Documentation Portfolio Assessment Plan
Purpose of the portfolio:
To evaluate the growth and progress of students' learning and to help studentsmonitor and reflect on their own learning by putting together in a portfolio the collectionof students work for the whole school year.
Specific Objectives
The portfolio intends to measure and to document
the overall knowledge of the learners to each particular lesson in Algebra 1, creativity of the students in presenting their work, the skills in organizing their works and, punctuality of the learners in submitting their works.

Procedure in developing the portfolio:
1. Identify teaching goals and objectives to be assessed through the portfolioand make it clear to the students.2. Specify the contents of the portfolio.3. Together with the students, identify significant or important outputs and
processes to be captured in the portfolio.4. Work together with the students, set and agree on the criteria to be applied in
the assessment of the portfolio.5. The criteria for selecting and assessing the portfolio contents must be clear to
the teacher and the students. It is a must that all students are to be informedabout the criteria by which work samples will be evaluated.
6. Give clear and detailed guidelines for portfolio presentation.
7. Provide an activity to be accomplished by the students for each lesson.8. Design scoring rubrics for each activity.9. Require students to collect and store work samples.
10. Before the end of the school year, ask the students to compile their works in adocumentation portfolio.
11. Ask the students to reflect on their own progress in relation to the teachinggoals set at the start. This will be their reflection.
12. Evaluate the students portfolio based on the agreed set of criteria.
13. Assess each portfolio entry with reference to its specific goals.
14. Teacher will provide feedback on the portfolios. Write a letter about the portfolio which details strengths and
weaknesses and generates a profile of a students ability, which is thenadded to the portfolio.
Prepare certificates which comment on the portfolio strengths andsuggest future goals.
