Conclusion: You Can’t Manage what You Can’t Measure
• Measurement is the foundation for improvement.
• Make measurement and data collection as efficient as possible.
• Monitor results using a dashboard that all can see.
• Report results and look for best practices.
Questions to Consider when Reviewing Run Charts
• When we will feel good about the care we are delivering? What is our goal?
• What are the data telling us? What do these data say to the staff?– How confident are we that the data accurately reflect care being provided?
– Do we care about this measure? If so, why?
– All over the map? Look at the denominator size.
– Steady increase? What did we do that worked?
– Decrease? What changed?
• What can we do to make it better?– Improved population management
– Improved patient engagement, e.g. self-management support
– Improved follow-up with patients, e.g., care management
• Is the way that we are displaying the data helpful?

Managing Lipids for our CAD Patients
Coach: Insert run charts that show the value of reviewing multiple measures at the same time. For example, when assessing LDL levels, look at both the % of CAD patients with lipid profiles ordered and % of patients with LDL<100.
