Use of Sign-In Sheet (see attached). Please complete the sign-in sheet prior to submitting
to the staff who enter the data into parent engagement tracker.
1. Header: Insert name of activity to identify what the topic is.
a. For example, ELA: In-Class Reading Support
2. Facilitator: Insert name of person(s) and/or people conducting activity in-case there
are questions from the front liner who is entering data. For example, Teacher Name, Staff
Name, etc.
3. Place/Room: Insert location of activity – this will assist with facilitation of activities
4. Description (IMPORTANT): Be clear about describing what is happening at the
activity. For example. Focus to support literacy via 20 minute group reading session.
5. Activities Type: Identify type of activity.
Event = targeted at large group functions or touchpoints with families
Volunteer = providing in-kind hours without payment
Training/Workshop = providing home-school connection opportunities to learn skills
ie. Providing a newsletter to parents highlighting ways families can assist with
classroom instructional support;
Committee = participating in on committee’s to support academic success ie. PTO
Board Meetings, School-Governance Council’s, Family Engagement Committees, etc.
