ABOUT THIS DOCUMENTThe intent of the FBMS User Guide is to answer questions pertaining to the use of the Factory and Boiler
Management System. The FBMS User Guide may be used as a tutorial to the Factory and Boiler
Management System and as a reference after one becomes familiar with FBMS.
INTENDED AUDIENCEThis document is intended for people who utilize FBMS. This audience includes:
i. Customers or end users for client organizations.
ii. Technical support personnel for supporting organizations.

GETTING STARTED
Using IE8 web browser, go to http://rajfab.rajasthan.gov.in. The FBMS Web Site provides a short
introductory page, followed by the login page. This login page is the common login interface for all FBMS
users.
User can download user manual for upgrade their knowledge of RAJFAB application system.
Already registered users may enter their username and password in the login window, select the language
and click on ‘Login’.
Already registered users may enter their application ID for searching their application status in
department

USER REGISTRATION
In case of new user, click on ‘Signup’, and the user registration form opens up.
The screen displayed above, is divided into 3 parts:
Factory Details:
If the factory is already registered, then click on the Yes radio button and if not then start filling the details
mentioned.
In case the factory is already registered, then a new text box appears asking to enter the “Registration
Number” of the factory.
i. Full Name of Factory: Mention the “Full Name of the Factory”, for example, if the name of factory is “NBC Bearing”, then mention
the name in this field. Alphanumeric and symbols are allowed in this field.
ii. Address of Factory: Enter the address of the factory which you want to register.
iii. Plot No. : In this field, enter Plot No at which the factory is located, For e.g., if your address is “116A, Civil Lines, Jaipur”, then enter
116A in this field.
iv. Street/Locality: In this field, enter the street name or locality where the factory is located. For e.g., if your address is “116A, Civil
Lines, Jaipur”, then enter Civil Lines in this field.
v. Town/City: In this field enters the name of city or town where the factory is located. For e.g., if your address is “116A, Civil Lines,
Jaipur”, then enter Jaipur in this field.
vi. District: This field has a drop down list. When you click on this field, list of all the districts in Rajasthan comes up in alphabetical
order. User can select the appropriate district. In order to select a district, click on the district which is appearing in the list.
vii. Pin code: In this field, enter the pin code of the address. You will be able to enter 6 digit pin code in this field.

viii. Mobile No.: In this field user needs to fill in his mobile no. You done need to pre-fix the country code or 0 before the number. Enter
the 10 digit mobile no. in this field.
ix. E Mail ID: In this field user needs to fill in his email id.
x. Manufacturing Process: In this field, mention the manufacturing process in which the factory is involved.
Designation of Occupier in the Factory: In this field, there is a drop down list. Select from the drop down list a. Proprietorb. Partnerc. Directord. Managing agent appointed by Central/State Govt. or by local bodiese. Factory Manager
Personal Details:
i. First Name: First Name is the name of the person who wants to register in the system. For e.g. in case of Ramprasad Sharma, first
name will be “Ramprasad”. This is a mandatory field. User will not be able to submit the data if he does not fill this field.
ii. Last Name: Last Name is the surname of the person who wants to register in the system. For e.g. in case of Ramprasad Sharma, last
name will be “Sharma”. This is a mandatory field. User will not be able to submit the data if he does not fill this field.
iii. Email ID: Email ID is the email id of the person who wants to register in the system. This is a mandatory field. User will not be able
to submit the data if he does not fill this field.
All the communication related to FBMS will be sent to this email id. So, it is important that the email id
entered should be correct.
Date of Birth: Date of Birth is the field where the user needs to select his birthday. The month shall have a
drop down. Click on the arrow beside the “Month”. List of all the months opens up. Select the appropriate
month of your birthday.
The year shall also have a drop down. Click on the arrow beside the “Year”. List of all the years opens up.
User can select the year of this birthday.
In the main calendar is the date. Once the month and year is selected, click on the date of your birthday.
The birthday selected by you get filled in the field.
iv. Mobile No.: In this field user needs to fill in his mobile no. You done need to pre-fix the country code or 0 before the number. Enter
the 10 digit mobile no. in this field.
v. Phone No.: Phone No. is the field where user fills his/her residential contact number. There are two boxes for this field. Fill the
STD code in the first box and the landline no. in the next box.
vi. Gender: Select the gender of the Factory Owner
vii. UID No.: In this field user needs to fill his/her UID No. User will be allowed to fill the 12 digit UID No.
Permanent Address
i. House No. /Plot No. /Name: In this field, enter the first line of the residential address, For e.g., if your address is “116A, Civil
Lines, Jaipur”, then enter 116A in this field.
ii. Street/Locality/Mohalla: In this field, enter the second line of the address. For e.g., if your address is “116A, Civil Lines, Jaipur”,
then enter Civil Lines in this field.

