E-Waste refers to “Electronic” waste. State and Federal Agencies have determined that certain working and non-working electronic products must be classified as hazardous waste if intended for disposal. Kraft Information Systems has authorized the following list of E Waste qualified items:

Section 2. What Is NOT E-Waste?

• HP: leased equipment (following lifecycle refresh cycle; for questions contact local HP field support):

• All HP Laptops• Desktops• Servers• Networking equipment (Cisco Routers and switches)

• Ricoh: leased printers, copiers, fax machines, MFD (multi-functional devices) and plotters, used toner cartridges

• Phones and phone systems

• Cell phones: All corporate and personal cell phones may recycled through the Sprint Program in the U.S, regardless of the type of cell service. Contact your local mailroom staff for further detail and mailing envelopes.

Which Service is Right for Me?

Option 1: Mailback Program Order boxes through Cleanlites online system. Kraft Offices fill box(es) and prepare for shipment. Fill the box at your leisure and schedule a pick-up anytime.

Option 2: Bulk Pickup Program

Use this option for large item clean-outs that may be done annually / bi-annually. Most likely unnecessary for majority of Kraft office sites.

Simple.We ask that you not use local recyclers even if it may be more convenient or free. Kraft needs 100% traceability so the e-waste does not end up at questionable end-sources where our corporate information ends up in the wrong hands.

Mailback Program: How It Works

Step 1•Order boxes through the Kraft online portal

Step 2•Fill the boxes at your leisure

Step 3•Use the prepaid shipping label (included) to schedule your FedEx Pickup, no need to contact Cleanlites.

Mailback Program & Boxes

• After filling out the order form, Cleanlites staff will process the order and call Kraft Offices to confirm details

• Upon ordering, Cleanlites reps will make sure to send enough boxes to accommodate all your waste

• Boxes will be delivered via FedEx mail and can be sent back at your convenience.

Mailback Program: Special Info to Know

• Pre-paid return shipping label(s) and instructions will be included with the box(es)

• Once box(es) is/are received, this label MUST be placed on the box immediately upon receipt prior to filling it, the pickup will be void if there is no label on the box at time of pickup.

• Fill the box(es) at your leisure. Once the box(es) is/are full or any time you need to get rid of the E-Waste that has been accumulated, follow the instructions sent with the box(es) and schedule a pickup (refer to Page 19) .

• Boxes have a poly liner and will protect from e-waste shifted during shipment and puncturing through the box. Bubble-wrap or Styrofoam is not needed to protect the shipment since e-waste will be dismantled and destroyed upon receipt at Cleanlites. Small boxes have a lid and boxes need to be taped shut with either duct tape, regular packing tape or brown UPS tape, or other tape which the Kraft Office can purchase in a local office supply store if needed.

• For the Mailback Program Option #1, FedEx has already been pre-paid through Cleanlites; and, must be used for this delivery service. A FedEx driver will pick up the box at no charge and it will be mailed to Cleanlites.

What do the Instructions Look Like?

a second order form to send package, it is a pre-paid return package

• The package cannot exceed 69lbs, please consult Cleanlites if you are concerned about this weight requirement

• Package must have return label (which is included) attached clearly

Bulk Pickup Program How It Works

Important Note: If the bulk pickup is an option you need, you must contact Cleanlites DIRECTLY at 513-641-4155 before moving forward.

After talking with Cleanlites, they will instruct you to:• Fill out the order form (if you need gaylords and pallets you must fill it out

twice, once to order packing supplies and again to schedule a pickup)• Fill the e-waste into the box provided• Schedule the Pickup

Bulk Pickup Program

If this is a need you will have to contact Cleanlites directly at 513-641-4155.

This option is similar to the Mailback Program option but uses pallets, gaylords (above) and is designed for bulk quantities of e-waste.

If a Cleanlites truck is dispatched (within 100-200 mile radius), Cleanlites can provide gaylords, pallets and shrink-wrap. If a FedEx truck is dispatched (outside 200 mile radius) Cleanlites is unable to provide pallets and shrink-wrap, but they will be able to provide gaylords which come on a pallet.

Bulk Pickup Program• After contacting Cleanlites directly to arrange a bulk pickup, a representative will help you to understand

more specific packaging and ordering instructions.

• Kraft Offices will receive a gaylord box or fill one already at the facility. Once the box(es) have been filled with e-waste, an additional order form must be completed in order to get the material picked up.

