1. Administration and Other Matters At the beginning of 2007 Community Building Credible Ownership (CBCO) South Africa Gauteng requested an office space in ELCSA CD Diakonia AIDS Ministry premises and they were allocated part of the Support Group Zozo at no charge. On 16th January 2007 – Ms. Drea Froechtling arrived in Soweto to assume her Bishop’s Chaplain Position and also help ELCSA CD Diakonia AIDS Ministry as a Pastor and fundraiser. Upon retirement of Rev. Z. S. Senosi as Coordinator of ELCSA CD Diakonia AIDS Ministry in March 2007, the Administrator led the organization until Rev. S.S. Mugivhi came on officially in January 2008. The Coordinators’ position is still not funded directly. Evangelical Lutheran Church in Southern Africa – Central Diocese supports Diakonia AIDS Ministry with Human Resource as they second pastors to the Ministry. On 27th January 2007, a special general meeting was postponed because of many apologies. This meeting was to look into the financial statements and approve them following rectifications of errors. On 31 January 2007, DAM officials had a meeting with Mr. Tshwanelo Shabe (facilitator of strategic planning) and agreed to R200 per hour facilitation of Strategic Planning Session. The facilitator was also responsible for writing a report of the session. 2. Submission to Directorate of NPO’s On 05 February 2007, a report for financial year 2005/06 was send to Directorate of Non Profit Organization’s. 3. DAM Tenants 3.1. Accommodation VSO volunteer ELCSA CD Diakonia AIDS Ministry housed a volunteer Raj Raman from VSO (Voluntary Service Organization) from April 2006. Mr. Raj Raman vacated the cottage on 05 May 2007 and went back to India. Drea Froechtling moved into the cottage due to her health condition. 3.2. Gauteng Volunteer Association (GVA) This organization rented part of ELCSA CD Diakonia AIDS Ministry offices for six months in 2006. After this period its chairperson (Mr. Ramoetsane) was called-in to

discuss removal of their property in the office of DAM. Mr. Ramoetsane paid R500 and removed GVA properties. 3.3. Soweto Circuit This Circuit expressed interest of using office space (Unit Alpha) for setting up the Circuit office. They started occupying the office on March 2008 and will be paying R600 per month over 12 months. 3.4. New International Volunteers coming to serve DAM 3.4.1. German Partners Berlin Mission Werks (BMW) A volunteer Fadl Dia (18yrs) came to serve Diakonia Aids Ministry from mid August 2007. This is as a result of an on going programme with Berlin Mission Werks (BMW). Evangelical Lutheran Mission (ELM) Ms. Drea Froechtling drawn up a new contract for ELM volunteers following engagement with Bishop, Deans and office of DAM 3.4.2. American Partners Mmapula Christiana Metzger from Evangelical Lutheran Church in America (ELCA) came to South Africa on 25 September 2007 and she will be with DAM for 24 months. She assumed a responsibility of Education and Training Officer. 4. Home Based Care 4.1. Overall objectives

To provide palliative care to sick and bed ridden individuals who are suffering from

chronic diseases and living with HIV/AIDS.

4.2. The strategy

To provide a holistic care to clients with physical, emotional/psychological, spiritual and

material support and strengthen emotionally those who are affected.

Each caregiver provided her clients with a basic health assessment, psycho – social

support, escorted them to special services (hospitals/clinics), referred them to relevant

social services such as Department of Social Development, Department of Home Affairs

provided care, for instance bed bathing, feeding, cleaning the room the sick person lives

in. They provided adherence monitoring for tuberculosis and ARV treatment and advised

their clients on nutrition.

Counseling forms a pivotal element of Home Based Care programme as it enable clients

draw energy and utilize it positively.

4.4. Positive Outcomes

- Through our Home Based Care project clients who were bed bound/ridden are able to

walk, hence their health conditions improved. Those who are HIV positive were

encouraged to join Diakonia AIDS Ministry support group.

- Following coaching on HIV positive status disclosure some Clients disclosed their

status to their family members.