iii. Village/Town/City: In this field enters the last line of the address. For e.g., if your address is “116A, Civil Lines, Jaipur”, then enter
Jaipur in this field.
iv. District: This field has a drop down list. When you click on this field, list of all the districts in Rajasthan comes up in alphabetical
order. User can select the appropriate district. In order to select a district, click on the district which is appearing in the list.
i. Pin code: In this field, enter the pin code of the address. You will be able to enter 6 digit pin code in this field.
Login Details
i. User Name: Username means a name that will uniquely identify the user on the Factory Boiler Management System. Username
can be chosen by the user himself. Username should be such, so that the user can remember it. For e.g., Ramprasad Sharma can
select a username like Ramprasads.
After filling in all the details, enter the captch code, as depicted in the box. Click on “SUBMIT” after
all the fields are filled
After entering all the basic details, and clicking on Submit, a onetime password gets generated and gets
displayed on the screen.
Note down the password displayed in the screen.

NOTE: The password is also messaged on mobile number mentioned by the registering user. Click on OK
and user gets directed back to the login page.
CHANGE ONE TIME PASSWORD
Enter the username and the one time password, select the preferred language and click on ‘Login’.
User gets directed to the change password screen.

Password is a secret word or phrase that must be used to gain admission to the system. Passwords are
used also for authentication, validation and verification in electronic commerce. Password can be
combination of alphabets, numbers and symbols.
Passwords should be such which is not easy to guess for general public however should be unforgettable
for the user creating the password.
In the Confirm Password field, enter the same password which you have entered in the Password field.
Hint Question: Hint Question andAnswer helps the user to remember the password that he has set. Hint
question field has a dropdown list. User can select any of the questions from the list.
Hint Answer: User can fill in the Hint Answer as a answer to the Hint Question. The hint answer shall be of
user’s choice.
After entering all the details and on clicking on Submit, user will get re-directed to the main login page.
User can now enter the username and the new password, select the preferred language as “English” and
click on LOGIN.
User can see the next screen as displayed in the screen below:
USER HOME PAGEThis is main login screen, once the user logs in the system.
On the left hand side, is the list of activities which the user can perform.

