PROCLAMATION WHEREAS, the Greene County Department of Human Services relies upon volunteers to provide and assist with vital services to the elderly residents of Greene County; and
WHEREAS, volunteers are crucial to many of the department’s Aging services, including home-delivered meals and transportation for medical appointments, and
WHEREAS, it has long been a tradition for the county RSVP volunteers to perform work of a high quality and to brighten the lives of others while making Greene County a better place in which to live by serving in community locations; and
WHEREAS, volunteers give freely of their energy, time, ability and experience; and
WHEREAS, the week of April 15 – April 21, 2018 is recognized throughout the United States as National Volunteer Week
NOW THEREFORE, we, the members of the Greene County Legislature, do hereby proclaim the month of April as:

AND FURTHER, with great pleasure, we honor these volunteers and convey the sincere gratitude and appreciation of all our citizens for the dedicated, selfless and compassionate efforts of these volunteers.

SHOPPING BUS The Greene County Department of Human Services offers a shopping bus to Greene County residents, age 60 or
older, living in the towns of Ashland, Athens, Cairo, Catskill, Coxsackie, Greenville, Hunter, Jewett, Prattsville and
Windham. Seniors are picked up at their door, driven to Catskill for shopping, and then have lunch at a local Senior
Center before returning home. Special trips on a periodic basis.

EXECUTIVE DIRECTOR
The department is contemplating a few changes to our transportation programming. I wanted to start releasing the news now, so that when we implement the changes, it won’t take anyone by surprise. As it now stands, we only offer individuals who need a wheelchair, rides on the shopping bus. We are working on a plan that would allow us to provide wheelchair transportation to those individuals who would like to go the Senior Centers for meals. We are, also, working on providing transportation for medical appointments to those individuals who need a wheelchair. A few things still have to be ironed out in order for this to become a reality. Stay tuned for further developments.
Who is RSVP? A federal initiative of the Corporation for National &
Community Service, sponsored locally by Greene County Department
of Human Services.
What does RSVP do? Recruits people 55 years & older, ignites their
will to serve & channel their passions to meet the priority needs of the
community.
Why does RSVP exist? It’s as simple as it is profound, we encourage
& foster development & maintenance of volunteer opportunities that
engage, support & enhance the lives of fellow Americans.
Where RSVP can be found? In local communities throughout the
nation.
How does RSVP do it? By identifying critical needs in the community
& partnering with agencies to carry out their mission utilizing senior
volunteers.
URGENT NEED FOR
MEDICAL TRANSPORTATION
VOLUNTEER DRIVERS
Medical Transportation Volunteers must:
possess a valid driver’s license
possess a reliable vehicle with valid insurance
be a registered volunteer with RSVP
complete Medical Transportation Volunteer training
Needed to drive homebound senior citizens to medical appointments within Greene County, and neighboring
counties.
Volunteers can request mileage reimbursement, at the rate of 50 cents a mile, for all miles driven during
transport.
Volunteers get advanced notice for all medical transportation appointments.

SENIOR ANGELS PROGRAM -
SPREADING CHEER 365 DAYS A YEAR
The Greene County Senior Angels Program operates
more than just at holiday time. Under their slogan,
“Spreading Cheer 365 Days a Year”, funds are used
year-round to provide one-time, emergency assistance to
senior citizens, when all other funds have been
exhausted, and there is no other source to help.
Due to the generous contributions of area residents,
business and clubs, the Program has been able to help
fulfill all qualified requests. Year-round donations can

