- Introduction narrative addressing Mix Design Methodology, recommendations, concerns, etc. Document shall be signed by individual responsible for technical content for the design.

- Identification of all Materials Sources - Coarse and Fine Aggregate Gradations and Plots, Curves - If Applicable, Combined Aggregate Gradation (USAF Gradation) - If Applicable, Coarseness/Workability Plots (USAF Gradation) - Coarse Aggregate Quality Test Results Including

- Deleterious Materials - ASR Data - Soundness - Los Angeles Abrasion - Fractured Faces - Flat and Elongated Pieces - If Applicable, Freeze / Thaw Data - Specific Gravity for Each Coarse Aggregate Size

- Fine Aggregate Quality Test Results including - Deleterious Materials - ASR Test Data - If Applicable, Freeze / Thaw Data - Sand Equivalency - Fineness Modulus - Specific Gravity for Each Fine Aggregate Size