- Caregivers also provided education regarding HIV/AIDS, Tuberculosis and on hygiene

as most clients have basic understanding of HIV/AIDS and other chronic diseases

(including their family members) and are able to take precautions.

4.5. Negative Outcome

- Clients who disclosed their status to their families experienced stigmatization and

discrimination.

- Some of the clients tended to be over dependent on caregivers. They do not want to take

the responsibility for their own health.

- Some de-motivated clients do not seek medical assistance from the clinic and hospital.

- Other clients once their health conditions improve, claimed that their health condition

has not yet improved, as a result they are seeking ongoing attention from their caregivers.

- Family members and significant others are also over depended on the caregivers for bed

bathing clients, cooking and cleaning, hence not taking any responsibility for their

family members (clients)

- Some clients tended to have low self-esteem

4.6. Number of Caregivers and Clients In this financial year 2007/08 Home Based Care Programme has 10 Caregivers (1 of which is a supervisor) and 9 Caregivers service a minimum of 5 clients. Whilst the programme specifically with client’s health and environment, Caregivers also engage in educational awareness campaigns of various forms such as Good Nutrition for Healthy Living and TB. This year a South Africa Condom Week (19 & 20 February 2008) was organized in collaboration with Soweto HBC. It was a door to door campaign distributing condoms and pertinent information on preventable ailments around Molapo -Soweto. 4.7. Opening Home Based Care programme opened on 08th January 2007. During this day a brief review of 2006 was done. The meeting looked at challenges and successes of caregivers in the past year. As a result of this process, an objective for the programme this year was formulated. DAM HBC programmes’ objective is “to provide patient-centered care giving service to 400 clients within 12 months in Soweto”. 4.8. Social Worker Hiring During the week of 02 February 2007 interviews of Social Workers was carried out and Ms. Rahab Ramodibe was considered for the position following careful study of the interviewers’ recommendations. 4.9. Debriefing of Care givers Noting that Caregivers need sessions for debriefing, on 16th Jan 2007 DAM staff met with a Psychologist (Inga) to discuss possibilities of doing debriefing sessions for the care givers. The discussion led to a first session held on 28th February 2007. The session was excellent and Inga used combination of methods that empowers an individual to cope with emotionally exhausting circumstances. “This session brought relief to me, I never felt like this in my life” – one Care giver said. 4.10. Monitoring and Evaluation Monitoring and Evaluation process of the Care-giving programme was started on 20 February 2007. The intention of the process was to check weather Caregivers are doing their work properly and seek amicable solutions to problems that they encounter. The

process was implemented by Drea, Evelyn Saohatse and Annelie. It was completed by mid-March 2007.

4.11. Funding Application

On 22 February 2007 – The administrator had a meeting with Sister Nomathemba at Chiawelo Clinic for DAM HBC funding application and subsequently submitted additional required information. The application was submitted in October 2006. The response from the Gauteng Department of Health from March 2007 has not come forth even after follow ups with regional offices. To that end a letter was send to Lucky Hlatswayo of Gauteng Department of Health (HAST) to retrieve the outcome of the funding application submitted, yet to no avail. Recently three applications were submitted to LUCSA for funding i.e. Home Based Care, Support Groups and Orphan Care programmes. LUCSA subsequently rejected the application of Home Based Care on the basis that the two other proposals were approved. 4.12. Teambuilding Exercise A weekend of 07-09 December 2007 was a conclusion of the year by Home based Caregivers and Diakonia Aids Ministry staff members. A total number of 17 people took part in the weekend away and teambuilding session which was held in Magaliesburg. Evaluation of the team building session was done. Questionnaire was developed and distributed amongst the Caregivers to fill out. All questionnaires were filled out and returned The summary of the answers indicate that people found the exercise to be helpful and uniting them as a team. This session afforded them time to interact with each other and learn strengths and weaknesses of all team members. 4.13. Partnerships Established The 07-09 December 2007 retreat began with a visitation of Rejoice Home Based Care of Mathopestad in quest of sharing experiences and ideas. It was discovered that a group of women are leading the project without any formal training, essential medical supplies, funding and no stipend. They operate from a structure that was build for a clinic (currently not functioning). On 23 January 2008 a follow up meeting was held and concluded that:

1. DAM Caregivers will train and mentor Rejoice Care Givers for a week - 11 to15 February 2008 3 Caregivers participated.