• Fill the box(es) at your leisure. Once the gaylord(s) is/are full or any time you need to get rid of the E-Waste that has been accumulated, follow the instructions sent with the gaylord(s) and schedule a pickup. The gaylord must be shrink-wrapped and palletized.

• For the Bulk Pickup Program Option #2, in most cases for gaylord pick up large 18-wheeler Federal Express or Cleanlites trucks will pick-up. Both options will take the gaylord(s) at no charge and it will be mailed (FedEx) or delivered (Cleanlites truck) to Cleanlites. Whether or not FedEx truck or Cleanlites truck will be utilized will depend on a case by case basis. Cleanlites truck can only pick up within 100-200 miles of facility. If there are Unique situations, you will talk thru it with Cleanlites to determine the best course of action.

• Please let Cleanlites know whether there are constraints limiting truck access to reach your office (i.e., limited dock access). In most cases for gaylord pick up (Bulk Pick-up Option #2) large 18-wheeler Federal Express or Cleanlites trucks will pick-up (there is a section for this in the order form which we went over in depth already)

• For the “PO” Field please put “Kraft Office” so we know it’s not a manufacturing facility.

• Please fill out every piece of e-waste (quantity of each) you have and be as specific as possible this will help as we determine how many boxes to send

The “Other 1” box is where you include the need for asset serial recording. Serial # recording is provided at a charge ($5 per item). Requesting serial # recording should be restricted ONLY to sensitive data containing devices i.e. hard drives and tapes and should not be used without permission from Kraft IS.

Specific questions about electing the Serial Asset Recording option, should be directed to Kraft IS. Call the “Kraft Help Desk” at phone # 1-800 KRAFT-01 who will route your call to Kraft IS.

• The “Additional Containers Needed” section is where you can order mailback boxes, please note that this can be an estimate. (other items on this section are irrelevant to smaller offices i.e. Gaylords & E Waste Labels)

• Depending on the e-waste you have, Cleanlites rep will review this order form, talk with you and make an executive decision on the amount of boxes needed.

• If you are using the bulk pickup option and you need pallets or shrink wrap, please put this in “Other Items Needed” fees will apply. This would be atypical for remote locations.

Pickup Detail & Confirmation

• If you have special instructions for your facility or you think that a semi-truck will not be able to pick up from your facility please note that here (if you are using the mailback box a regular FedEx mail truck will be picking up so please do not worry about this.)

• Hit submit only once after you have filled out relevant information on form.

• If you hit submit by accident that’s ok just fill out the form again, the first form will be ignored.

A receipt confirmation will be sent to your email address and a copy will be sent to Kraft IS

What Happens Next?• After the material arrives at our facility, it will be processed; an invoice

and Certificate of Destruction (COD) will be generated.

• The invoice and COD will be sent to Kraft IS, and a copy will be emailed to the Kraft Office (to whoever fills out the order form).

• Kraft Office is to maintain/copy the invoice and COD onto the Kraft IS SharePoint site link listed below. https://collaboration.kraftfoods.com/sites/EWaste/sitepages/default.aspx

• Please use this naming convention when saving the file:

six digit date DDMMYY and the name of your office (example: “031215_rogers”).

Reference note:

COD is to be stored electronically and maintained for 5 years.

Minimum Amounts• Using the Mailback Program Option #1, there are no minimum

amounts needed to send to us.

• With both Mailback Program Option #1 and Bulk Pickup Program Option #2, we recommend you fill the box(es) as much as possible so as to avoid multiple half-full boxes and to maximize the shipment and minimize Kraft’s shipping costs.

• Do not overfill the box(es). The box(es) cannot be overfilled otherwise they will not be picked up. The box lid must be able to fit on the box.

• If you are unsure about weight or box size, please contact Cleanlites and we will help you determine.

A Note About Asset Recording• Kraft IS has asked that asset recording only be used on Hard

Drives or Tapes. There is no requirement for serial # asset recording unless you have an hard drive or a tape. (printers, monitors, keyboards, etc… should not be recorded).

• Requesting serial # recording should be restricted ONLY to sensitive data containing devices i.e. hard drives and tapes and should not be used without permission from Kraft IS.

all of these activities• Kraft Offices do not need to worry about payment

Billing and Certificates: Process• After processing, the Certificates of Destruction and shipping

receipt will be emailed to Kraft IS and to the Kraft Offices (to the person that completes the order form).

• It is expected that the Kraft Offices keep all this paperwork on file electronically. Please save onto the Kraft IS SharePoint site link listed below. https://collaboration.kraftfoods.com/sites/EWaste/sitepages/default.aspx

• Please use this naming convention when saving the file:
