DESCRIPTION

This presentation was delivered by Gillian Baird, a quality leader with the BC Patient Safety & Quality Council, at the kick-off event for CLeAR on October 9, 2013. The aim of CLeAR – our Call for Less Antipsychotics in Residential Care – is to achieve a reduction in the number of seniors in residential care on antipsychotic medications by 50% across BC by December 31, 2014 through a province-wide, voluntary initiative that supports participating sites. How Do Driver Diagrams Work?

• Primary Drivers: – These are key areas that research shows we need to

address in order to reach our goal.

• Secondary Drivers: – These are the actions we can take to successfully

implement primary drivers.

• Change Ideas: – One more step. These are suggestions that can help us

implement secondary drivers.

Team Meeting #2

Where to Start?

Page 11: Driver Diagrams: CLeAR Kick-Off Event

What idea or change are we going to try?

Who is responsible?

When? Where?

How will we measure its success?

Describe what actually happened. What did we observe that was not part of the plan? What were the problems and surprises.

What action are we going to take as a result of this cycle (Adopt, Adapt or Abandon)? Try with more people? Ready to implement? What are you going to do next?