2. Awareness Campaign (Good Nutrition implemented as a pilot project including Door to Door) took place from 14 to 15 March 2008

3. Exchange Programme - Rejoice Caregivers come to Soweto for capacity building programme and on the job training – 12 -16 May 2008

On 17th March 2008 an assessment of Mathopestad Awareness Campaign (Good Nutrition) was done with the Caregivers during a weekly Monday meeting.. 4.14. Training of Additional Caregivers Early this year Rev. Mugivhi, Mr. Magagane and Ms. Ramodibe engaged on a campaign to search for an accredited service provider – for Home Based Care training. The intention was to train 30 additional caregivers who will be deployed in Western, Eastern and Pretoria Circuit. It was discovered that service providers train people that are recommended by the Government either through extended public works programme or otherwise. The search for an independent and accredited service provider in continuing. 4.15. Donor Partner Visitation On 30th April 2007 Lucsa delegation led by Mrs Verna Mzezewa visited Diakonia Aids Ministry to discuss DAM project impact assessment on the community or beneficiaries. Caregivers, DAM Staff and support group participated in the session. The day was concluded by site visitation of the clients of Home Based Care givers which impressed the Lucsa delegation. 4.16. “A service to helpless senior citizens” On 12 September 2007 caregivers embarked on a cleaning up operation for a senior citizen family in Diepkloof. The living condition of the family was really bad. DAM further donated a bed and sponge for a patient on recommendation of the caregivers. 5. Orphan Care and Support

5.1. Overall objectives

To provide sustainable multifaceted mechanism that contributes to good livelihood of

Orphaned children.

5.2. The strategy

To engage both guardians and orphaned children through programmes dealing with

relevant issues affecting them.

5.3. Activities

The activities that are designed to reach individual child and guardian. In collaboration

with HIVSA children go on a school holiday camps. The Camp helps children with team

work skills; build their self esteem and creativity. On the one hand fellowship functions

have activities such as Talk With Guardians, Life Skills, Career Planning & Guidance,

Fun and Games.

In case children do not have proper identification documents or do not receive services

entitled to them such as Grants Diakonia AIDS Ministry intervene.

Counseling and Material support also forms part of the programme.

5.4. Positive Outcomes

- Good performance at school.

- Children have confidence.

- Disciplined children

- Guardians help each other to find solutions for problems they encounter with their

children.

5.5. Negative Outcome

- Dependency on the organization.

- Slowly changing negative attitudes of some Guardians towards children.

5.6. Number of Orphan children About 80 children access the organizations’ services. 5.7. Camp Sizanani Following a successful partnership with HIVSA (organization based in Chris Hani Baragwanath) Girls aged 13-16yrs went on camp that took place from 27th March to 08 April 2007. The camp was held in Magaliesburg north west of Johannesburg.

