Example Homework Slip Directions: Before completing tonight’s homework, fill out the first two questions. Once you have finished the homework assignment fill out the last three questions.

After: 3. How do you feel now that you have completed the assignment? 4. How long did it take you? 5. Did anything to distract you, and if so, what was it? (texting, phone calls, Internet, video games, family, pets, etc.)
