EXHIBIT C-1 SUBCONTRACTOR SAFETY REQUIREMENTS

In addition to what is required in by the General Contract Documents, SUBCONTRACTOR (including its tier subcontractor(s)) is required to observe all safety rules established for protection and guidance, abide by all laws and regulations set forth by the local, state, and/or federal government and CONTRACTOR, use appropriate safety equipment, and always work using safe methods.

MINIMUM SAFETY QUALIFICATION

SUBCONTRACTOR may not have an Experience Modification Rate (EMR) for the last three (3) years greater than or equal to (Š) 125 for two (2) years or more. Any exceptions must be approved by CONTRACTOR.

SUBCONTRACTOR will obtain the EMR from any of its tier subcontractor(s) for the last three (3) years and may not use any tier subcontractor that has an EMR that is greater than or equal to (Š) 125 for two (2) years or more. Any exceptions must be approved by CONTRACTOR.

SUBCONTRACTOR not meeting the minimum safety qualification may request Special Care. Special Care will be approved at CONTRACTOR’S sole discretion. Any costs associated with the Special Care requirements shall be borne at SUBCONTRACTOR’S sole cost.

SAFETY PRECONSTRUCTION MEETING

Prior to commencement of work in the field, SUBCONTRACTOR will participate in CONTRACTOR’S Subcontractor Safety Preconstruction Meeting.

The SUBCONTRACTOR’S Field Superintendent/Supervisor and Project Manager are required to attend.

SAFETY ORIENTATION

Prior to commencing Work, every employee of SUBCONTRACTOR working at the project must complete the CONTRACTOR’S Subcontractor Safety Orientation.

SUBCONTRACTOR will not allow any employee that has not completed the CONTRACTOR’S Subcontractor Safety Orientation to work at the jobsite.

DRUG TESTING

SUBCONTRACTOR certifies that its employees who will work on the jobsite have had pre−employment drug screens and that the results of the drug screens for each employee assigned to this project were negative.

SUBCONTRACTOR must also agree to conduct post-accident drug screens on all and/or any employees involved in an occurrence on or about the project site, including, but not limited to altercations; accidents involving bodily injury to the employee or bystander; possession of any intoxicant (including, but not limited to, alcohol, amphetamines, barbiturates, cocaine, narcotics, PCP, marijuana, misuse of prescribed drugs, and any other substance that adversely alters one’s senses or affects one’s ability to

function in his/her job in a safe, productive manner); accident involving damage to any property; and or action/behavior which reasonably could have resulted in an accident or altercation.

SUBCONTRACTOR will not allow any employee with a positive drug test result (pre−employment, post− accident, and/or for cause) to work at the jobsite, except where SUBCONTRACTOR has a policy to return to probationary work provision within their drug policy.

PERSONAL PROTECTIVE EQUIPMENT FOR FIELD CRAFTSMAN

Long pants, shirt with shoulders covered, and work boots (no sneakers, tennis, or casual−type footwear) are required at all times while on the jobsite.

Hard hats & eye protection are required at all times while on the jobsite. In addition, hand protection will be required at all times for field craftsmen.

Any/all other personal protective equipment that is required to perform the work safely and/or is required by law must be made available to your employee(s), the employee(s) must be trained (if required), and the equipment must be in good condition, and used properly.

PERSONAL PROTECTIVE EQUIPMENT FOR NON-FIELD PERSONNEL

SUBCONTRACTOR’S non−field personnel are required to wear work boots (no sneakers, tennis, or casual−type footwear), hard hats, eye protection, and safety vests at all times while on the jobsite.

SAFETY PRE-PLANNING

A Job Hazard Analysis (JHA) is required for the following High Hazard Activities and must be submitted prior to start of work at the Subcontractor Safety Preconstruction Meeting:

∉ Confined Space;

∉ Hoisting/Rigging Activities (including cranes, derricks, etc.);

∉ Demolition Activities & Hazardous Materials Assessment (asbestos, lead, biohazards or other chemicals in the workplace, as well as general demolition hazard assessment);

∉ Framing Activities;

∉ Excavation & Trenching;

∉ Fall Hazards (6’ and overhead work); ∉ Public Exposure (phased occupancy, partial demolition, traffic control, etc.); and

∉ Scaffolding.

SUBCONTRACTOR is required to complete and review Pre−Task Planning Cards daily and/or with each new activity. Each employee of the SUBCONTRACTOR is required to understand and be familiar with the safe practices and hazards associated with his/her task. The foreperson of each crew is required to have the Pre−Task Planning Card available at all times. Pre−Task Planning Cards must be turned into CONTRACTOR weekly.

FALL PROTECTION

Fall protection is required at six feet. If it is impracticable, an alternate plan must be submitted and approved by CONTRACTOR prior to start of work.