A follow up meeting with children revealed excitement and lessons they learned from the camp. 5.8. Winter Fellowship Function DAM hosted successfully a winter fellowship function for orphaned children on 04 July 2007 at the Diocesan Centre, in Soweto. 5.9. Planetarium Tour Select orphan children (13) participated on an educational tour at Johannesburg Planetarium aptly titled the Sky Tonight. They were introduced to the study of stars. This took place on Thursday 28 June 2007. 5.10. Christmas Orphan Fellowship Function Christmas Orphan Fellowship Function – Held at the Diocesan Centre on Wednesday 05 December 2007. Orphan children and parents from around the Diocese gathered at DAM. The event celebrated togetherness in Christ. The programme was divided into stations. There were fun and games for younger children, Life skills for teenagers and Talk with Guardians. The Games (such as soccer) was designed in order to encourage children to work together. Other games tested individual skills while working together on teams. Discussion with the Guardians was an opportunity for these dedicated mothers to encourage and support each other. They shared the experiences with the children which were mostly negative during sharing time. The activity was refocus when the facilitator Mr. D. Magagane asked Guardians if there anyone with a success story and how it came about. An old lady said: “I do not have problems with my child. The pillar of my strength is on praying for God’s blessings”. The stations lasted about 2 hours. The day concluded with lunch and distribution of blankets and food parcels to the families of the orphans. This programme has been granted R50 000 (Fifty Thousand Rand) by Lutheran Communion in Southern Africa (LUCSA). The funds will be used for the following:

1. OVC Fellowship Functions in Circuits – two circuits (Eastern and Western) already hosted the event on 26 March and 09 April 2008 respectively. Up coming events will be held in Soweto and Pretoria circuits. Whilst the traditional Diocesan events will be held on 02 July and 03 December 2008.

6. Support Groups

6.1. Overall objectives

To create systematic support unit that provides hope to hopeless.

6.2. The strategy

To provide physical, emotional/psychological, spiritual and material support and

strengthen emotionally those who are living with HIV/AIDS.

6.3. Activities

6.4. Positive Outcomes

- People who join the support group with little hope and dying – they spirits and morale is

boosted and get lively.

6.5. Negative Outcome

6.6. Number of members of the support group Diocesan based support group has plus 80 members who attend weekly group meetings. Male composition of this group has significantly increased, currently they constitute 12.5%. Orlando based support group has 22 members. They meet on Wednesdays. This project was established by Rev. T. Tjao and Ms. A. Makoane. It receives material support from Diocesan Office of DAM. 6.7. Opening for the year On 16th and 18th January 2007 Soweto Support Group officially opened. New leadership of Support group was elected and underwent a leadership training that was facilitated by the administrator. 6.8. Education As part of on going education- DVDs (supplied by Mindset) on Arv and treatment was screened for the support group. Members had an opportunity to ask questions and discuss issues raised by the television programme.

6.9. Diakonia Gospel Choir On 11 - 15 June 2007 the Diakonia Gospel choir was recorded a 10 song CD titled “Wake Up Call” in Johannesburg Meropa Productions. The CD has been send to all South African Broadcasting Cooperation (SABC) radios. A music video was subsequently shot. It will also be submitted to the broadcasting houses and be sold to the public.

To create communities that decisively uses pertinent information to positively change

their lives.

To provide trainings, workshops and awareness campaigns that are health related to

communities.

7.6. Gauteng Department of Health financially supported programme

Gauteng Department of Health released the funds for the HIV/AIDS Education and Training programme on 26 September 2007. The brief from the Gauteng Department of Health is to reach 4590 church leaders and member with HIV/AIDS education. Planning of the project was started by a committee of three (Mrs. S.L. Mogatusi, Rev. S.S. Mugivhi and Mr. D. Magagane) that were tasked by DAM Executive Committee. The meeting of the committee of three held on 26th October 2007 resolved that:

1. Mr. D. Magagane will be the project director. 2. Any member of the DAM shall not be entitled for payment should they carryout

work for the project. 3. The presenters shall not receive remuneration. 4. Other churches should be invited to participate.

