This is a rare opportunity to join one of the only truly democratic, private groups in the state of Delaware. Located in a beautiful historic city, this group has been in place over 10 years and enjoys superb support from both the hospital administration and the medical staff. The ED is a level 3 trauma center with a census topping 50K. The hospital is a 2011 Distinguished Hospital Award winner for Clinical Excellence.

The hospital recently completed a multi-million dollar building project including a gorgeous new, state of the art ED including a helipad. In addition a satellite, free-standing ED is staffed by the group during the peak summer months. With strong ties to the Univ. of DE, the only cases transferred there are serious Neuro, and PICU cases get stabilized and sent to DuPont Kids. The department is totally computerized with the IBEX system.

Physicians start as employees of the group and become equity owner/partners after one year or after board certifi cation is achieved, whichever comes fi rst. Shift equity starts from day one and all physicians have a vote in the management of the department. The ED group enjoys exceptionally strong patient satisfaction scores and low liability history.

Docs work 10-hour shifts for 130 to 140 hours per month, seeing well under 2.5 patients per hour. During the summer the census increases a bit in the main, but the primary increase occurs in the satellite site with the usual summer, recreational injuries and cases.

There is no buy-in for partnership/equal equity ownership, and the fi rst year is considered a “sweat equity” trial period. Incoming physicians will earn a package of over $150 an hour, including paid leave, comprehensive health and dental benefi ts, full malpractice plus tail, CME and professional expenses and a great pension and retirement contribution. Once partnership is earned, the income increases considerably. Last year, the average partner enjoyed an additional $140K in variable compensation, some topping $400K.

If you are a BCEM, residency trained physician or a 2013 graduating resident and are seeking guaranteed equity partnership with a great group in a fi nancially sound facility, this is the opportunity you want to investigate. With a level 2 trauma center seeing 60K annual volume, and a neighboring level 2 seeing under 15K, their outstanding performance has been achieved with superb coverage and an FP physician handling triage. The hospital has some of the highest JCAHO scores (98 with no type 1s) in the nation and the ED has some of the highest Press Ganey patient satisfaction scores.

Compensation includes a base salary of $170.00 per hour. Quarterly incentive bonuses add an average of $15.00 per hour. Benefi ts include full family health, dental and vision, disability and malpractice insurance including tail coverage, plus a $5K professional stipend for CME and dues. There are even 80 hours of paid leave! Every physician is guaranteed equity partnership, by contract, at the end of two years spent as an employee of the group, but those who adapt quickly can earn partnership more quickly. Equal equity ownership includes $30K annual retirement contribution by the group. This package totals well over $350K pre-partnership. Partner income averages over $450K.

Look up: pigs are flying. Look down: hell is freezing over.That’s right, Charleston, San Diego, Omaha, and evenPortland and Utah have jobs. This is the location report I

never thought I would write, but here it is. Jobs are everywhere!Don’t run off to Salt Lake City. These difficult-to-penetrateareas have one, perhaps two, opportunities, and if you want ashot at them you better have some pretty impressive credentialsand a relative who works there. The market is so wide openright now anything is possible, but those prime lifestyle areasremain tough to crack, just not quite as tough. The other thingyou need to keep an eye on is quality. A big jump in quantity isusually accompanied by a big dive in quality. Ask a lot ofquestions, and read between the lines.I contacted the larger groups for an idea of where they neededphysicians for the coming season. I also asked them how longthey predict the job market for EPs will continue to be floodedwith openings, and what they are doing to keep up with thedemand. The group leaders all agreed that thecurrent candidate-driven market was going tocontinue into the foreseeable future, largelybecause the emergency physician shortagepersists, particularly in rural areas.

The MidwestThe greatest need for emergency physicians

will be in Ohio and the 12 states of theMidwest. Look for strong opportunities inCleveland, Cincinnati, and Columbus and inall other parts of the state. Illinois is rife with opportunitythroughout the state with Chicago topping the chart. Highnumbers of jobs can also be found in Indiana, Missouri, andWisconsin and multiple opportunities in Kansas City (bothsides) and St. Louis. Iowa, Michigan, Kansas, and Minnesotawill have fewer jobs, but Nebraska and the Dakotas have thefewest listings, though they do exist. Thirty-two percent ofpositions throughout the Midwest are open to physicians boardcertified in primary care. Fifty-eight percent of Missouriemployers, but only eight percent of those in Wisconsin, willaccept physicians not board certified in emergency medicine.Detroit will be tight as will Indianapolis, Minneapolis, and DesMoines, but higher potential exists in Ft. Wayne, Topeka, andOmaha.