UNSAFE ACTS, HAZARDOUS CONDITIONS & EVENTS

SUBCONTRACTOR is required to promptly report any unsafe acts or hazardous conditions to CONTRACTOR.

SUBCONTRACTOR is required to report any/all incident, accident, or close−call (near miss), no matter how minor, to CONTRACTOR within 24 hours of the event.

An incident is any event that causes property damage. An accident is any event that causes personal harm/bodily injury. A close−call (near−miss) is any event that could have caused either an incident or an accident.

SAFETY MEETINGS & SAFETY AUDITS

SUBCONTRACTOR is required to conduct a weekly safety meeting with its employees and provide documentation to CONTRACTOR.

Each employee of SUBCONTRACTOR is required to attend a jobsite wide monthly safety meeting conducted by CONTRACTOR as well as any other special safety meeting as determined by CONTRACTOR.

SUBCONTRACTOR is required to conduct weekly jobsite safety inspections of its work. These inspections must be submitted weekly to CONTRACTOR.

TRAINING/CERTIFICATIONS

SUBCONTRACTOR will provide a minimum of one 10−Hour Occupational Health & Safety in Construction trained employee while performing work at the jobsite. Additional training may be required at the discretion of CONTRACTOR.

SUBCONTRACTOR will provide one designated Competent Person as required by law (e.g. Trench/Excavation, Scaffold, etc.).

SUBCONTRACTOR will provide licensed operators while using equipment at the jobsite (e.g. Forklift, Scissor lift, Cranes, etc.).

SUBCONTRACTOR will provide a minimum of one person at the project who is CPR/First Aid/Blood borne Pathogen certified.

STORM WATER POLLUTION PREVENTION PLAN (SWPPP) SUBCONTRACTOR is required to comply with the SWPPP, including but not limited to, compliance with and enforcement of the Best Management Practices established by the SWPPP, adherence with the Erosion and Sediment Control procedures and practices, proper storage of materials, and inspections.

SUBCONTRACTOR shall comply with the lawful requirements of any applicable municipality, the District, drainage district, and other local agencies regarding discharges of storm water to separate storm drain system or other watercourses under their jurisdiction, including applicable requirements in municipal storm water management programs.

FEES AND COSTS

In addition to all other remedies available to CONTRACTOR, CONTRACTOR’S Risk Management Department in its sole discretion may charge SUBCONTRACTOR any amounts associated with SUBCONTRACTOR’S failure to comply with any of these Safety Requirements. These charges may include, but not be limited to:

∉ Current T & M Rates for CONTRACTOR’S Risk Management staff time

∉ $50 per SUBCONTRACTOR’S employee who has not completed the Subcontractor Safety Orientation prior to arriving at the Project

∉ Safety Violations; • First Offense: Verbal Warning • Second Offense: Written Warning Plus $500 Fine • Third Offense: $1,000 Fine • Subsequent Offenses: $2,000 Fine Per Each

CONTRACTOR will issue the SUBCONTRACTOR a deductive change order for any items listed above.

MODIFICATIONS

CONTRACTOR reserves the right to amend any/all requirements herein.

SUBCONTRACTOR SAFETY SUBMITTAL CHECKLIST

SUBCONTRACTOR is required to submit the following Project specific documentation prior to

commencing WORK:

1. Site Specific Safety Plan, including but not limited to:

a. Injury and Illness Prevention Program (IIPP)

i. Must contain name of person responsible for the program.

b. Competent/Qualified Person list & documentation

c. Name of onsite Designated Safety Person with OSHA 10−Hour Occupational Health &

Safety in Construction certification.

d. Job Hazard Analysis (JHA)

e. Fall Protection Plan

f. Name of onsite CPR/First Aid/Blood borne certified personnel

g. Training documents applicable to work

h. Emergency Response Plan

2. Code of Safe Practices

3. Heat Illness Prevention Plan

4. Hazard Communication Plan with copies of site specific Material Safety Data Sheets (MSDS).

5. Crane Operations

a. Documentation of operator certification

b. Documentation verification of Riggers & Signal Persons

c. Annual and four year inspections must be submitted

d. All cranes must be equipped with an anti−two block device.

Additionally, SUBCONTRACTOR must submit the following documentation throughout the performance

of the Work:

1. Toolbox Safety Meeting Reports must be turned in weekly.

2. Pre−Task Planning Cards must be completed and reviewed with crew daily and turned in weekly.

3. Jobsite safety inspections must be conducted weekly and turned in weekly.

4. Documented equipment inspections must be made available upon request.

5. Accident, incident, and close−call (near miss) reports must be submitted to CONTRACTOR’s

jobsite office within 24 hours of event.

6. Updated documentation as needed (i.e. designated safety person, etc.).

7. Any other documentation as may be required while performing work.

SUBCONTRACTOR will be required to provide any and/all additional documentation as requested by

CONTRACTOR.