The training of delegates from the four circuits of the Diocese took place from 05-09 November 2007 at the Central Western Jabavu. The financing of this project was for the period July 2007 to March 2008. It is of great importance that it is highlighted that the lateness of funds disbursement from the Gauteng Department of Health had an impact on the outlook of the programme. Programme planning had to include working during festive season of 2007. Although beginning of 2008 became a vigorous project implementation phase. The project was organized into levels. The first level involved equipping the circuit personnel with necessary tools and skills to enable them to organize successfully the circuit programmes. The target of the Gauteng Department of Health was reached and exceeded. The highlight of the programme was when Good Nutrition For Healthy Living campaign took place. The campaign was implemented from 31 March to 10 April 2008. The objective of the campaign was to use Good Nutrition as an entry point of seriously engaging HIV/AIDS education. Places that were visited through the Good Nutrition Campaign include informal settlements of Siyahlala, Smoke Down, Rietvalei Ext 2 & 3, Swaneville, Bekkersdal, Tshepisong West, Slovoville and Kagiso. Home Based Caregivers were deployed to facilitate the nutrition lessons. Amid short space of time the project had to be implemented they were the relevant resource person available. Caregivers were trained by Mmapula Metzger and Rahab Ramodibe on Good Nutrition and presentation. In February DAM was visited by the Gauteng Department of Health to review Education and Training programmes and financial records. Two members of the Department meticulously reviewed finances, programmes and other documents related to the operations of the Education and Training programme. On 11-13th March 2008 – Nutrition Presentation “Good Nutrition for Healthy Living” presented to DAM Support Groups (Diocesan office and Orlando). Total 68 people attended. 7.7. Other Education related activities Pastors’ refresher course on HIV & AIDS and counseling issues took place from 4-9 Nov 2007. 8. Skills Development and Training 8.1. Carpentry A project was put on hold as the search for a new facilitator continues. Mr. Rakgantsho of Diepkloof was previously identified as a possible replacement for Mr. Mafatlane, however he has not yet responded to the proposal presented to him. 8.2. Dairy Productions 08 – 10th May 2007 the Administrator attended a cheese-making course in Centurion. The administrator organized and facilitated trial cheese course in the Kagiso (St Stephens Church) from 09-11 July 2007 and 5 persons attended the course.

9. Local Partnerships 9.1. Boitumelo Project Working relationship with Boitumelo Project of ELCSA NT in Hillbrow has culminated to a Wednesday training programme held at DAM Diocesan Centre. The members of the support group from Central Western Jabavu and Orlando participate in the programme. There are about 20 students attending the class. The course started with exploring the life history of each individual and created a theme based designs then embroided on cloth. 9.2. DAM and New Creation A notable partnership also was between DAM and New Creation wherein a capacity building workshop was conceptualized and implemented. This workshop targeted people active in the Social Ministries of the Church. The programme helped participants in conceiving and implementing community development programs. The workshops were held each Saturday (at Hillbrow and DAM Diocesan Centre) in the month of February 2008 and the last Sunday of the month for a total of 5 sessions. All four circuits were invited. A total of 41 persons attended the workshops. 9.3. Aids Consortium Capacity Building On 12 – 14 February 2007 – a member of the support group leadership attended an HIV/Aids Workshop, and then shared the information with other members of the support group. 10. Strategic Planning Session Two sessions were held on 03 March and 14 April 2007. The vision, mission and objectives of Diakonia aids Ministry were reviewed and these are the proposals: 10.1. Vision To be a leading faith based organization in people-centered health and development services

10.2. Mission statement To provide a mechanism that addresses socio-economic factors and engage strategic partners. 10.3. Objectives:

o To provide support and counseling for people infected and affected by HIV/Aids and related conditions.

o To develop and provide health educational programmes o To implement youth empowerment programme o Provide affected and infected house holds with care and support

11. Donations and Donor Partners Evangelical Lutheran Church in America (ELCA) have donated R77 000 to Diakonia Aids Ministry. Pauline Mabotja visited DAM office and donated 10 X 25KG of immune booster corn soya (meal) and veggies for support group and home based care. A letter of thanks has been send to her. LUCSA (Lutheran Communion in Southern Africa) donation of R50 000 for Orphan Care and Support programme has not yet reach DAM account. Follow up will continue with the Diocese and Church Centre. Ms. Drea Froechtling accompanied donor partners from Burgdorf circuit during their visit to Western Circuit in October/November 2007. Amid load shedding of Eskom (South African Electricity supplier) funds were secured for the repairs of the Diocesan Centre generator. In addition money for fuel to run the generator has been set aside.