The SoutheastThe Southeast leads the country in opportunities for

physicians board certified in primary care at 37 percent, withstaggeringly high numbers in states like Arkansas (88%),Alabama (63%), and Mississippi (52%). The 10 states of theSoutheast represent 27 percent of the nation’s job opportunities

with Tennessee leading the way, followed by Florida and NorthCarolina. The major cities in Florida, from Miami to Tampa toJacksonville, will offer many opportunities. Look for NorthCarolina jobs in the western Smoky Mountain area, Charlotte,and the Atlantic coastal regions, but forget Research Triangle; itis the only area of the country that remains closed. Nashvilleand Knoxville opportunities are strong as are the mountainareas in the east. Georgia has jobs this year, as does Atlanta —big time! Kentucky has jobs all over with a few in Louisville andLexington. Mississippi and South Carolina don’t offer much inthe major cities (one job in Charleston probably won’t lastlong), and the same goes for Alabama and Arkansas.

WesternThe 10 Western states will provide about 16 percent of the

country’s opportunities with 20 percent open to primary carephysicians. California and Texas lead the way, including Los

Angeles, San Francisco, Houston,Dallas/Ft. Worth, and even San Diego. Bignumbers will be found in the San Joseregion of California with groups of allsizes. San Antonio and Austin in Texas alsohave high numbers of jobs. Arizona,Colorado, Nevada, and New Mexico willhave sporadic opportunities with little tobe found in major cities like Phoenix andDenver. Only a few spots are available inHawaii and Utah, but Oklahoma is on themap this year, including the major cities.

Middle AtlanticProviding another 16 percent of the country’s jobs are the

seven Middle Atlantic because of jobs in Pennsylvania, Virginia,New Jersey, and Maryland. Look for strong opportunity inPhiladelphia, Pittsburgh, Richmond, and the entireWashington, DC, metro area, including the Virginia andMaryland suburbs. Only 15 percent of the jobs in this regionwill be open to primary care physicians with emergencymedicine experience. A moderate number of jobs can be foundin West Virginia; surprisingly only five percent are open toprimary care physicians. This may be because of the highnumber of opportunities in Charleston. Delaware has a fewspots, and New Jersey is busy along the coast, near New YorkCity, and near Philadelphia. Baltimore leads the way for city jobavailability this year, with a wide variety of employer models.

The Northeastern Less than 10 percent of the seven Northeastern states are

open to primary care physicians, with no employers inVermont, Rhode Island, Massachusetts, and New Hampshireaccepting primary care. Forty-one percent of the jobs in Maine,however, will accept primary care physicians. New York Cityhas opportunities, as does the Syracuse region. Boston andProvidence are ripe with openings, but only nominal spots existin Vermont and New Hampshire. Look in westernMassachusetts and all over Connecticut for strongopportunities.

NorthwestThe Northwest as usual provides less than five percent of

the nation’s job opportunities, but 31 percent will be open toprimary care physicians with emergency medicine experience.Washington and Oregon will have spots in suburban, rural, andurban areas, including Seattle and Portland. My favorite joblisting, in fact, comes from Seattle: “Do you love medicine asmuch as we do? Is it in your blood? Does it exude from yourpores? Do you care about patient satisfaction and clinicalexcellence? Are you excellent?”

The job market for the coming year looks intriguing!Speaking of intrigue, you may need to hire a spy to find a jobin Alaska, but there are a few and even a bunch in themountain areas of Wyoming, Montana, and Idaho.

Stick a pin in the U.S. map, and chances are you’ll find ajob there, unless, of course, it’s where pigs really don’t fly thisyear: Raleigh-Durham, NC.

The July 29 New York Times front-page headlineproclaimed, “Doctor Shortage Likely to Worsen WithHealth Law,” followed by a subhead stating that

“primary care is scarce.” My first reaction was amazement;they’re just figuring that out now? I’ve been predicting theseissues for more than a year, and hospitals preparing for thechange have been doing so far longer than that. Still, it waspretty astounding to see the nation’s leading newspaper finallyspell it out in black and white.

I asked a few leaders of the larger groups how much longerthey thought the job market for EPs would continue to beoverwhelmed with openings and what they were doing to keepup with the demand. One regional medical director said thequestion was complex because of changing regulations, courtrulings, presumed increases in ED volumes, limited residencyprograms, and increased use of physician extenders, but that hethought the EP shortage would continue well into the future.He said incentives help keep his company’s retention rate high.They are also focusing on using scribes in some locations andexpanding the floater physician team for additional supportwhere needed.

One would assume that this skyrocketing need would makesalaries rise, but that has not been the case. More groups areoffering higher front-money perks, but salaries are staying onpar with last season and are even going down a bit, particularlyin parts of the Northeast and Southwest. The availability andsize of sign-on bonuses is location-specific and outside popularlifestyle areas. Loan forgiveness is a large group or employeeperk, primarily in location-challenged areas, but not somethingto be found in smaller, democratic groups.

NortheastAverage salaries in the Northeast are under $300,000 in all

states but Connecticut where the average sits right at thatnumber. Massachusetts comes in at about $280,000, NewHampshire at $275,000, and as usual, New York is offering anaverage annual income of $260,000, an abysmal rate whencompared with the cost of living. Look for peaks as high as$400,000 in Massachusetts but only in one or two partnershipopportunities. Sign-on bonuses are low, averaging $5,000 to$15,000, although New York has one or two location-challenged venues offering as much as $50,000 plus some loanpayback. Loan payback in other northeastern states is sporadic.

