxes D and E.

Box F - Total Number of Exterior Roofmount Transponders

Add the number of vehicles that have “Roofmount” circled. Enter the total inBox F.

Box G - Total Number of Exterior License Plate Transponders

Add the number of vehicles that have “License Plate” circled. Enter the total inBox G.

Note: Do not order windshield transponders for vehicles that have flat, perfectly verticalwindshields or for vehicles which have any metal hanging over the windshield (i.e., visor,storage rack, boom or crane). If your vehicle fits this description, you must order anexterior transponder.

PART 3. PRE-PAYMENT CALCULATION – (Page 2)

To open an account, you must make an initial payment that includes the purchase of transponders and an advance toll payment amount for each transponder for two-axlevehicles and for each axle for three or more axle vehicles.  E-ZPass Maryland businessaccount holders will be enrolled in the Post-Usage Discount Plan and the SupplementalRebate Plan for vehicles with three or more axles. The Post-Usage Discount Plan is

TWO-AXLE VEHICLES

Business Plan OptionThe Business Plan allows you to enroll in the E-ZPass 

program and pay the full-fare toll at all E-ZPass 

equipped toll plazas.No minimum use is required. 

Discount Plan (for two-axle vehicles)If you enroll in an E-ZPass Maryland two-axle vehiclediscount plan, you will automatically have Standard Planprivileges at all other E-ZPass equipped toll facilitieswhere you do not have a discount plan. All two-axlevehicle discount plans are time sensitive. Trips will bededucted from your account as they are taken. Once youbegin a discount plan cycle, you will not receive a refundfor any remaining trips in that cycle. Any unuseddiscount trips will be deducted from your account afterthe plan cycle ends.

THREE OR MORE AXLE VEHICLES

Discount Plans E-ZPass

 Maryland business account holders will beenrolled in the Post-Usage Discount Plan and theSupplemental Rebate Plan for vehicles with three ormore axles. Discounts for both plans are credited toaccounts thirty (30) days after completion of a cycle.

Post-Usage PlanThe Post-Usage Discount Plan is account specific and isbased on all transponders used on the account at eligiblefacilities. The cycle begins with the first toll transactionafter the plan is added to your account and runs for thirty(30) days. Calculation of the post-usage discount isbased on the following and does not include anyapplicable fees that may have been deducted from youraccount during the calculation period.

Supplemental Rebate PlanThe Supplemental Rebate Plan is transponder specific and is based on the number of trips used by a singletransponder at eligible facilities. The cycle begins whenyou open your account and runs from anniversary date toanniversary date each month. A 10% discount will beoffered for a single transponder that makes 100 or moretrips in an anniversary period.  Because tolls are onlycollected in one direction at the John F. KennedyMemorial Highway, Thomas J. Hatem Memorial Bridge,Governor Harry W. Nice Memorial Bridge and theWilliam Preston Lane, Jr. Memorial (Bay) Bridge, onetrip on these facilities will be counted as two trips.
