3 Education and Scholarship
3.1 Undergraduate lectures/tutorials per year:
Lectures: I lecture to 4th year medical students in Medicine (Cardiomyopathy) and
Pharmacology (Rheumatic Fever). I give 8-12 lectures/seminars of 1 hour each
per year. The objective of this course is to provide a sound theoretical basis in
clinical cardiology.
Tutorials: I give a 1-hour weekly tutorial to 6th year medical students on clinical
methods. I give approximately 26 of these tutorials per year. The objective of
this course is to equip medical students with practical skills in the clinical
assessment of patients.
Examinations: I examine fourth and sixth year medical students in clinical internal
medicine. There are approximately four 2-hour sessions per year.
3.2 Postgraduate lectures/tutorials per year:
Tutorials: I conduct clinical tutorials for medical registrars preparing for the Colleges of
Medicine examination (approximately 10 sessions of 1 hour each per year).
Examinations: I am an examiner for the Fellowship for the College of Physicians
examinations of the Colleges of Medicine of South Africa. This activity involves setting
questions and marking scripts, including the participation in clinical examinations.

There are two contributions of this study to new knowledge. First is the discovery
of founder genetic effect in the plakophylin 2 gene in 4 Afrikaner families with
arryhthmogenic right ventricular cardiomyopathy (ARVC). An additional familiy
has subsequently been found, and these families with a disease on the same
haplotype have been expanded to about 100 individuals - setting up a unique
global resource for genotype-phenotype correlation studies in ARVC. The second
observation was the identification of a high mortality associated with syncope and
ventricular arrhythmia. These data have convinced medical insurance companies
and the government services to provide implantable cardioverter defibrillators for
high-risk ARVC patients before they have an event.

Characteristics, complications, and gaps in evidence-based interventions in rheumatic

REMEDY is the first multi-centre study of rheumatic heart disease in the world. It
is also the largest study which has proved the key information that is required for
the implementation of measures to prevent the complications of rheumatic heart
disease. The paper has not only highlighted the low uptake of effective
interventions (i.e., penicillin for secondary prevention and warfarin for prevention
of stroke), it also reveals the dire neglect of reproductive services for women who
are affected with rheumatic heart disease. REMEDY provides the global platform
for the testing of mechanisms for achieving the target of 25% reduction in
morbidity and mortality that has been set for ministries of health in affected

This study has provided a new standard for the rapid diagnosis of tuberculous
pericarditis. It showed that the measurement of unstimulated interferon gamma
levels in pericardial fluid is superior to the existing rapid screening test (adenosine
deaminase level) and the new rapid test (Gene Xpert or polymerase chain reaction
based DNA test for TB) in the diagnosis of tuberculosis. An affordable and
accessible interferon gamma based diagnostic test is being developed for clinical
use by our group. The findings of this study have set a new standard for clinical
practice.

The IMPI trial is a landmark study in tuberculous pericarditis. The ground-
breaking findings have changed clinical practice on the use of glucocorticoids in
the treatment of the disease. This is the largest clinical trial in pericarditis in
history, with the largest number of HIV-infected patients. The finding that

immunotherapy is associated with an increased incidence of cancer has major
implications for the development of host-directed vaccines and therapies for
tuberculosis. The implications of the findings of the IMPI trial for clinical practice
are that adjunctive steroids should be used selectively in tuberculous pericarditis
with preference for HIV negative patients in order to avoid cancer. Clinical
guidelines are being aligned to these findings.

600 delegates in 2014 – making it one of the largest meetings of General Physicians

Convenor, All Africa Conferences on Heart Disease, Diabetes and Stroke for the Pan
I convene the 2-yearly Pan African Society of Cardiology Congresses (Ghana 2004;
Kenya, 2007; Nigeria, 2009; Uganda 2011; Senegal 2013; Mauritius 2015). This
meeting has grown from a meeting of 40 people in 2004 to over 800 delegates in
Senegal. It is the premier clinical and scientific meeting on cardiovascular disease in
Africa.
6 Research projects in progress
My research will continue to focus on the two fields of (1) poverty-related heart diseases,
and (2) genetics of heart disease and fibrosis
Poverty-related heart disease
This work addresses the clinical characteristics, outcome, pathogenesis, treatment and
prevention of tuberculous pericarditis, rheumatic heart disease, cardiomyopathy and
myocarditis.
Tuberculous pericarditis
The Investigation of the Management of Pericarditis (IMPI) study programme is
proposing the continuation of the IMPI registry of thousands of patients with pericarditis
at the collaborating sites in Africa and other parts of the world. The registry will support
new clinical trials and mechanistic studies. The Second Investigation of the Management
of Pericarditis (IMPI-2) Trial has been funded in part by the South African Medical
Research Council to investigate the role of compete pericardial drainage by
intrapericardial fibrinolysis in reducing the incidence of constriction and tamponade in
patients with large pericardial effusion. IMPI-2 which will recruit 2,000 patients will also
investigate the optimal diagnosis of tuberculosis using a cohort design.
Rheumatic heart disease
We have recently completed a study of the prevalence of rheumatic heart disease in 4,700
schoolchildren in Cape Town, South Africa and Jimma, Ethiopia. We believe that a large
scale study of 100,000 school children is required to determine the nature and outcome of
silent rheumatic heart disease that is being revealed by echocardiographic screening
studies in schoolchildren. The Global Rheumatic Heart Disease Registry (the REMEDY
study) is planning to report the findings of the 2-year follow-up component of the registry
by the middle of 2015. The plan is to scale up the pilot phase of REMEDY from the
current 3,000 patients (mainly in Africa) to 30,000 patients recruited from all endemic
countries in the world. We plan to use this global platform to conduct clinical trials of

