DH 2013, Care in local communities: A new vision and model for district nursing supports the empowerment of patients and sharing information with them to achieve improved outcomes
Royal College of GPs & Royal College of Nursing are driving changes forward to support the agenda
Patient Groups want change and many are proactive about moving the agenda forward
What are the Challenges?
Moving Forward and not allowing the agenda to become side tracked or derailed
Preparing the health care workforce to support empowered patients and embedding concepts in curricula
Preparing patients to work in partnership with health professionals and to manage information
The Role of Nurses
Nurses have a major role in supporting the estimated 15 million patients in the UK with long term conditions
The empowered patient can present challenges for nurses and nurses need preparing appropriately to facilitate this
6Cs: Care, Compassion, Communication, Courage, Competency, Commitment are fundamental values in nursing and partnership working is at the heart of all of them
