    Dental Facility Considerations

    The construction of a new dental clinic requires careful planning to ensure that the

    resulting facility is functional and a pleasant place in which to work. ithout proper

    planning! a myriad of ma"or and minor pro#lems can occur during the construction phase.

    $ental clinics #uilt #y the Indian Health Service %IHS& undergo an e'tensive planning

    process using standardi(ed designs to provide uniformity. $ental clinics #uilt #y Tri#aland )r#an Indian programs! on the other hand! display a wide variety of designs! #ecause

    these programs have more fle'i#ility when choosing clinic designs. *ot #eing #ound #y

    standard designs provides some o#vious advantages! #ut in some cases architects hired #yTri#es or )r#an programs have little or no e'perience in designing dental clinics.

    hether new clinics are #eing constructed #y the IHS! Tri#es! or )r#an %I+T+)& Indian

    programs! the planning phase should include input from sources that are well,versed inthe #uilding of dental facilities.

    The purpose of this su#section is to provide some general recommendations regarding

    dental clinic design and dental equipment selection which should #e helpful to programsthat are planning new dental clinics.

    General Recommendations

    hether the clinic is #eing #uilt #y the IHS! a Tri#e! or an )r#an Indian

    program! it is a good idea to consult the Safety *et $ental -linic anual

    at www.dentalclinicmanual.com.

    /ollowing are some of the types of information provided #y the facilities planning

    manual0

    $ental clinic designs for facilities of various si(es.

    1quipment lists! #y room! for real property %furnished when a #uilding is

    #uilt and usually consisting of large items which are #olted to the wall or

    floor& and for ma"or unattached equipment such as dental chairs!

    autoclaves! film processors! and handpieces. $isposa#le items and handinstruments are not included in these master lists.

    2m#ient light recommendations.

    Safety codes0 3isit other I+T+) dental clinics. $on4t rule out any possi#ilities!

    including military installations and veterans hospitals! if they are

    the 2rea /acilities Planning Officer.

    eet with representatives from dental supply houses. They can

    provide useful information a#out clinic design! even though theirprimary "o# is to sell equipment.

    )se the toll,free num#er of equipment manufacturers to get advice

    regarding facility requirements for the equipment that you intend to

    place in the new clinic.

    O#tain and use government price lists for equipment. Having the

    catalogs on hand will facilitate the ordering of equipment #y allowing

    for comparison of prices and features.

    Get actively involved in the pro"ect. ost construction pro"ects have a

    #etter result if the dentists and other dental staff at the facility have

    direct input at the design stage. If an architect or company

    representative has made an o#vious error! such as forgetting to include

    a sink in the la#oratory! dental staff mem#ers should #e a#le to detectthe error.

    )se manufacturer4s templates and si(e guidelines. If a certain amount

    of floor space is required! avoid the temptation to get #y with less.

    /ollow the manufacturer4s instructions:

    ake every operatory identical! with the e'ception of quiet rooms

    and+or operatories used for sedative procedures %these will require

    increased space for personnel and monitoring&. This will help to

    eliminate the pro#lem of providers using ;favorite rooms< the ma"orityof the time! which can have a negative effect on dental clinic

    efficiency. If possi#le! have windows in the operatories to facilitate the selection

    of tooth shades and to provide a pleasant atmosphere for patients andstaff. *orth,facing windows are #est! and east,facing are second,#est.

    Plan for proper air e'change in each operatory. It should #e driven

    with its own special system. -onsult the Safety *et $ental -linic

    Provide adequate overhead light in the operatories to prevent eyestrain.

    Provide vacuum and air systems with their own e'haust+intake lines.

    The trap for the vacuum system should #e easily accessi#le for

    cleaning. The discharge point for the e'haust should not #e near afresh air inlet or open windows! and it should not discharge into asupply room or other occupied place.

    Provide each operatory with two sinks.

    Provide each operatory with its own ',ray machine.

    Provide plenty of electrical power for each operatory for future

    e'pansion.

    Provide each operatory with telephone and data "acks.

    Provide a staff changing area! with lockers! within the dental clinic

    area. This will facilitate the separation of clothing used to treat patients

    from clothing that is worn outside of the clinic! which is important forinfection control.

    Delivery Systems

    The dental delivery system traditionally used #y the IHS is the rear,delivery system.

    hile it has proven to #e an economical! functional system! the rear,delivery method

    does have some shortcomings. 2s a result! many Tri#al and )r#an programs and someIHS programs recently have chosen alternative delivery systems. /ollowing is a

    Equipment Considerations

    hen selecting dental equipment! research the manufacturer of the #rand of equipment

    you are considering to determine %6& the sta#ility of the company and %7& if they provide

    good customer support for their equipment. 2lso ask dentists and dental equipment repairservices a#out the relia#ility and dura#ility of the #rand of equipment you are

    considering.

    This radiograph shows the entire head and is taken using a panoramic

    machine with special attachments and may #e conventional or digitally

    produced. Oral surgeons and orthodontists mostly use this type of ,ray. This type of radiograph is nice to have #ut not required unless

    your clinic has an orthodontic program.

    1'posure of photographic film to ,rays and then chemically develop

    the film in a developer in a darkroom or via a day,light loader4

    -osts of conventional developing are cheaper in the short run! #ut

    increasing due to cost of chemical handling.

    /ilms are archiva#le! #ut require physical space to store radiographs

    $igital Fadiography0

    Images are captured electronically via a charge couple device %--$&

    or via a phosphor plate system after e'posure to ,rays.

    These images are then uploaded! viewed! and stored on a computer.

    multi,provider medical and dental offices to alert the providers when a

    patient is in a treatment area or when they have a phone call or message.

    2n internal paging or intercom system also may #e considered.

    Intercom systems can #e installed as stand,alone system or as part of theclinic?s phone system! decide which type will #e used #efore a phone

    system is purchased and installed. Eocal dentists or local group practices

    may #e a#le to help you decide which system will #est suit your needs.