new oral anticoagulants and other interventions in RHD. Furthermore, we plan to extend
the studies of the molecular epidemiology of streptococcus pyogenes pharyngitis to all
countries in Africa, as a prelude to vaccine trials of on the continent, in collaboration with
colleagues in the USA and Australia.
Cardiomyopathy and myocarditis
We are in the process of establishing a multi-centre African Cardiomyopathy and
Myocarditis Registry Programme (the IMHOTEP study) to examine the clinical
characteristics, outcome, treatment, genetics and barriers to care on the continent. The
IMPOTEP protocol, which has received seed funding from the South African Medical
Research Council, is being piloted at Groote Schuur Hospital and will be ready to be
launched at the end of 2015. IMHOTEP will capitalise on the networks and success of
IMPI and REMEDY.
Genetics of heart disease and multi-system fibrosis
Heart disease
We have been funded by the Wellcome Trust to conduct a genome-wide association
study of 3,000 cases of rheumatic heart disease and 3,000 controls to identify
susceptibility and protective factors. We have already recruited 500 cases and 500
controls, and will complete the study at the end of 2016. This study is likely to provide
information on genetic risk factors for RHD which may be used to identify high-risk
individuals for preventive measures (e.g., vaccine).
We have 1000 patients with cardiomyopathy in our database who have DNA available
for analysis. We plan to conduct mutation screening of the 50 candidate genes for
cardiomyopathy, in collaboration with Danish collaborators during the course of this
year.
Multi-system fibrosis
We have identified an inherited form of multisystem fibrosis that is due to mutations in
FAM111B. Very little is known about the tissue expression, function, ligand of
FAM111B, such that the pathogenesis of fibrosis in this disease is unknown.
We have been funded by the National Research Foundation to pursue the following
scientific objectives:
1. To determine the tissue expression and cellular localisation of fam111b in normal
and affected individuals;
2. To delineate the function and substrates of fam111b;
3. To develop models of FAM111B-related hereditary fibrosis using induced
pluripotent stem cells.
Methods: We are using state of the art techniques that are established in the laboratories
of Professors Dirk Lang (Pathology), Edward Sturrock (Biochemistry), and Susan Kidson
(Cell Biology) to pursue the objectives of this study.
Significance: The elucidation of the pathology, biochemistry and cell biology of
FAM111B-related disease will offer new insights into the pathogenesis of fibrosis, and

identify new strategies and targets for treatment in the future. This study serves to train
seven postgraduate students at postdoctoral (n = 2), doctoral (n = 1), and masters (n = 1)
levels.
6 Leadership and Administration
7.1 Professor and Head of Department of Medicine at Groote Schuur Hospital
and University of Cape Town (2006 – present)
I have had the pleasure and privilege to lead the Department of Medicine at
Groote Schuur Hospital and the University of Cape Town for nearly 10 years. The
UCT/GSH Department of Medicine is a large and complex clinical department
which plays a leading role in medical education and research, and provides
clinical services to the communities of the Western Cape and, in the case of our
highly specialised services, to patients throughout southern Africa. The
Department consists of 21 clinical and service divisions, 11 major research units,
and 5 affiliated departments of medicine at Mitchell’s Plain Hospital, New
Somerset, II Military, George and Victoria hospitals, the secondary/district level
teaching hospitals that are affiliated to GSH, and Brooklyn Chest hospital, a
dedicated TB hospital.
o Our Staff – we have 16 Professors, 22 Associate Professors, 25 Senior
Lecturers, 14 Clinical Scholars / Postdoctoral Fellows, 18 Senior
Registrars, 49 Registrars, and 20 Supernumerary Trainees (i.e., Self-
funding Registrars and Senior Registrars from other South African
institutions or other countries such as Botswana, Ghana, Kenya, Libya,
Namibia, Swaziland, Zambia, and Zimbabwe), who are supported by 36
permanent administrative and clerical staff employed by UCT.
o Our Clinical Service – our clinical activities take place at Groote Schuur,
II Military, Victoria, Mitchell’s Plain, New Somerset and Brooklyn Chest
hospitals in the western metropole of Cape Town. Each day, we serve
more than 2,000 patients in our clinics, admit 100 patients to our hospital
services and perform 210 procedures. In addition to a comprehensive
range of specialist and subspecialist service, we offered a number of
highly specialised services that are unique nationally such as bone marrow
transplantation, liver transplantation, cardiac electrophysiology and a
Stroke Unit.
o Our Education Programmes – we have a medium-sized registrar and
senior registrar training programme with 77 registrars and senior registrars
in 10 subspecialties of Medicine. The Postgraduate Training Committee
supervises the professional and academic training programme. We also
teach over 1000 undergraduate medical students in Years 2-6 of the 6 year
MBChB programme, and 155 students in occupational therapy and
physiotherapy.
o Our Research – over the last 10 years, we have made significant
contributions to the diagnosis, treatment and prevention of human
suffering through research. We have experienced a significant increase in

quantity and quality of research outputs over the past. The number of
publications has increased from just under 100 articles in 2005 to about
460 publications in 2014. Our researchers publish regularly in leading
international journals including the New England Journal of Medicine,
The Lancet, and PLoS Medicine.
Over the past 10 years, there has been significant progress made towards
achieving excellence in the clinical service, teaching, and research at the
UCT/GSH Department of Medicine. This push towards excellence has been
achieved through the implementation of a wide range of transformative initiatives
which are described in the Photo Essay that is attached and entitled ‘A Week in
the Life of the UCT/GSH Department of Medicine – Preparing for the next 100
Years’.