i. Add New: When user wants to fill Form I, he needs to click on this link.
ii. View MAPS List: When user wants to view the filled Form I, he needs to click on this link.
2. Registration of Factory:
i. Form 2: When user wants to apply for the “registration of factory:, he has to click on this link.
ii. View Form 2 List: When user wants to know the status or view the “Form II”, he has to click on this link.
iii. Pay: When user wants to make payment for the filled Form II, he has to click on this link.
3. Renewal of Factory:
i. Form 2 for Renewal List: When user wants to apply for renewal of factory, he has to click on this link.
ii. View Renewal of Form 2 List: When user wants to view the filled form or know the status of the filled form for renewal
of factory, he has to click on this link.
iii. Pay: When user wants to make payment for “renewal of factory”, he has to click on this link.
4. Amendment:
i. License:
a) Form 2: When user wants to make amendment in the approved Form 2, he has to click on this link.
b) View Amendment of Form 2: When user wants to view the filled form or know the status of the filled form for
amendment of factory, he has to click on this link.
c) Pay: When user wants to make payment for “amendment of form 2”, he has to click on this link.
ii. Map:
a) Maps for Amendment: When user wants to make amendment in Form 1, he has to click on this link.
b) View Amendment MAPS List: When user wants to view the form of amendment, he has to click on this link.
5. Report Accident:
a) Notice of Accident resulting in death / bodily injury (Form 20): When user wants to report an accident in Form 20,
he has to click on this link.
b) Notification of a Major Accident (Schedule 6): When user wants to report a major accident in Schedule 6, he has to
click on this link.
c) Notice of Dangerous Accident but not resulting to death/bodily injury (Form 20A): When user wants to report an
accident in Form 20 A, he has to click on this link.
d) Boiler Accident Report (Form D): When user wants to report an accident in Form D, he has to click on this link.
6. View:
e) Notice of Accident resulting in death / bodily injury (Form 20): When user wants to view the report of the accident in
Form 20, he has to click on this link.
f) Notification of a Major Accident (Schedule 6): When user wants to view the report of a major accident in Schedule 6,
he has to click on this link.
g) Notice of Dangerous Accident but not resulting to death/bodily injury (Form 20A): When user wants to view the
report of an accident in Form 20 A, he has to click on this link.

1. Details of occupier:- i. Name:- Mention the name of the occupier in this field. E.g. Megha Verma.Only alphabets are allowed in this field. In case of already
registered factory, this field will come auto-filled.ii. Designation: Mention the designation of the owner. In case of already registered factory, this field will come auto-filled.
iii. Plot No./ Name: Mention the Plot No. or Name of the Factory. In case of already registered factory, this field will come auto-filled.iv. Street/Locality:- Mention the Street or Locality name in this field. In case of already registered factory, this field will come auto-
filled.v. City/ Town:- Mention the City or Town name in this field. In case of already registered factory, this field will come auto-filled.
vi. District:-Name of the district of the factory office. In case of alreasy registered factory, this field will come auto-filled.vii. Area:-This field will include the area/place of the factory office. In case of already registered factory, this field will come auto-
filled.viii. Pin code: Mention the pin code of the address where the factory is located. In case of already registered factory, this field will
come auto-filled.ix. Mobile:-Mobile no. of the occupier. Must contain 10digits with no 0 prefix.(This field is mandatory).x. Email ID: Mention the Email ID of the factory owner in this field.
Residential Address:-
i. Plot No. /Name: In this field, enter the first line of the residential address, For e.g., if your address is “116A, Civil Lines, Jaipur”, then enter 116A in this field.
ii. Street/Locality: In this field, enter the second line of the address. For e.g., if your address is “116A, Civil Lines, Jaipur”, then enter Civil Lines in this field.
iii. Town/City: In this field enters the last line of the address. For e.g., if your address is “116A, Civil Lines, Jaipur”, then enter Jaipur in this field.

v. Area: Select the area in which the residence of the factory owner is located.vi. Pin code: In this field, enter the pin code of the address. You will be able to enter 6 digit pin code in this field.
2. Details of factory:- i. Full Name of Factory:-Mention the Full Name of the Factory in this field. . In case of already registered factory, this field will come
auto-filled.ii. Plot No./ Name: Mention the Plot No. or Name of the Factory. In case of already registered factory, this field will come auto-filled.
iii. Street/Locality:- Mention the Street or Locality name in this field. In case of already registered factory, this field will come auto-filled.
iv. City/ Town: - Mention the City or Town name in this field. In case of already registered factory, this field will come auto-filled.v. District:-Name of the district of the factory office. In case of already registered factory, this field will come auto-filled.
vi. Area:-This field will include the area/place of the factory office. In case of already registered factory, this field will come auto-filled.
vii. Pin code: Mention the pin code of the address where the factory is located. In case of already registered factory, this field will come auto-filled.
viii. Nearest Railway Station:-Select the name of the railway station from the drop down list availableix. Nearest Police Station:- Select the name of the police station from the drop down list available
3. Particulars of Plant to be installed and manufacturing process:- This will contain the detail about the particulars of the plant to be installed at the factory and also about the manufacturing process.
4. Maximum Number of Workers to be employed:
i. Total No. of Shifts: Mention the total no of shifts operating/ to be operated in the factoryii. Total Male Employees Employed: Mention the total male employees employed / to be employed in the factory
iii. Total Female Employees Employed: Mention the total female employees employed / to be employed in the factoryiv. Total Employees: This field shall be auto-calculated based on the number mentioned in the above two fields.