Middle Atlantic The Middle Atlantic states have some hot spots, particularly

in Virginia and West Virginia. High levels in Virginiapartnerships can reach $450,000, and averages around the statecome in at $365,000 with sign-on bonuses topping out at

$30,000. Pennsylvania has the best chance for high front-money, and the income averages are in the $315,000 range,with significantly higher incomes west of Philadelphia.Compensation in the Pittsburgh region runs as much as 35percent higher. Maryland has some strong potential averaging$305,000 with big perks in the western part of the state. Lookfor $350,000 to $400,000 in West Virginia. New Jerseyprovides a wide range with a high of $390,000 and a low of$243,000. Maryland and West Virginia provide the best chancefor the front-money perks outside Pennsylvania.

SoutheastMississippi tops the Southeast again with an average of

$400,000 around the state, including a partnership or twoalong the Gulf Coast; one even claims $350 an hour as apartner. Florida and Kentucky have group opportunities thatpromise $500,000 potential as partners, but the statewidenorms are closer to $300,000-$325,000. Alabama, Georgia, andNorth and South Carolina are all in the same range, withpockets of $400,000 potential in Georgia and North Carolina.Louisiana is seeing a strong hike in income, coming in withaverages in the $360,000 level, just nosing out Tennessee’saverages of $350,000. Residents who sign early with one largegroup there can receive a stipend as high as $36,000. Second-year residents earn $1,000 per month, and that goes up to$2,000 per month in the third year. Florida has some loanpayback programs as do Kentucky and South Carolina. Sign-onbonus averages are highest in Florida, Mississippi, andTennessee — as high as $40,000 — but averages in the regionare closer to $25,000.

West Incomes in the West run the gamut with lows in Colorado

averaging $250,000 and highs in California more than$400,000 because of heavy use of RVU compensation modelsby the bigger groups. CEP America reported that guaranteedhourly rates, annual profit-sharing, and ownership ofpartnership assets set their incomes about 10 percent higherthan others in the state. They also have loan programs, new hirebridge loans, and even maternity/paternity loans for qualifiedphysicians. The other big payday comes from Texas withaverages of $350,000 and high levels in the $450,000 range.You can also find strong perks in the state. Nevada and Arizonaare averaging under $300,000 with very little up front. Hawaiiisn’t worth discussing, and Utah isn’t talking. Bright spots existin Oklahoma with an average income of $350,000, and NewMexico ranks just under that at $325,000. Sign-on levels are allover the place but highest in Texas.

MidwestThe Midwest is the land of front-money with strong loan

payback, sign-on bonuses, and even stipends in Iowa, Illinois,Ohio, and Wisconsin. Wisconsin has the highest incomeaverage at $395,000, just beating out Iowa at $390,000 andIllinois and Nebraska at $375,000. Missouri and Kansas areaveraging $345,000-$350,000 with low sign-on bonuses in the$15,000 range. Indiana and the Dakotas are averaging$335,000-$340,000, with Michigan and Ohio sitting at thelowest point in the region with $290,000 and $300,000averages respectively. With such high levels of need in Ohio,lower incomes can be a problem. The best incomes appear torange from $350,000-$400,000 in the northwest portion of thestate. High spots of $500,000 are being reported in Wisconsin,Iowa, and Illinois, but some of those reported actually includethe addition of one-time perks.

The Pacific NorthwestAs usual, bringing up the rear is the Pacific Northwest. The

bright spot may be the averages of $320,000 in Washington and$325,000 in Oregon, plus strong perk potential. Montana andIdaho are well under the $300,000 average with no front-money being reported, and I found only one income report inWyoming that came in at $160-$175 an hour. Then there is thelisting for a job in Anchorage, Alaska, claiming to pay $289 to$363 per hour!

These are averages, and shouldn’t be treated as set-in-stonerates to be demanded on an interview. It’s a matter of supplyand demand. Where the demand is higher and the supplylower, the money is higher in most cases. Location-challengedareas are usually the most lucrative. If you are lucky enough toget an interview in Utah or Colorado, don’t lead with “what’syour sign-on bonus?” They are likely to reply, “How much doyou want to pay us?”

WASHINGTON-Richland 57,000 volume Kadlec Medical Center has a 28 bed ED, 6 bed fast track and new 8 bed intermediate section with 62 hours of physician and 38 hours of MLP coverage daily. The admission rate of patients the physician sees is 20% with major burns, critical pediatrics and selected neurosurgical cases transferred out. The EPIC system is used. Kadlec being the main referral hospital in the SE region they receive a signifi cant portion of area trauma. We are seeking an emergency medicine trained/board eligible/certifi ed physician to help staff this ED effective approximately June 2013.
