Parastomale hernias

• Is een erg frequente complicatie van het

plaatsen van een stoma

• Soorten stomata:

– Colostoma

– Ileostoma

– « Ileal conduit » na Bricker cystectomie

• Vaak geeft een kleine peristomale hernia

weinig problemen

• Meestal wordt een afwachtende houding

aangenomen bij mensen met een

peristomale hernia

• Traditioneel heeft het heelkundig herstel van

een peristomale hernia hoge recidief

percentages

• Bijgevolg is het gebruikelijk een herstel

enkel aan te raden bij ernstige problemen

– Pijnklachten

– Omvang van het stoma

– Inklemmingsverschijnselen

– Moeilijkheden met efficientie applicatie van

stomamateriaal

Een parastomale hernia is een abnormale uitstulping van

de inhoud van de buikholte of van pre-peritoneaal vet

doorheen het buikwanddefect gemaakt tijdens het

aanleggen van een colostoma, een ileostoma of een

Bricker stoma.

The laparoscopic modified Sugarbaker technique is

safe and has a low recurrence rate:

a multicentre cohort study.

• 61 consecutive patients (2005-2010)

• 55 colostomy

• 4 ileostomy

• 2 ileal conduit (Bricker)

• Laparoscopic modified Sugarbaker with ePTFE mesh

• Morbidity 19 %

• Recurrence rate (mean follow up 26 months) 6.6%

Sometimes all attemps to classify or

standardize treatment will fail.