5. Process: There are 5 processes from which the user can select. Click on the appropriate radio button based on the process followed / to be followed by the factory. NOTE: The background colour changes as per the selection made.
In the grid shown below, attach the required documents as per the selection made in the process. The documents which are to be attached shall be highlighted by a background colour. It is mandatory to attach all the documents,.
Once all the documents are attached, re-check the form and click on submit.

On clicking on Submit, an acknowledgment appears along with the Application ID. This ID shall be used for
future reference.

TO VIEW AN APPLICATION FORM
You can view the filled application form also.
Click on “View Maps List” top view a filled application.

On the right hand side panel, you can view all the application forms filled by you in a grid format.
The grid has certain columns like:
1. Application ID
2. Occupier Name
3. Factory Name
4. Date of Registration
5. Status
To view any application, click on the Application Number, which is a hyperlink.
On clicking on the application number, the filled form opens up and the user can view the form.

WORKFLOW OF MAPS APPROVAL FORM
Once a form is filled by the user (citizen), it gets directed to the LDC/UDC of that particular district who has been authorized to take action on the form.
LDC will mark the authority to the particular application which is belongs to the particular district for e.g. if
the applicant belongs to Jaipur district and he/she applied from Jaipur district then the only Jaipur district
authority/ LDC will take further action on that application. Therefore LDC/ UDC will login by their
credentials. Welcome window will get opened where all the registered cases will appear as given below:-

List of applications will appear by some specific columns as:-
1. Application ID: - which represents the unique number of application which is generated by the system at the time of registration.
This column will be a hyperlink.
2. Occupier Name: - Represents the owner name which is filled at the time of application registration.
3. Factory Name: - It represents the name of factory which is filled at the time of registration.
4. Date of Process: - This date is application date when the application registered.
5. Application status: - Action status denotes that action taken by the User/ Authority/ LDC-UDC on that particular application.
When the LDC will click on the application number, the filled form will open up.

LDC / UDC can check the form and put in his comments if he feels that there is any clarification required on
the attachments attached by the citizen.
For putting on his comments, he needs to click on the check box beside each attachment. On clicking on the
check box, a text box appears where he needs to put in his comments. Once he has completed the review of
the form, he needs to mark the form to the Head Draftsman.
Mark To column contains the list of all the authorities names belongs to the same district by which the
application belongs. Mark To column is placed to assign the authority for a related application for which
authority will take further action. Select authority name from the given drop down list which is placed at in
front of “Mark To” column. Click Submit Button.

The form gets forwarded and an acknowledgement appears.
Now the form is pending with the “Inspector” to whom the form is forwarded.

Inspector can review the form and take an appropriate action on it. At this level, the authority can take
many actions on the form:
1. Ask for clarifications on the attached documents
2. Change the Process Type
3. Ask for clarifications in the Materials and Products used for manufacturing
4. Any extra clarification

5. Accept the form
6. Mark the form back to LDC, Head draftsman, Inspector, Dy. Chief, Legal Department

NOTE:
1. IF THE CHIEF ASKS FOR CLARIFICATION, THEN THE FORM IS DIRECTLY FORWARDED TO THE CITIZEN. CITIZEN NEEDS TO MAKE COMPLIANCE AND THE WORKFLOW STARTS AGAIN AS EXPLAINED ABOVE.
2. IF THE FORM IS MARKED TO ANY DEPARTMENT USER, THEN THE FORM GETS PENDING AT THAT USER LEVEL.
2. APPLICATION FOR REGISTRATION OF FACTORY – FORM 2
To apply for the “Registration of Factory”, a citizen needs to apply by filling Form 2.
In order to fill Form 2, he needs to click on the text shown in red in the image above.
On the right hand side, the link to fill the Form 2 appears. Click on the Application ID.
On clicking on the Application ID, the Form 2 appears as shown below.