Welcome back to our Caregiver Connection. We hope you find the information beneficial to you as family
caregivers. As you read, you’ll come across terms commonly used in Human Services & Health Care settings that
you may find confusing or have never heard before. Each month, we will offer an alphabetical list of terms with
simple explanations.
H - M
HIPAA - Health Insurance Portability and Accountability Act (HIPAA) is a federal law designed to protect private
medical information from unauthorized users (people who should not see this information, such as employers or
marketers). HIPAA requires health care providers to keep this information safe and secure. The law does not prevent
providers from sharing needed information with family caregivers.
Home-delivered Meals - Often referred to as Meals on Wheels. These are meals brought to people who cannot
prepare their own meals or are homebound and unable to leave their residences.
Home Health Care - Services given to patients, at home, by various workers who are trained to provide health care
tasks for those who are elderly or disabled. This may be nurses, therapists, aides, or other trained workers. Insurance
plans have guidelines for which home care services they will cover, under what circumstances (medical needs), and
for how long. Certified home health agencies often provide and coordinate these services.
Hospice - A program of medical and social services for people diagnosed with terminal (end-stage) illnesses. Hospice
services can be given at home, in a hospital, hospice residence, or nursing home. They are designed to help both the
patient and his/her family. Hospice care stresses pain control and symptom management. It also offers emotional and
spiritual support. Medicare will pay for hospice if a doctor states that a person probably has 6 months or less to live.
Hospice care can last longer than six months in some cases.
Incontinence - Loss of bladder or bowel movement control.
Informed Consent - Legal term meaning that a person with mental capacity has given permission for medical
treatment. Consent comes after a full disclosure of treatment risks and options.
Living Will - A type of advance directive that outlines a person’s treatment wishes if he or she is too ill or injured to
make such choices. Laws about living wills vary from state to state.
Long Term Care (LTC) - A term used to describe the care needed by someone who must depend on others for help
with daily needs. LTC is designed to help people with chronic health problems or dementia live as independently as
possible. While many people think that long-term care happens only in a nursing home, in fact most long-term care is
given by family caregivers in the patient’s home. Private insurance designed to pay for the expenses associated with
LTC in a facility other than a hospital is called Long Term Care Insurance. Each plan has a different range of
benefits. Medicare, Medicare supplemental insurance (Medigap) and “regular” health insurance do not pay for this
type of care.
Managed Care Plans - A type of health insurance plan, either Health Maintenance Organizations (HMO) or
Medicare Advantage plans. Managed Care Plans offer incentives, such as lower co-pays to beneficiaries for using
medical facilities and providers with whom they have contracted for reduced (“in-network”) costs.

1. Denial about the disease and its effect on the person who has been diagnosed.

MEDICARE ADVANTAGE (MA) PLANS
HMO (Health Maintenance Organization)
HMOs are the most basic and restrictive type of MA plan, in that they only provide coverage for non-emergency care
within a network of providers. Plan members are required to choose a primary care physician (PCP) and may need to
get referrals to see specialists. HMO plan members that elect to have Part D must receive Part D coverage as part of
that specific plan; they cannot enroll in a separate stand-alone Part D plan (PDP).
HMO-POS (Point of Service)
HMO-POS adds limited out-of-network coverage to the HMO plan. The out-of-network coverage may only include
certain types of services and may have a limited amount of coverage. Plan members are required to choose a primary
care physician (PCP) and may need to get referrals to see specialists. HMO-POS plan members that elect to have Part
D must receive Part D coverage as part of that specific plan; they cannot enroll in a separate stand-alone Part D plan
(PDP).
PPO (Preferred Provider Organization)
PPOs add more comprehensive out-of-network coverage, in addition to their in-network coverage. PPOs provide
coverage for all Medicare covered services out-of-network. PPO plan members are not required to choose a primary
care physician (PCP) and do not need to get referrals to see specialists. PPO plan members that elect to have Part D
must receive Part D coverage as part of that specific plan; they cannot enroll in a separate stand-alone Part D plan
(PDP).
PFFS (Private Fee for Service)
PFFS plans are similar to PPO plans in that they provide both in and out of network coverage. Like PPO plan
members, PFFS plan members are not required to choose a primary care physician (PCP) and do not need to get
referrals to see specialists. However, unlike HMO and PPO plans, PFFS plan members that elect to have Part D can
enroll in a separate stand-alone Part D plan (PDP) if they choose a PFFS plan without Part D.
SNP (Special Needs Plan)
SNPs are a type of Medicare Advantage plan that is only available for certain Medicare beneficiaries. Most SNPs in
NYS are limited to beneficiaries that are dual-eligible (Medicare and Medicaid) but some SNPs are limited to
beneficiaries that are institutionalized while other SNPs are only for beneficiaries that have certain chronic conditions.
All SNP plans in NYS are HMO type plans which include the features of standard HMO plans.
VITAMIN D, THE MUST-HAVE FOR SENIORS Vitamin D is produced by our bodies in response to sunlight. It is extremely beneficial
for bone strength and helps the body absorb calcium. Without Vitamin D the body may
lose bone tissue resulting in bone pain, muscle weakness and possible skeletal
deformity.
Why is this deficiency so common among seniors?
Decreased dietary intake of Vitamin D
Less exposure to sunlight
Reduced skin thickness
Impaired intestinal absorption
Reasons why we need Vitamin D:
Prevent falls and fractures
Lower risk for osteoporosis and other bone disorders
Maintain physical mobility and independence
Decreased risk of cardiovascular problems, diabetes and some cancers
Seniors over the age of 71 should get 800 IU of Vitamin D daily. Your doctor can help you determine whether you
are getting the right amount by a simple blood test and can recommend a supplement. Be sure to always speak with
your doctor first!
Ways to increase Vitamin D with food:
Tuna fish
Swordfish
Fortified foods such as: milk; cereals; orange juice
TUNA BURGER An easy recipe to increase your Vitamin D
1/2 cup bread crumbs 2 tbsp. pepper sauce
2 tsp mustard 1 medium egg
2 cans tuna, drained 1 dash of pepper
1 tbsp. olive oil
Mix all ingredients, except oil, in a bowl. Season to taste with pepper.
Divide mixture into four balls, and flatten into patties.
Heat olive oil in a non-stick pan over medium heat. Cook until nicely browned, about 3 - 4 minutes per side.
Calories 330, Carbohydrates 33 grams, Fat 6 grams, Protein 36 grams, Sodium 0, Vitamin D 43.9 International Units
SENIOR SERVICE CENTERS in GREENE COUNTY
Each site serves the noon-time menu item for the day, Monday - Friday, for a suggested
donation of $4.00. All meals include: Meat/Alternative Entrée, Vegetables & Fruit, Bread,
Milk & Dessert. We ask that you call at least one day ahead, if you wish to be included in the
lunch count. Centers are closed on legal holidays and inclement weather. In addition, each
center offers Aging Information & Assistance/Medicare Minute each month.

