Within each organisation:● Describe the intervention and process of implementation● Contextualise it within wider strategies on LTC, engagement and
patient experience● Explore views on whether it has been a useful approach for
clinicians and their patients ● Explore views on whether it has resulted in any changes to
thinking or practice● Support local identification of relevant outcome data● Examine local evidence of impact in terms of health outcome
improvements and consequences for organisations