The work of DAM have been introduced to a group a tentative donor partners (Hermannsburg mission) by Ms. Drea Froechtling. In March 2008 staff from PEPFAR (American Presidential funding agency for HIV/AIDS programme) visited DAM to assess funding possibility for Home Based Care (HBC) or Orphans and Vulnerable Children (OVC) program. Financial Support to Administrator position Burgdorf Circuit (Lutheran Church in Germany) is the financier of the Social Worker and Administrators’ position. 12. Circuits Matters

12.1. Soweto 12.1.1. Health committee On 04th February 2007, DAM was invited to the Orlando Parish for installation of Health committee. DAM expects the health committee to implement on-going health education for the congregations, including HIV/Aids. 12.1.2. Orlando Support Group Orlando Support Group of Rev. Tjao’s Parish is growing. Currently there are 20 members (People Living with HIV/Aids). The Group meetings are held on Wednesdays. The Support Group is also producing beads and necklaces (see the catalogue of DAM products). 12.2. Western On 11 February 2007, World Aids Day 2006 evaluation was held in Randfontein, Mohlakeng with the congregation. The aim was assess the impact that the programme had in the congregation and discuss way forward. An interim health committee was established to champion the health programme. A template of a plan was given to them and they are working towards the final draft.

On 08 June 2007 I attended a circuit meeting in Kagiso 2 (St Stephens). Only two people came for the meeting. The resident pastor was also not available nonetheless the meeting carried on. The meeting discussed the sport, arts and culture, cheese course and drama workshops.

13. Other Matters 13.1. Rev. Z. Senosi’s Farewell Function This function took place on Sunday 01 July 2007 at Bethesda Parish (Naledi). It was well attended. More than R25 000 was collected from parishes & congregations and presented to Rev Z. Senosi as a farewell gift. 13.2. Itireleng Housing Project This project involved follow ups with the Department of Housing on behalf of the members of the support group. The Department of Housing informed us that they are focusing on the applicants of 1996 as a priority and they have an enormous backlog. However applicants should continue checking with the local authorities. 13.3. Material Support This programme support select persons with food. The foods that distributed are non perishable. Following consultation with a dietician a list of basic items were drawn. These items are distributed on a monthly basis to needy beneficiaries. The list includes:

About 120 beneficiaries receive the food parcels monthly. The beneficiaries include members of the support group (who are HIV positive), orphan families and select clients of Home Based Caregivers. 14. Visitation and Network On 18 January 2007 DAM hosted visitors from Germany – the delegation was led by Rev E. Kunz. 29th January 2007 DAM had visitors from Burgdorf Circuit, Germany. The visitors included Hans-Dieter Pauli, Dean Klahr and Ricarda Kraume. The discussion with the delegation was on funding of Social Worker position (which will be from 2007-11 renewable annually). Canadian (cooperative) Delegation and Soweto Caregivers visited DAM on 06 February 2007 to exchange experiences on Caregiver programme. 19 February 2007, the Administrator attended a consultative meeting (organized by Treatment Action Campaign) on National Strategic Plan 2007-11 of Department of health for inputs from organizations. 19 – 22 February 2007 – Rev Z.S. Senosi attended monitoring and project assessment workshop at Lutheran Communion of Southern Africa (LUCSA). On 26th February 2007 – Dam officials met with senior managers from Department of Social Development JHB region to discuss possible partnership. The process has begun to action the partnership. On 03 April 2007 young people from Norway visited DAM. Purpose of the visit was to learn more about the work of DAM 19 April 2007 DAM was visited by Dobsonville Prayer Women’s League who brought prayers, words of encouragement and presents to the support groups.

On 13 June 2007 the administrator and social worker visited a Boitshoko Home Based Care programme in Kagiso ext 12 to discuss and exchange Home based care experiences. On 24th September 2007 Hans Dieter Pauli and Ricarda Krause from Burgdorf Circuit visited DAM once more.