Application for registration/renewal/transfer of license and notice occupation
1. Period of license :- Years for which license is applied for:-This will include the no. of years for which the license is applied (From-To). In the From column, select the date from the calendar and in the To column, select the Year till which you want to apply.
2. General information:- a. Full Name of the factory:- This field shall be pre-filled.b. Factory Registration Number : This field shall be pre-filled.
3. Address and contact information:- The address field shall be pre-filled.
4. Nature of manufacturing processes:-
a. Date of start of production:-On clicking on this field, a calendar appears. Select the date from the calendar.b. In the next field, select the appropriate radio button, by clicking on the radio button.c. Details of products manufactured during the last twelve months:-This will include the complete detail about the project
carried out during last 12months.
5. Workers Employed:-a. Maximum no. of workers proposed to be employed during the year:- This field is divided into 3 parts, Male, Female and Total.
Enter the maximum number of workers in the Male and Female field and the Total field shall be filled automatically by the system.b. Maximum no. of workers employed during the last twelve months on any day:- This field is divided into 3 parts, Male, Female
and Total. Enter the maximum number of workers in the Male and Female field and the Total field shall be filled automatically by the system.
c. Number of workers ordinarily employed in the factory:- This field is divided into 3 parts, Male, Female and Total. Enter the maximum number of workers in the Male and Female field and the Total field shall be filled automatically by the system.
6. Power installed:-a. Total rate horse power:-This will include the power installed or to be installed (in HP)b. Max. Amount of power proposed to be used:-It will include the max. Amount of power being proposed for the factory.
7. Particulars of factory manager:-a. Name:-This field will include the name of the factory manager E.g. Megha Verma.Only alphabets are allowed in this field.(This field
is mandatory).b. Plot No./Name: Mention the Plot No in which the Factory Manager staysc. Street/Locality:-In this field user need to enter the locality/street of the manager’s residence.d. City:-Name of the city or town of the factory manager.e. District:-Name of the district of the factory manager.f. Area:-This field will include the area/place of the manager’s residence.g. Mobile:-Mobile no. of the manager. Must contain 10digits with no 0 prefix.(This field is mandatory).
8. Particulars of occupier:-a. Occupier Type: Select the Occupier Type by clicking on the drop down list. Click on the appropriate item in the list.b. Name:-This field will include the name of the Occupier E.g. Megha Verma.Only alphabets are allowed in this field.(This field is
mandatory).c. Area:-This field will include the area/place of the Director’s residence.d. Street/Locality:-In this field user need to enter the locality/street of the Director’s residence.e. City:-Name of the city or town of the Director.f. District:-Name of the district of the Director.g. Mobile:-Mobile no. of the director. Must contain 10digits with no 0 prefix.(This field is mandatory).

Land and building:-
9. Particulars of the land owner:- a. Name:-This field will include the name of the land owner E.g. Megha Verma.Only alphabets are allowed in this field. (This
field is mandatory).b. Area:-This field will include the area/place of the land owner’s residence.c. Street/Locality:-In this field user need to enter the locality/street of the land owner’s residence.d. City:-Name of the city or town of the land owner.e. District:-Name of the district of the land owner.f. Mobile:-Mobile no. of the land owner. Must contain 10digits with no 0 prefix.(This field is mandatory).
10. Reference number and date of approval of the plan for the site:-This will include the reference no. and date of approval of the plan for the site.

On the left hand side, is the list of activities which the user can perform.
7. Approval (MAPS):

k) Notice of Dangerous Accident but not resulting to death/bodily injury (Form 20A): When user wants to report an
accident in Form 20 A, he has to click on this link.
l) Boiler Accident Report (Form D): When user wants to report an accident in Form D, he has to click on this link.

