• Certificate of completion must be submitted before the
season begins – no exceptions
Concussion Policy and Procedure
• If a player is removed from a practice or a game and your coach
suspects a concussion:
1. Coach is to promptly inform parents and ask them to observe
player behavior and look for warning signs
2. Player will be held out of practice and games until medical
clearance is received
3. Coach will report player’s name to Coordinator and Kristen Gaebel,
board member supervising our concussion policy
4. Kristen will contact parents via phone and e-mail and remind them
that medical clearance will be needed for player to return
5. Parents are to get medical clearance to Kristen and she will then
clear the player to return to active participation, via e-mail to
Coordinator and coach
6. Coaches are encouraged to following the full CDC guidelines
regarding a player returning to practice/games
Code of Conduct
• Every Player, Parent and
Coach will receive and be
asked to sign the League
Code of Conduct.
• Please take the time to review
it and reflect on the positive
example you can set.
• Review it with your son(s) –
have a meaningful
conversation on how we all
should carry ourselves.
• Honor the game, and be
respectful.
We need your help!
• Parent/Team Manager for each team
• Communicate games and practices to team parents
• Manage team schedule on team website
• Confirm games and field locations; confirm referees
for home games
• Enter scores on league website
• JRs and SRs – need Parent to run the clock for each
home game