were treated to a delicious dinner for St. Patrick’s Day. The menu consisted of Corned Beef, Steamed Cabbage,
Boiled Red Potato, Carrots, and Irish Cream Trifle. At the Rivertown Center in Athens, seniors were even treated to
music by Mike Siciliano.

GREENE COUNTY DEPARTMENT of HUMAN SERVICES CONTRIBUTIONS POLICY
For services under the Older Americans Act and in Community Services for the Elderly which propose contributions,
such contributions must be used for costs allowable under applicable regulations and incurred during the budget
period that contributions were received. The suggested contributions will not be more than the actual cost of the
service. The purpose is to offer participants the opportunity to contribute and to increase the units of services,
particularly to those in greatest need. Envelopes are provided to participants that desire to make voluntary and
confidential contributions.
Services will not be denied to anyone unable or unwilling to make a contribution.
CONSULTATION and ASSISTANCE: This includes such client assistance activities as case assistance,
HIICAP, help with public benefit applications, etc.
The suggested level is $5.00 per office visit (Actual cost $20.00/hour).
IN-HOME SERVICES: This is discussed with the client by the case manager at the time of assessment.
The suggested level of contribution is $3.00 per hour (Actual cost $21.56/hour).
LEGAL SERVICES: The suggested level of contribution is $10.00 per attorney consultation (Actual cost $149.00/hour).
NUTRITION: Senior Congregate Meal or Home Delivered Meals: The suggested level of contribution is $4.00 (Actual cost $12.01/meal).
Nutrition Counseling:
Clients requesting diet counseling from the Registered Dietitian are provided with an opportunity to contribute.
The suggested level of contribution is $5.00. (Actual cost $65.00/hour).
Health Promotions: There is no suggested contribution for this service. Donations always accepted.
TRANSPORTATION:
Shopping Bus: This door-to-door service is provided directly by the Department.

Are you unemployed, age 55+ and looking for work? Associates for Training and Development (A4TD) adminis-
ters the Senior Community Service Employment Program (SCSEP), which helps mature people get back into the
workforce by providing paid occupational skills and workplace experience training.
Designed specifically for income-qualified workers age 55+, the purpose of the SCSEP is to provide training
and skill-building opportunities to program participants while providing much needed community service to local non
-profits and public agencies. Some examples of training sites include: hospitals, libraries, senior centers, schools,
town clerks, museums and more. These agencies provide internship opportunities where program participants learn
updated job skills. In addition to on-site training, each participant receives career assessments, technology skills
updates, supportive services referrals, and career counseling.
If you are looking to train for available jobs, learn updated job search skills and connect with local employers, we can
help.
