ESE ETP SUPPLEMENTS EXTRA PERIOD SUPPLEMENT (ETPS) 110-F (990):
• A teacher is eligible for an ETPS 110-F if they are ESE certified and are providing direct services to ESE students in an ESE setting for an extra period. – A teacher assigned to consultation/collaboration/
facilitation/co-teaching for all periods is NOT eligible • Classroom teacher-student ratio considerations
• Type of instructional delivery model • Matrix of Services -111, 112, 113, 245, 255 • Standard or modified curriculum courses • The level of support and assistance needed by students • Available in the Budget Allocation Handbook

ESE SUPPLEMENT - 1022
• ESE SUPPLEMENT 1022 (804) FOR ESE TEACHERS: • Per the UTD contract, a teacher must be exclusively
assigned to students with IND, OI/OHI/TBI, EBD and/or ASD. The teacher’s schedule from ISIS will be reviewed Teachers assigned to mixed exceptionality groups
including SLD, VI and/or DHOH are not eligible for an ESE 1022.
• A Supplement Approval Request Form (SARF) must be submitted to Compensation to request the ESE 1022 supplement for an eligible teacher.