FORM 20 A – TO REPORT AN ACCIDENT
To fill Form 20 A, click on the link and the details to fill their form appears on the right hand side, as shown below.
Go through the guidelines to fill the form and then click on “Go to Form” button. On clicking on this button, the blank form opens up, as shown below.

1. District: Select the name of the district where the accident took place. The list of all the districts is in the drop down list.
2. Area: Select the Area where the accident took place. The name of area gets populated on the basis of the district selected in the prior field.
3. Name of Factory: Mention the name of the factory where the accident took place. This is a text box where alphabets, numbers and symbols are allowed. Maximum characters allowed in this box shall be 30. Mandatory field
4. Address of Factory: Mention the address of the factory. This is a text box where alphabets, numbers and symbols are allowed. Maximum characters allowed in this box shall be 50. Mandatory field

5. Name of Manager: Mention the name of the factory’s manager. This is a text box where only alphabet is allowed. Maximum characters allowed in this box shall be 20. Mandatory field
6. Nature of Industry: Select the nature of industry from the drop down list. Mandatory field.7. Branch or department where the occurrence took place: Mention the branch or department where the
accident took place. This is a text box where only alphabet is allowed. Maximum characters allowed in this box shall be 20. Mandatory field
8. Exact Place where the occurrence took place: Mention the exact place where the accident took place. This is a text box where only alphabet is allowed. Maximum characters allowed in this box shall be 20. Mandatory field
9. Date of Occurrence of Accident: Select the date of occurrence of accident from the calendar control. Mandatory field
10. Time of Occurrence of Accident: Enter the time of occurrence of accident in HH:MM format. Mandatory field
11. Nature of Dangerous Occurrence: Mention the nature of occurrence. Text box up to 50 characters.12. Scanned Signature of Occupier/Manager: Attach the scanned signature of occupier/manager. PDF and
image file allowed. Size up to 2 MB.
Click on the check-box of the declaration and click on “Submit” button.
Click on SUBMIT and a new acknowledgement appears.

On clicking on Submit, an acknowledgment appears along with the Application ID. This ID shall be used for
future reference.

On the right hand side panel, you can view all the application forms filled by you in a grid format.
The grid has certain columns like:

To view any application, click on the Case Number, which is a hyperlink.

WORKFLOW OF ACCIDENT REPORT FORM
Once a form is filled by the user (citizen), it gets directed to the Area Inspector of that particular district who has been authorized to take action on the form.
Area Inspector will review the particular application and take appropriate action on the form. To view a
form, he needs to click on the link shown below.
In the Accident Report Menu Item, there are many links (explained below):
1. Case Pending at my level: Cases pending at the level of the department user who has logged in the application
2. Case Pending at other departmental officer level: Cases which are pending at the level of some other department user
other than the one who has logged in the application
3. Case Pending for State Government Decision: Cases which are pending for the decision of the state government
4. Case Pending for Court Decision: Cases which are pending for court decision
5. Closed Cases: Cases on which decision has been taken

Click on the Case No. to view the form and take action on it.
In the grid marked in red above, the area inspector can fill in the inspection report and forward the form to
the relevant authority for taking further action.

10. Mark the application to the next level for further investigation

Once a form is forwarded by the area inspector, it gets directed to the officer of that particular district who has been authorized to take action on the form.
In this case the form has been forwarded to the Senior Inspector who will review the particular application
and take appropriate action on the form. To view a form, he needs to click on the link shown below.

He can attach any document if he wants to and enter his comments.
Then select the name of the department user from the drop down box and click on “Forward”

appropriate action on the form

WORKFLOW OF INSPECTION LIST GENERATION
1. On selecting “New Inspection”, List of Factories are generated for inspection by inspector,

The generated list is based on type of factories i.e. Non-Hazardous, Hazardous, Dangerous Operation or
Major Accident Hazard (MAH) and number of workers employed.
Documents are not required for Factory/Boiler renewal if there is no change in process.
