Any offer not received as per terms and conditions of the tender inquiry is liable to be
ignored. No offer shall be considered if:-
a) The tender is un-signed. b) It is received after the time and date fixed for its receipt. c) The offer is ambiguous. d) The offer is conditional. e) The offer is from a firm black-listed/suspended by the Government. f) The offer received is of shorter validity than required in the tender inquiry. g) The offer is received through Telegram/Fax. h) The offer is for stores not conforming to specifications indicated in the Tender Inquiry.
2. The offer will remain valid up-till 30th June 2016 from the date of opening of Tender and
extendable up to three months. 3. All the tenders must be submitted in double cover. The outer cover should indicate the name
of addressee. Tender No. & date of opening, but not the name of the firm which must appear on the inner envelop clearly mentioning tender No. & name of the firm. Both envelops must be properly sealed. There should be two separate sealed offers inside envelope one Financial offers and the other offer clearly specifying Technical specifications and other documents (as per PPRA rules 2014). The combine offer found in one envelop shall be rejected and returned at the spot.
4. The department reserves the right to purchase in bulk or in parts or to ignore/scrap the tender/quotation without assigning any reason to the parties concerned.
5. If the successful bidders fails to supply the goods as per Orders/Terms and Conditions within the stipulated period times of the contract or they try to withdraw/amend/revise their offers within the validity period, the offer shall stand cancelled, the earnest money /call deposit/security deposited will be forfeited & the relevant goods will be purchased at the risk & cost of the bidder.
6. The Department reserves the right to claim compensation for the losses caused by delay in the delivery of stores as per rules.
7. Delivery of the goods at the consignee end will be the responsibility of the supplier and the department will pay no extra charges in this regard.
8. Cash receipt (in-original) or photocopy having purchased the tender form must accompany with the offer.
9. Tender should be addressed to Executive District officer (Health) Attock. 10. Tenderers should quote their firms final rates both in words and figures. 11. Tenderers should specifically indicate their circle of the Income Tax Department along with
documentary evidences. 12. The persons signing the tender must properly sign any cutting/crossing etc appearing in the
offer. Moreover all pages to the tender must also be properly signed. Offer with any over writing/cutting shall in no circumstances be accepted.
13. The successful bidders shall abide all the terms & conditions of the contract. 14. The tender should be submitted on the basis on accounting unit specified in the
quotation/tender.
15. The successful tenderer may be required to furnish Performance Guarantee 5% in the favor of EDO (Health) Attock or Purchaser within the period specified in advance acceptance of tender, failing which purchase shall be made at their risk and expenses.
16. If the tender is accepted, supplier will have to deliver the medicines at Office of the Executive District Officer Health Attock/Office of concerned Drawing and Disbursing Officer within 60 days. However authority is competent to extend supply period if reasons are given genuinely, otherwise 2% per month late delivery charge can be imposed by the Executive District Officer Health Attock and Chairman standing Purchase Committee.
17. The supply will be accepted subject to the prior approval of the purchase committee. 18. The quoted price should be inclusive of sales tax as prescribed by the Government. 19. The Income Tax will be deducted at source as per Government rules. 20. Specification & size, make by manufactured should be clearly mentioned in the Tender
along with Book let etc. 21. The Quantity of Medicines can be decreased and increased according to budget. 22. Tender samples where needed should be submitted by the tenderer along with their offers.
Bulky samples should be delivered at the time of opening of the tenders, failing which the offer shall not be entertained.
23. The delivery of medicines will be in one lot, piecemeal, split supply will not be accepted in routine, in case of sound reasons authority can accept the part supply.
24. The Bidder shall provide samples of quoted medicines along with the bids.
25. The locally manufactured and locally imported drugs and medicines would be strictly in accordance with the GREEN COLOUR and packing to be prescribed by the DISTRICT GOVERNMENT and the following words should be marked with bold letter (English & Urdu) on each ampoule/vial/strip/sachet/tube etc in additional to the outer packing. “DISTRICT GOVERNMENT ATTOCK PROPERTY NOT FOR SALE.”
26. The supplying firm will be responsible for expiry date of item and in case of declaring any item of Medicines substandard by DTL, the supplier is bound to replace the said item. The supplied medicines should be accepted/rejected as per as per report of DTL.
27. The shelf life of all store should be 80 % for imported items & 90% in case of local manufacture item at the time of delivery.
28. Bid Security 2% of the Quoted Item’s in the shape of fresh CDR/Bank Guarantee issued by any schedule bank of Pakistan in the name of Executive District Officer Health Attock, (to be attached with Technical Offer).
